Asthma is chronic inflammatory disease of airways. Prevalence of asthma and prevalence of vitamin D insufficiency is constantly rising in past decades. According to the numerous study results we can hypothesize that vitamin D insufficiency can be important contributor to pathogenesis, stage, disease control and therapeutic response in asthma. Vitamin D has antimicrobial properties as well, because of its influence on catelicidin, human peptide produced by neutrophils, macrophages, skin, respiratory an digestive tract cells with wide antimicrobial activity (Gram-positive and Gram-negative bacteria, some viruses and fungus) (18). Pavlovska","doi":"10.5937/medi55-37222","DOIUrl":"https://doi.org/10.5937/medi55-37222","url":null,"abstract":"Leser-Trélat (LT) sign (syndrome) is a rare, distinctive clinical phenomenon that is manifested by a sudden, eruptive, appearance of multiple itchy seborrheic keratoses (SK) that sometimes coincide with occult malignancy. A 73-year-old patient came for examination of numerous, large seborrheic keratoses (SK) on the skin. According to the patient's statement, SK did not occur suddenly and \"eruptively\". Nevertheless, ultrasound and endoscopic evaluation were suggested to the patient to exclude the Leser-Trélat phenomenon. The patient was then diagnosed with an occult, asymptomatic rectal neoplasm. The occurrence of numerous seborrheic keratoses (especially if they are \"eruptive\", large and bizarre in shape), should raise suspicion of LT phenomenon, or internal malignancy in the patient, and result in relevant diagnostic procedures to detect possible latent malignancy. Anti-measles vaccination contributed to a decreasing number of SSPE patients, but not to its eradication. The aim of our study is to evaluate the course of the disease in our SSPE patients with a focus on vaccinated children. The main goal is considering possibilities for improving prevention of the disease. Methods: A retrospective study included the patients with SSPE treated in the period from December 2010 to December 2020 at the Pediatric Clinic of the Institute. The inclusion criteria were the patients with the diagnosis of SSPE based on clinical presentation, neuroimaging, electroencephalography and positive IgG anti-measles antibodies, both in serum and CSF. Results: Five children with fulminant course of SSPE were included. All these patients were suffering from measles at an early age. Three of them had been vaccinated against measles and two had not. All of them had previously been healthy, immune-competent children, with normal general development. The course was extremely fulminant with lethal outcome within three months since the initial symptoms in four cases. Progressive motor and cognitive decline, behavior changes, movement disorders, myoclonic jerks and seizures were dominant in clinical presentation. Conclusion: Despite vaccination, SSPE has not been eradicated. An increasing number of vaccinated immune-competent children with fulminant form of SSPE and history of measles infection at an early age were treated at our Clinic. Nikolic","doi":"10.5937/medist1703001m","DOIUrl":"https://doi.org/10.5937/medist1703001m","url":null,"abstract":"Bacterial infections in liver cirrhosis are very common and currently represent one of the most important causes of progression of liver disease, repeated hospitalizations and leading cause of death. The incidence of bacterial infections is 4-5 times higher in patients with cirrhosis compared to the general population, while mortality is 3.75 times higher in patients who develop bacterial infection. Spontaneous bacterial peritonitis, urinary infection, pneumonia, skin and soft tissue infections, and bacteremia are the most common bacterial infections, and E. coli, Klebsiella pneumoniae and Enterobacteriaceae are among the most common causes of these. In recent years, research has shown that increasingly frequent bacterial infections are present in patients with cirrhosis of multi-drug resistant bacteria (MDRs) such as Enterobacteriaceae extended spectrum that produce b-lactamase (ESBL), non-fermentable gram negative bacilli such as Pseudomonas aeruginosa , Stenotrophomonas maltophilia, Acinetobacter baumanii, Meticillin-resistant Staphylococcus aureus (MRSA), Vancomycin-sensitive or resistant Enterococci (VSE, VRE). While supervision is clearly a rewarding duty, it is also associated with a number of challenges. It is clear that supervision involves many responsibilities, which means that the supervisor needs to possess a certain set of skills. While many of these skills are learned in the direct interaction with PhD candidates, it would be beneficial if a more formal and structured training was offered to supervisors, especially at the beginning of their supervision careers, so as to avoid frequent mistakes in the supervisor-PhD candidate relationship. Hence, the idea is to shift from experience-based supervision to professionalization, believing that such an approach would reduce the risk of poor supervision. At present, many universities offer formal education of supervisors. The practice of organized education of supervisors at University of Belgrade Faculty of Medicine started in 2019 with the aim of preparing future supervisors to establish productive and responsive relationships with PhD candidates. After a break due to the COVID-19 pandemic, a revised supervision course was organized in 2022. The survey conducted among the attendees of the two organized courses supported the need for better education of university teachers in terms of supervision skills and leading of PhD candidates. The protagonists of neuroteology believe that mystical and religious states can be voluntarily or involuntarily caused by manipulation of the structures of the limbic system, including the hippocampus, hypothalamus, amygdala and right frontal and temporal lobe. Neuroteology studies are quite intriguing and act as an important link in determining the position of the spiritual in man. It inspires us to think intensively about the complexity of the relationship between religion and the brain. Đurić","doi":"10.5937/medi56-44450","DOIUrl":"https://doi.org/10.5937/medi56-44450","url":null,"abstract":"With the passage of time and the lack of quality archival materials, both at the Institute and at the Faculty of Medicine and bearing in mind the \"culture of memory\", the work on this text, which is a continuation of the work on the published monograph on the occasion of the 100th anniversary of the establishment of the Institute of Medical Physiology of the Faculty of Medicine of the University of Belgrade and the beginning of classes, was an extremely challenging undertaking. The texts previously published by fellow teachers in faculty publications on various anniversaries, as well as in the \"Serbian Archive\" journal, significantly helped the realization of this undertaking. The willingness and kindness in providing important historiographical material, documents and photographs, by the family members of Professor Richard A. Burian from the USA (Richard M. Burian, Jim Huntington), Milutin Nešković from France and Belgrade (Nenad and Nikola Nešković, respectively) and Božidar Nikolić (Emilija Nikolić Đorić) should be appreciated, and then by numerous colleagues from our country, including Vojislav Voki Kostić, son of the famous professors Aleksandar Đ. Kostić and Smilja Joksić-Kostić. I owe everyone a big thank you, because they made it possible to systematize these important data. The history of intermittent symptoms of cerebrovascular insufficiency, which may be provoked by rotatory movements of the head and the neck, should raise suspicion of the presence of carotid artery redundancy. On the basis of the clinical features and modern diagnostic procedures, we can determine the hemodynamic significance of kinking and coiling. Maximal systolic velocity, measured by color duplex ultrasonography, is an important parameter for reaching the diagnosis. Arteriography, CT or classic, is reserved only for symptomatic cases of kinking and coiling. Although the largest amount of morphological information is provided by intra-arterial digital subtraction arteriography, the number of surgical decisions made on the basis of non-invasive tests has increased. Marić","doi":"10.5937/medist1701009p","DOIUrl":"https://doi.org/10.5937/medist1701009p","url":null,"abstract":"Aspiration-inhalation of solid or liquid materials into the airways-occurs when solids or liquids are introduced into the supraglottic area rapidly or in large volumes or when airway defences are reduced by structural abnormalities or neurological impairment. Tracheobronchial aspiration (TBA) syndrome is currently among leading causes of morbidity and mortality in the world when it comes to accidents, and it is the leading cause in one-year-old children. During the last several years, destructive effects of some food and drugs on respiratory system have been revealed. TBA may cause no complications at all or may result in a range of complications from paroxysmal cough to respiratory failure and lethal outcome. Rapid diagnosis is of utmost importance in correct interventions that should be done in a timely manner, and in prevention of complications, which are sometimes irreversible. This is a review of TBA syndrome etiopathogenesis with special reference to those caused by solid foreign bodies, including those of biological origin, hydrocarbons, lipids, and water-drowning. Special attention is given to TBA syndrome in children, and to complications such as aspiration pneumonia and pneumonitis. Current approach in treatment of the syndrome is explained in dependence of its etiology. Cognitive and social skills decline during the aging process, which affects the level of health literacy. Health literacy refers to the ability to obtain, process and understand basic health information, and act upon information within the healthcare system. Elderly with an adequate level of health literacy are empowered to manage their health, as opposed to those with a low level of health literacy who have the greater chance of developing chronic illnesses and premature death. Also, elderly with low level of health literacy rarely use preventive services, but seek emergency services more often, they know less about their basic chronic illness and are more often faced with difficulties in managing their own disease. They have worse health outcomes and self-perception of health than those with adequate health literacy.
