Inflammation induce mechanical stress and changes in theextracellular matrix in the trabecular meshwork, resulting in hypertonicity and resistance toaqueous humor outflow. Vitamin D can modulate cytokine production by suppressing TH1 cellexpression and inducing TH2 cell expression. The production of pro-inflammatory cytokines can besuppressed by the presence of vitamin D. \u0000MethodsAn experimental study of 30 wistar glaucoma model were divided into 3 groups, non treatmentgroup ( A and B) and the oral vitamin D (1200 IU/kg/day) group (C) for 14 days. TNF-? expressionsin trabecular meshwork were measured by immunohistochemical staining. Group A was measuredon day 3, while group B and C were measured on day 14. The differences between the three groupswere analyzed statistically using one way ANOVA test. Significantly different if the p<0.05. \u0000ResultsThe mean expression of TNF- ? in the trabecular meshwork of wistar glaucoma model in group A, B,and C was 22,2 ± 2,51%, 39,1 ± 3.98%, 34.2 ± 3,19 % respectively. There was significantlydifference in TNF-? expressions between the three groups (p<0.001). \u0000ConclusionVitamin D per oral can decrease TNF-? expression in the trabecular meshwork of wistar glaucomamodel on day 14 . It manifests as either simulating an ophthalmic disease or denying ophthalmic disease. Thorough history-taking and careful examination of the patient are essential in reaching the diagnosis. \u0000Case Illustration : A 17-year-old female came with complaints of a spinning headache, sudden and painless blurry vision in both eyes, double vision, and a history of syncope for the last two days. Ophthalmology examination revealed UCVA was 0.5/60 for both eyes with inconsistent answers from the patient. The confrontation test, light reflex, funduscopy, Humphrey visual test, OCT, and MRI were normal. Malingering was suspected and a psychiatric consultation was made. The patient was found to be a victim of bullying and had a history of visual hallucinations. The patient was then diagnosed with severe depression with psychotic signs. \u0000Discussion : \u0000Conclusion : FVL can be suspected when there is inconsistency in the results of some clinical basic tests. Despite the association between FVL and psychological problems like depression is complicated, this condition may occur in a bidirectional relationship in which depression leads to disability or contrarily. In conclusion, diagnosing visual problems in malingering patients can be challenging. Subjective and objective tests are needed to help practitioners confirm the malingering state of the patient. According to WHO data from 2019, 246 million people suffer from low vision. Ophthalmologists must prioritize low-vision services such as refractive correction and low-vision assessment. This study aims to learn more about the relationship between characteristics and underlying eye disorders with optical aid selection, as well as differences in visual acuity status in children with low vision before and after optical aid selection. \u0000Methods: This study was an analytic observational retrospective cohort using medical records of children with low vision in RSUD Dr. Soetomo Surabaya. This study analyzed the relationship between characteristics (gender, age, education level, intervention history) and underlying eye disorders with optical aid selection using the Chi-Square test, while visual acuity status differences before and after optical aid selection were analyzed using the Wilcoxon test. \u0000Results: There were no relationships between characteristics (gender, age, education level, intervention history) and underlying eye disorders with optical aid selection (p > 0.05). There were differences in visual acuity status before and after optical aid selection (p < 0.05) with a minimum decrease of 0.1 logMAR which means an increase in visual acuity in all 72 samples of children with low vision after optical aid selection. \u0000Conclusion: These data provide new evidence of essential links between visual acuity status before and after optical aid selection. This injury results in retinal ganglion cells ischemic leading to excitotoxicity and apoptosis. Coenzyme Q10 (CoQ10) is an antioxidant and cofactor in electron transport within mitochondria that inhibits glutamate excitotoxicity and thereby preventing retinal ganglion cell apoptosis. This research aims to prove the effect of coenzyme Q10 on NMDA receptor expression and retinal ganglion cell density in rat model of traumatic optic neuropathy. \u0000Methods : This study use post-test only randomized controlled groups design. Traumatic optic neuropathy wistar rats model was made by optic nerve crushed method, then given oral CoQ10 100mg/kgBW after 1 hour for 14 days in treatment group. Retinal histopathological specimen was made with immunohistochemistry and Haematoxyllin-eosin staining and examined under microscope with 400x magnification. Data was verified and statistically analyzed. \u0000Results : The NMDA receptor expression in the treatment group was significantly lower than control (p=0.009). The retinal ganglion cell density in the treatment group was significantly higher than control (p=0.009). There was a significant correlation between NMDA receptor expression and retinal ganglion cell density in the treatment group (p = 0.012) with a strong negative correlation direction (r = - 0.652). \u0000Conclusion : Administration of CoQ10 in traumatic optic neuropathy model rats affects NMDA receptor expression and retinal ganglion cell density. NMDA receptor expression was lower with higher retinal ganglion cell density in the treatment group. There are guidelines recommended for management based on grading of severity and the surgeon’s preference. Large recession of superior rectus muscle and prismatic glasses yield good result in unilateral DVD reported in this paper. \u0000Case Illustration : A 21 year-old student stated difficulty in working with his microscope since 2 years ago. He had a history of double vision since childhood. The visual acuity in both eyes were 5/5. Qualitative strabismus examination resulted in hypertropia RE and intermittent exotropia. When the occluder was removed from RE, a non fixating eye would appear elevated and did not correspond with the contralateral eye hypotropia. Krimsky test revealed RE hypertropia 30? PD, exotropia 10? PD. Sensory evaluation using WFDT was normal in near and crossed diplopia in distance. Patient diagnosed with unilateral DVD then underwent superior rectus recession in 10 mm. The following month, patient was still experiencing vertical diplopia. The patient was given 3 PD prismatic glasses. Consequently, diplopia was absent and stereoacuity test using TNO was 480 seconds of arch \u0000Discussion : DVD can’t be fully corrected by surgery due to central cortical innervational problem. The residual symptom can be corrected by conservative management. To determine the effectiveness of adding Anti VEGF bevacizumab to the results of trabeculectomy in glaucoma. \u0000Methods : Searches were conducted in a structured way using keywords across several data sources, including Pubmed, Proquest and Cochrane library. The keywords used for searching were “Bevacizumab”, “Trabeculectomy” and “Glaucoma”. The reviewed articles are Randomized Control Trial in english, conducted on humans in the last 15 years. The review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. \u0000Results : From a total of 353 articles, there were 6 studies with 362 eyes of 352 patients been evaluated for the effectiveness of bevacizumab addition to trabeculectomy. The primary outcome of all articles was the assessment of Intra Ocular Pressure (IOP). No significant difference in IOP was found between the subconjunctival bevacizumab + Mytomycin-c group and mytomycin-c alone group (MD 0.11 ; 95% CI -0.10 to 0.33), as well as subconjunctival comparison with mytomycin-c (MD 0.11 ; 95% CI -0.29 to 0.50). Similar results were also found in the intracameral and placebo groups (MD 0.08 ; -0.22 to 0.38). However, 3 articles reported significant differences in peripheral vascularity of the bleb both by the Moorfields Bleb Grading System and by the Indiana Bleb Appearance System. \u0000Conclusion : The addition of bevacizumab to trabeculectomy did not show any benefit in reducing IOP when compared with MMC. It has been reported as one of the most frequent anterior segment ocular manifestations in HIV patients, occurring late in the disease. The study aimed to investigate clinical correlation of CD4 count and severity of the disease. \u0000Methods : This study was a hospital based cross-sectional study where 75 HIV patients (150 eyes) aged 18-50 years old were referred to ophthalmology clinic in VCT for ocular surface related complaints and were included in the study. \u0000Results : The study showed significant moderate positive correlation between CD4 count with Tear Break Up Time (TBUT) (r-value=0.509;p-value=0.05) and negatively correlated with Ocular Surface Disease Index (OSDI) score (r-value=-0.252, p-value=0.035). There were a significant difference in CD4 count between mild and moderate dry eyes (p-value=0.005) and between mild and severe dry eyes (p-value=0.005), however no significant difference was observed between moderate and severe dry eyes (p value=0.435). The severity of dry eye in HIV patients also tends to differ in-between clinical stage of the disease (p value=0.513) and with the presence of co-infection (p value=0.228), although the result was not statistically significant. \u0000Conclusion : There is a correlation between CD4 Count and Dry Eye Disease severity among HIV patients. The lower the CD4, the higher the risk for HIV patients to get severe dry eye disease. This study aims toanalyze the difference of visual outcome between early and delayed vitrectomy in endophthalmitispatients operated in Dr. Kariadi General Hospital \u0000MethodsA Retrospective study held from medical record of patients with endophthalmitis underwentvitrectomy surgery from January 2021 – January 2023 in Dr. Kariadi General Hospital, Semarang.Endophthalmitis patients caused by trauma were excluded. All of the patients had immediatevitrectomy, and based on duration of symptoms, the patients were divided into two groups, earlyvitrectomy (duration of symptoms ? 7 days) and delayed vitrectomy (duration of symptoms > 7days). Independent t- test method was used to analyze the data (significant if p<0.05) \u0000ResultsTotally 30 endophthalmitis patients (30 eyes) were obtained. There were 18 patients (60%) had earlyvitrectomy and 12 patients (40%) had delayed vitrectomy. Postoperative examination showed 13(72%) patients with early vitrectomy had improved VA, and 5 (28%) had constant VA. On the otherhand, patients with delayed vitrectomy showed 5 (42%) had improved VA, 6 (50%) had constant VA,and 1 (8%) had decreased VA. Intraorbital injection of triamcinolone acetonide (TCA) in GO resulted in clinical improvement, but its effect on the quality of life is unknown. The objective of this study was to compare the changes in quality of life after intraorbital injection of TCA between different groups based on changes in disease severity. \u0000Methods : Intraorbital injection of TCA was performed on 30 subjects with GO. Disease severity was assessed according to the European Group of Graves Ophthalmopathy (EUGOGO) criteria. Quality of life was measured using the Indonesian version of Graves Ophthalmopathy Quality of Life (GO-QOL) questionnaire. Changes in disease severity and GO-QOL scores were recorded before intraorbital injection of TCA and 1 month after. Subjects were divided into groups based on changes in disease severity: decreased, remained constant, and increased. Changes in quality of life between the groups were compared statistically. \u0000Results : Changes in GO-QOL appearance scores in patients with decreased disease severity were significantly higher (p= 0.016) than those in patients whose disease severity remained constant. No constraints based on language or publication date were applied. Only randomized controlled trials (RCTs) comparing intravitreal aflibercept injection with vitrectomy procedure were included. Search results were screened independently by two authors. Data extraction and synthesis were performed according to The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines. The primary outcome measures were best-corrected visual acuity obtained at least 6 months following interventions, rate of recurrent hemorrhage, and periprocedural ocular adverse events. The quality of evidence was assessed using the GRADE tools. \u0000Results : Two clinical trials that randomized 239 eyes were included in this review. There was no evidence ofa difference in primary outcome of mean visual acuity at six months between randomized groups. However, patients receiving intravitreous aflibercept were more likely to experience recurrent VH than those undergoing vitrectomy (RR 3.12 95% CI 1.95 - 4.99, absolute effect of 32 more people with recurrent VH per 100 people treated with aflibercept). Rates of periprocedural ocular complications were similar in both groups. \u0000Conclusion : Intravitreous aflibercept safety and efficacy appear to be non-inferior compared to vitrectomy for initial management of diabetic VH.
