It may result in severe visual function impairment and potentially lower the patient's quality of life. IOL exchange is an unusual procedure as the incidence rate was 2 per 1000 surgeries over 8 years. It is the surgical procedure of choice for symptomatic IOL discoloration. \u0000Case Illustration : A 50-year-old man came to the hospital complaining of a right eye blurry vision for the previous two months. His right eye had undergone cataract surgery seven years earlier. After having cataract surgery, the patient's vision was clear, but five years later, it began to feel blurry and glare. The patient has a history of well-controlled type II diabetes and hypertension. An intraocular lens discoloration was discovered after a careful examination of the eye (figure 1). For his right eye, the patient underwent an IOL exchange procedure (figure 2). A square-edge single-piece foldable IOL was obtained from the surgery. Follow-up examinations after surgery revealed an improved visual function and a clear intraocular lens (figure 3). \u0000Discussion : IOL discoloration may occur years after surgery. It is related to the type of IOL, local ocular factors, and the patient's systemic condition. The IOL exchange procedure consists of IOL extraction and secondary IOL implantation. The extraction can be done by cutting, splitting (bisecting), or folding the IOL. Different methods of fixation can be used for secondary IOL implantation. \u0000Conclusion : The IOL exchange procedure can optimally restore visual functions after a severe decline caused by IOL discoloration.

But it was not always in stock at a hospital in remote area with low resource. Use of intravenous catheter to exchange the unavailable slicone tube was a possible option. \u0000Case Illustration : We reported two cases of inferior canalicular laceration at NTB provincial hospital. Both cases were adult, one female and one male. First case caused by nail and second because of traffic accident. There were no globe involvement in both case. Reconstruction surgeries with monocanalicular approach were performed. Intravenous catheter was used for stenting the canaliculus. First case was using a 24G, the other was 22G. The catheters were well inserted and planned to be removed at3 month. At the day 7 follow-up, the 24G catheter was extruded spontaneously while the patient with 22G was not. Both patients did not complain of profuse tearing. \u0000Discussion : The diameter and length of 24G iv catheter were 0.7mm and 19mm, while 22G were 0.9mm and 25mm. The diameters were similar with the silicone tube (0.6mm) and the lengths were long enough to pass the lower and common canaliculus which has a total 15mm long. The catheter in the first case was extruded possibly because in 24G there was only 4mm inside th lacrimal sac. A high pressure air from the nose would push the catheter backward and extruded. The 22G would have 10mm inside the sac. \u0000Conclusion : Intravenous catheter was an alternative option for inferior canalicular laceration stenting in low resource setting.

The dialysis process may change the IOP and Mean Arterial Pressure (MAP)which may lead to glaucoma. The aims of this study are to determine whether there was a change inIOP and MAP and to determine the correlation between changes in IOP and MAP due to dialysis. \u0000MethodsThis is an observational study with cross sectional approach using stratified sampling method. Thesubjects were all regular dialysis patient at the Dialysis Unit in Sanglah Hospital period April-May2021 with total 63 people. The data taken are age, gender ,duration of dialysis, fasting blood sugar, urea and serum creatinine level before dialysis. Subjects were measured IOP and calculated MAPbefore and after dialysis. \u0000ResultsMean of age of this study was 47,03±12,06 years old and dominated by male. Paired T-Test showedno significant changes IOP after dialysis but there were significant decreased on systolic, diastolicand MAP. Pearson correlation showed no correlation between changes of IOP and MAP afterdialysis. Multivariate analysis shows significant correlation between changes of IOP with urea levelbefore dialysis. \u0000ConclusionIOP after dialysis was not increased in this study. Systolic, diastolic and MAP were decreased afterdialysis compared to before dialysis. There is no correlation between changes in IOP and MAP afterdialysis compared to before dialysis. Preterm birth leads to incomplete development of retina that will cause ROP. ROP screening is an early strategy to prevent irreversible blindness in ROP. Objective: The purpose of this study was to describe the characteristics of ROP in Makassar between January 2020 and December 2022. \u0000Methods : A multi-center observational descriptive study at ten hospitals in Makassar. \u0000Results : The total patients in this study were 168 infants (336 eyes) included 53.25% male and 46.15% female. The incidence of ROP in Makassar between January 2020 and December 2022 was 14.87%. Most of the patients had history of preterm birth with gestational age ranged from 28 to 33 weeks and low birth weight less than 1500 gram in 50% patients. Majority of patients had history of oxygen therapy (77.98%), and other risk factors such as sepsis, twin birth, history of transfusion, or apnea. About 98.81% patients had bilateral ROP and 33.93% were diagnosed with stage 3 at first screening. Photocoagulation laser was done in 41 patients (24.40%). \u0000Conclusion : The ROP screening in Makassar had a good outcome in managing the premature infants, so they can be diagnosed and got treated earlier. Recurrences risk factors are patients above 40 years, patients with de novo toxoplasmosis lesions or with less than one year after the first episode, macular area involvement, lesions greater than one disc diameter, congenital toxoplasmosis, and bilateral compromise. \u0000Case Illustration : Female, twenty years old, complained blurred vision on the RE for two years. The patient had a history of cat contact and no systemic manifestation. Anterior segment examination in both eyes is normal with right eye visual acuity 1/60 and left eye visual acuity VA 6/6. Posterior segment examination on the left eye showed normal limit while the right eye showed hyperpigmentation in inferotemporal optic disc and nasal macula. Retina showed hard exudate. Macula showed yellow- white exudate with subretinal bleeding and macular star appearance. Foveal reflex under normal limit. Toxoplasma serology was IgG (+) and IgM (-). The patient has been treated with cotrimoxazole 2x960mg and methylprednisolone 1x32mg. \u0000Discussion : Visual acuity is severely damaged in recurrence toxoplasma chorioretinitis. There are several factors that affect the recurrences of toxoplasma chorioretinitis. In this patient, the risk factors found are de novo lesion, lesions greater than 1 disc diameter, and macular involvement. One of many risk factors for DED is cataract surgery. Cataract surgery is known to modify tear film and ocular surface through corneal incision and ocular toxicity of peri- and postoperative medications. The aim of this study is to analyze tear film before and after phacoemulsification in primary eye care facility by measuring tear break-up time (TBUT), fluorescein staining, Schirmer I test (ST-I), and ocular surface disease index (OSDI) score. \u0000Methods : This was a longitudinal observational study with prospective cohort design, which included patients diagnosed with senile cataract and underwent uncomplicated phacoemulsification surgery at primary eye care center Sawangan Eye Clinic within period of June to December 2022. Friedman, Wilcoxon, and Cochran tests were used to analyze the data. \u0000Results : Thirty nine eyes from 39 patients met the inclusion criteria and were followed up to 3-month postoperatively. Mean age of the patients was 61.1±10.54 years, which consited of 53.8% males and 46.2% females. Mean preoperative OSDI score, corneal staining, TBUT, and ST-I were 24.94±10.24, 1.38±1.99, 2.94±2.17, and 15.46±8.80 respectively; while 3-month postoperative, these parameters’ means were 11.22±13.15 (p=0.0001), 1.38±2.56 (p=0.118), 3.13±1.79 (p=0.192), and 13.31±9.45 (p=0.006) respectively. OSDI was the only parameter which showed statistically significant changes from preoperative to postoperative follow ups (p<0.05). \u0000Conclusion : Phacoemulsification did not alter corneal staining and TBUT significantly after 3 months postoperatively, even though both of these parameters' preoperative value were already lower; yet OSDI score improved over time after surgery.

Early and proper diagnostics and management are needed to reduce morbidity. In this paper, we reviewed the diagnostic and surgery trends of IOFB in the last 20 years. We aim to serve the update and better understand how to manage cases of IOFBs properly. \u0000Methods : We conducted a literature review on Scopus from 2002 to 2022 using the PRISMA method (assisted by the Watase UAKE application). We include imaging and surgery data in cases of metallic and non- metallic IOFB in the anterior, intralenticular, and posterior segments. \u0000Results : Thirty-one studies were included from Scopus and reference digging. We then classified the study into two main topics, surgery and diagnostic. In the last 20 years, imaging in diagnosis has been beneficial. CT-scan and MRI are still the leading options in visualizing metallic and non-metallic IOFB, respectively. However, in the case of tiny IOFB, MRI might be performed to visualize the metallic component with special considerations. IOCT also be considered to ensure no IOFB is left behind after removal. As for surgery, combining IOFB removal with pars plana vitrectomy remains the primary choice for IOFB in the posterior segment, combined with phacoemulsification and IOL implantation for the intralenticular and magnetic for the anterior segment. Combining two or more surgical techniques improves outcomes and prevents postoperative complications. \u0000Conclusion : A proper understanding of the components, location, and size of IOFB is required to determine the use of practical examinations and management of IOFB cases.
