It affects patients’ productivity and quality of life. The management for myopic astigmatism is challenging because no consensus on the ideal method was reached. \u0000Case Illustration : Case 1: 19-year-old male with Uncorrected Visual Acuity (UCVA) 3/60 in the right eye with a manifest refraction of S-3.25 C-2.00 x 0 and 2/60 in the left eye with a manifest refraction S-5.50 C-0.50 x 0. Case 2: 18-year-old male with UCVA 3/60 in both eyes with a manifest refraction of S-3.00 C-2.25 x 5 and S-2.50 C-2.00 x 0. Case 3: 18-year-old male with UCVA 3/60 in both eyes witha manifest refraction of S-2.75 C-2.00 x 0 and S-2.50 C-2.25 x 180. Slit lamp, fundus examination, corneal topography, tomography, epithelial thickness profile, and aberrometry were within normal limits. The patients underwent SMILE using Visumax 800 Femtosecond laser with 3-4 mm incision at 120°, optical zone between 6-6.8 mm and cap thickness between 100-120 ?m. \u0000Discussion : In this case series, SMILE was chosen because studies described SMILE as an effective and safe refractive procedure compared to Femtosecond Laser-assisted in situ keratomileusis (FS-LASIK). Target refraction is emmetropia without cyclotorsion adjustment, it showed that post operative UCVA in all eyes were 6/6 using Snellen chart and none of the eyes loss the BCVA. \u0000Conclusion : New Generation of SMILE as management of myopic astigmatism with high astigmatism value shows good result improving visual outcomes.

Changes in cellular structure after trabeculectomy underlie the instability of the tear film. This study aims to determine the relationship between DED and post-trabeculectomy bleb morphology. \u0000Methods : An observational study with survival analysis design to evaluate the time to DED in primary glaucoma patients with post- trabeculectomy blebs was undertaken at the Eye Health Polyclinic of Mohammad Hoesin Hospital Palembang from January to March 2022. There were 30 primary glaucoma patients who met the inclusion criteria. Bivariate analysis to determine the relationship between bleb morphology and DED was analyzed using the Fisher Exact test. DED survival analysis was conducted using Kaplan Meier analysis. \u0000Results : Three months after trabeculectomy, 11 of 30 (36.7%) patients suffered from DED. There was a significant relationship between bleb height and DED three months after trabeculectomy [PR = 14.222 (Cl 95% 2.324- 87.028; p = 0.004)]. In addition, the results showed that there was a significant difference in the survival time of DED in glaucoma patients based on bleb height (p = 0.001). \u0000Conclusion : Bleb height is a predictor of DED in glaucoma patients after trabeculectomy.

In unresolved cases, lacrimal probing is considered a primary treatment; however, no consensus on the best timing to perform probing is available. Therefore, this study aims to compare the effectiveness of immediate probing with deferred probing and to assess the best time to perform primary probing. \u0000Methods : A search for relevant literature was performed on four electronic databases: PubMed, Cochrane Library, EMBASE, and Scopus with predetermined keywords. The inclusion criteria consist of systematic reviews, RCTs, and prospective/retrospective studies published between 2013-2023 regarding lacrimal probing as an intervention for congenital NLDO. \u0000Results : We included seven studies. Three studies compared immediate and deferred probing, while four focused on the success rate of probing for NLDO at various age groups. Studies suggest immediate and deferred probing had a comparable effectivity in treating NLDO; however, one systematic review suggests immediate probing was superior to deferred probing. Studies also demonstrate that the success rate of probing declines with age. Lacrimal probing performed on NLDO patients under 12 months old had a success rate of more than 80%. \u0000Conclusion : Despite findings of comparable effectiveness between immediate and deferred probing, probing performed at a younger age yielded a higher success rate. This suggests that doctors should consider the patient's age at diagnosis to decide between immediate and deferred probing. Further evidence is needed to determine cut-offs for the best time to perform primary probing.

Pars PlanaVitrectomy (PPV) is a common surgical procedure aimed at removing vitreous gel and replacing itwith variety of substances denominated tamponades. Vitreous removal and replacement may causeincreased intraocular pressure (IOP). The IOP can be reduced with medical treatment or surgery. The purpose of this study is to describe the characteristics of secondary glaucoma after vitreoretinalsurgery in National Eye Center Cicendo Hospital from January 2019 - December 2020. \u0000MethodsThis study was a retrospective descriptive study which data were obtained from medical records.Data such as age, gender, vitrectomy tamponades, IOP, onset of glaucoma, and glaucoma treatmentwere collected. \u0000ResultsOne hundred fifty-three eyes were referred to Glaucoma Unit with increasing IOP after vitreoretinalsurgery. PPV with tamponade was done in 141 eyes (92.16%) and silicone oil (SO) 1000 centistokes(cSt) was the most used tamponade in 101 eyes (66.07%). Secondary glaucoma with open angle wasthe most common type in 124 eyes (81.04%). Secondary glaucoma developed in 4-7 weeks aftersurgery, with the highest IOP recorded at 31-40 mmHg. The IOP decreased after SO evacuation withinitial glaucoma treatment in 121 eyes (79.09%), and glaucoma surgery was performed in 32 eyes(20.91%). \u0000ConclusionSilicon oil as a tamponade agent after PPV may cause rise IOP in the post-operative period.Evacuation SO needs to be done after retinal adhesion to prevent secondary glaucoma. Pathogen detection from ocular fluid samples is often necessary to support infectious uveitis diagnosis, particularly in cases presenting with atypical clinical appearance. This study aims to present the process of developing targeted multiplex PCR and its application in infectious uveitis. \u0000Methods : This was a cross sectional study to evaluate the diagnostic performance of targeted multiplex PCR in infectious uveitis. We obtained ocular fluid samples and reviewed medical records of uveitis patients who underwent ocular fluid analysis at Cipto Mangunkusumo Hospital from February 2022 to March 2023. PCR detection threshold values (DNA copies/mL) were 10.9 for Mycobacterium tuberculosis (Mtb), 672 for Epstein-Barr virus, 4.77 for Cytomegalovirus, 6.37 for Toxoplasma gondii, and 5.53 for Herpes simplex virus. With every two-fold increase of pathogen selection, this method requires a half volume of extracted DNA template from aqueous/vitreous samples than uniplex PCR. The ophthalmologist selected the pathogen combination to be detected, allowing for a tailored examination. \u0000Results : Forty-seven aqueous or vitreous samples were analyzed. The positivity rate was 23.4% (11/47) with Mtb yielded the highest positivity (7/34; 20.6%). With final diagnosis as a reference, targeted multiplex PCR resulted in 32.3% sensitivity, 93.8% specificity, 90.9% positive predictive value and 19.2% negative predictive value. \u0000Conclusion : With its high specificity, targeted multiplex PCR is useful as a confirmatory but not screening tool in uveitis diagnosis. The loss of RGC and inner plexiformlayer (IPL) is strongly correlated with the loss of visual field defect. RGC and inner plexiform layerare potential targets to evaluate the progression of glaucoma. Glaucoma treatment not only focus onreducing IOP. Decreasing IOP according to the target pressure often can’t stop the process of RGCdamage and make it permanently and irreversible. Vitamin D3 is known as neuroprotector which hasthe ability to protect RGC and prevent layer thinning of GC-IPL in retina. In Indonesia, the Rapid Assessment of Avoidable Blindness survey showed the largest number of moderate and severe visual impairment was found in South Sulawesi, particularly uncorrected refractive error (33.8%). The results of this study are expected to initiate and introduce IPCEC-based eye health services and promotion at the sub-district level. \u0000Methods: The program started with advocacy of eye health programs at the sub-district level to policy makers. In schools, education and training for visual acuity examination were given to teachers. All children who failed the screening or have ocular abnormalities are referred to eye care center. Mass counseling and examination were also carried out at the health center simultaneously with screening for mass cataract surgery at the Barru District Hospital. \u0000Results: Implementation of IPCEC was welcomed by Barru District government. School eye health program conducted by the teachers in 5 schools screened 1263 students with 90.74% of the students had normal visual acuity. Mass screening at the Pekkae and Pancana Public Health Centers found that most participants had cataracts (58%) followed by refractive error (38%). \u0000Conclusion: Visual acuity examination among school-age children by the teacher will improve early detection of refractive errors.
