Heart-rate variability (HRV), modulated by the sympathetic and parasympathetic branches of the autonomic nervous system (ANS), offers insights into cardiac autonomic control and cardiovascular well-being. The present study aimed to explore the impact of GABA supplementation on emotional metrics, sleep quality, and HRV in sedentary women with overweight or obesity partaking in physical exercise. A randomized, double-blind, placebo-controlled clinical trial was undertaken involving 30 sedentary women with overweight or obesity. Volunteers were assigned randomly to two groups: the intervention group receiving GABA (200 mg) once daily for a total of 90 supplementation doses, and the placebo group. Both groups engaged in physical exercise, while the supplementation regimen spanned 90 days. Assessments were conducted at three intervals: baseline (T0), midway through the study (T45), and study culmination (T90). Following 90 days of GABA supplementation, the intervention group demonstrated enhancements in habitual sleep efficiency, as indicated by reductions in Pittsburgh Sleep Quality Index (PSQI) scores. Moreover, an improved emotional response was observed, characterized by diminished negative affect. GABA supplementation yielded ameliorations in depression scores as per the Depression, Anxiety, and Stress Scale (DASS-21). Notably, an augmented HRV was noted, attributed to heightened parasympathetic autonomic nervous system predominance. For some, genetic factors may influence sleep disturbances. L-methylfolate, the active form of folate, plays a critical role in regulation of monoamine neurotransmitters known to have significant impact on sleep regulation: dopamine, serotonin, norepinephrine. Single nucleotide polymorphisms of the enzyme methylene-tetrahydrofolate-reductase are common and can impact monoamine production. The goal of this study was to evaluate effects of L-methylfolate supplementation on sleep in a cohort with reduced methylene tetrahydrofolate reductase (MTHFR) activity.</p><p><strong>Methods: </strong>A retrospective cohort of patients being treated with L-methylfolate in a concierge medical clinic setting was studied. Patients presenting with sleep complaints were evaluated using the Patient-Reported Outcomes Measurement Information System at baseline. Patients with known MTHFR polymorphisms at either C667T and/or A1298C were recommended 5 mg of L-methylfolate daily and were reevaluated at 2 wks, at 4 wks, and at 8 wks of supplementation. Statistical comparisons were made utilizing ANOVA and T-test comparisons.</p><p><strong>Results: </strong>Ten were included in the final cohort: six male and four female, average age 43 ± 16 years. Beginning at wk 2, average sleep disturbance improved significantly by -6.94 points (<i>p</i> = 0.005) and by 8 wks, all patients had improvement with a -14.34 change in disturbance from baseline (<i>p</i> = 0.001).</p><p><strong>Conclusion: </strong>Improvement in sleep disturbance was seen in both low and intermediate function phenotypes. To ensure the benefits of cranberry dietary supplement products, the anti-adhesion activity (AAA) against uropathogenic bacteria is routinely used in in vitro bioassays to determine the activity in whole product formulations, isolated compounds, and ex vivo bioassays to assess urinary activity following intake. D-mannose is another dietary supplement taken for UTI prevention, based on the anti-adhesion mechanism.</p><p><strong>Objective: </strong>Compare the relative AAA of cranberry and D-mannose dietary supplements against the most important bacterial types contributing to the pathogenesis of UTI, and consider how certain components potentially induce in vivo activity.</p><p><strong>Methods: </strong>The current study used a crossover design to determine ex vivo AAA against both P- and Type 1-fimbriated uropathogenic <i>Escherichia coli</i> of either D-mannose or a cranberry fruit juice dry extract product containing 36 mg of soluble proanthocyanidins (PACs), using bioassays that measure urinary activity following consumption. AAA of extracted cranberry compound fractions and D-mannose were compared in vitro and potential induction mechanisms of urinary AAA explored.</p><p><strong>Results: </strong>The cranberry dietary supplement exhibited both P-type and Type 1 in vitro and ex vivo AAA, while D-mannose only prevented Type 1 adhesion. Cranberry also demonstrated more robust and consistent ex vivo urinary AAA than D-mannose over each 1-week study period at different urine collection time points. Encapsulation is used to increase their solubility in water-based food products. However, it is not yet known whether encapsulation with a combination of lecithin and medium-chain triglycerides improves carotenoid bioaccessibility and intestinal cell uptake. The relative bioaccessibility and Caco-2 cell uptake of two water-soluble carotenoid (i.e. lutein and astaxanthin) dispersions in a liquid form (VitaSperse<sup>®</sup>) and a powdered form (VitaDry<sup>®</sup>) were compared to the carotenoid ingredient alone. An <i>in vitro</i> digestion model was used to assess bioaccessibility, measuring the micellarized fraction postdigestion. The micelle fraction was incubated with Caco-2 cells to assess intestinal uptake of carotenoids. Encapsulation (by either VitaDry<sup>®</sup> or Vitasperse<sup>®</sup>) increased total astaxanthin bioaccessibility 2-2.4× and cell uptake by ∼2× relative to control. Encapsulation also increased total lutein bioaccessibility by 3-5× and cell uptake 2.3× relative to control. There was no significant difference between VitaDry<sup>®</sup> and VitaSperse<sup>®</sup> products in regards to Caco-2 cell uptake. Increased bioaccessibility largely drove increased carotenoid cell uptake from the encapsulated formulations. Fears of rancidity, or the oxidation of consumer Ω3 supplements, has been debated in the literature with several prior authors reporting contradictory findings. We report the peroxide value (PV), para-anisidine value (p-AV) and total oxidation values (TOTOX) associated with 72 consumer Ω3 supplements sold in the United States sampled from 2014-2020. The effect of flavoring on the oxidation of the supplements was examined in an adjusted fixed effects model controlling for type of delivery system (enteric, liquid, animal- and vegetable-derived gelatin softgel, spray), source (algae, calamari, fish, krill, mussels), and certifications assigned by third-party organizations (e.g. USP). Overall, our results revealed that 68% (23/34) of flavored and 13% (5/38) unflavored consumer Ω3 supplements exceeded the TOTOX upper limit set by the Global Organization for EPA and DHA (GOED) voluntary monograph standard of ≤ 26, with 65% (22/34) flavored supplements and 32% (12/38) unflavored supplements failing the PV upper limit of ≤ 5 and 62% (21/34) flavored supplements exceeding the p-AV upper limit of ≤ 20. In the majority of these supplements, the most abundant plant sterol is saw palmetto extract or its' principal component, beta-sitosterol. In terms of prostate health, previous work almost exclusively focused on the effects of beta-sitosterol on prostatic epithelium, with little attention paid to the effects on prostatic stroma. This omission is a concern, as the abnormal accumulation of collagen, or fibrosis, of the prostatic stroma has been identified as a factor contributing to lower urinary tract symptoms and dysfunction in aging men. To address whether beta-sitosterol may be promoting prostatic fibrosis, immortalized and primary prostate stromal fibroblasts were subjected to immunoblotting, immunofluorescence, qRT-PCR, ELISA, and image quantitation and analysis techniques to elucidate the effects of beta-sitosterol on cell viability and collagen expression and cellular localization. The results of these studies show that beta-sitosterol is nontoxic to prostatic fibroblasts and does not stimulate collagen production by these cells. However, beta-sitosterol alters collagen distribution and sequesters collagen within prostatic fibroblasts, likely in an age-dependent manner. The interplay between immune cells and signaling pathways is a topic for investigating novel therapies. Past research has shown how the Th1/Th2 ratio plays a key role in the pathogenesis of MS lesions. Modulating the Th1/Th2 ratios with an efficacious dietary supplement may improve some of the consequences of MS.</p><p><strong>Methods: </strong>Participants (<i>n</i> = 15) diagnosed with MS for an average of 12.4 years (standard deviation = 7.4; range = 2, 25) were enrolled in a clinical trial in which they consumed a dietary supplement regimen daily for 12 months. Venous blood was drawn at baseline and 12-month follow-up and peripheral blood mononuclear cells, cytokines, and growth factors were quantified. Infections, physical functioning, and quality of life were also assessed at baseline and 12 months.</p><p><strong>Results: </strong>The IL-2/IL-10 and IFN-γ/IL-10 ratios were significantly higher than those of the healthy adults, and while only IFN-γ/IL-10 increased significantly at 12 months, all ratios other than IFN-γ/TNF-α increased over the course of the intervention. The decrease in yeast infections was inversely correlated with IL-2/TNF-α and IFN-γ/TNF-α. Dietary collagen peptides have been reported to be beneficial for health conditions. A randomized, double-blinded, placebo-controlled, parallel-group study investigated the effects of collagen peptides on mood status including fatigue, physical condition, and immunological status. The participants who were healthy but easily fatigued consumed active food containing collagen peptides (10 g/day) (<i>n</i> = 33) or placebo food (<i>n</i> = 33) for eight weeks. POMS® 2 was used to assess the perceived fatigue and other mood status. The primary outcome was the T-scores of Fatigue-Inertias at eight weeks. In addition, their physical condition and immunological parameters were evaluated. The data set was a per protocol set of 31 participants each in both groups. As for POMS® 2, the T-score of Fatigue-Inertias at eight weeks was significantly lower in the active group than in the placebo group (47.0 ± 7.4 versus 51.5 ± 9.0, p = 0.045). The T-score of Vigor-Activity was significantly higher in the active group than in the placebo group after eight weeks (53.9 ± 10.7 versus 47.3 ± 9.6, <i>p</i> = 0.002). Regarding the questionnaire on the physical condition, the Likert scale score on fatigue after a night's sleep at eight weeks was significantly lower in the active group than in the placebo group (median; 3.0 versus 4.0, <i>p</i> = 0.038). There were no significant differences in the measured values of immunological parameters. No safety-related issues were reported in this trial. The intake of collagen peptides (10 g/day) for eight weeks ameliorated the mood status of fatigue and vigor and increased the feeling of sleep restfulness. This is an observational, open, single-arm multicenter study aimed to evaluate the effectiveness of a nutritional supplement in patients with knee and/or hip OA. A total of 186 patients were recruited from Spanish centers and received a supplement containing hydrolyzed collagen (3000 mg), chondroitin sulfate (800 mg), glucosamine sulfate (700 mg), turmeric extract (250 mg) and devil's claw (150 mg), once daily during 6 months. The primary outcome was the patients' self-perceived pain in the affected joints measured with a visual analogue scale (VAS). Secondary outcome was the patient's functioning, measured with the Lequesne Functional Index and the Western Ontario and McMaster Universities Arthritis Index (WOMAC). Participants showed a significant reduction in self-perceived pain after 3 (mean reduction ± standard deviation, 1.99 ± 1.05) and 6 months (3.57 ± 1.39) of treatment (<i>p</i> < 0.0001 in both comparisons). Lequesne Functional Index score was significantly reduced at 3 months (3.86 ± 2.94) and at 6 months (6.73 ± 4.30) of treatment (<i>p</i> < 0.0001 in both comparisons). The WOMAC index was also significantly reduced after 3 (14.24 ± 10.04) and 6 months (26.43 ± 17.35) of treatment (<i>p</i> < 0.0001 in both comparisons). Significant reductions in WOMAC subdomains (<i>p</i> < 0.0001 in all comparisons) were observed. No severe adverse events were reported during the study.
