Objetivos Avaliar a prevalência de aneurismas viscerais raros em angiotomografias realizadas para diagnóstico e seguimento de pacientes com diferentes patologias vasculares. Métodos Este estudo transversal partiu do acesso a todos os laudos de angiotomografias realizadas entre janeiro de 2005 e julho de 2021 em hospital privado de excelência situado na cidade de São Paulo. Foi utilizado um programa de mecanismo de busca de laudos pré-indexados, cuja base de dados é o Sistema de Informação Radiológica (RIS), para acesso aos laudos de pacientes com aneurismas intra-abdominais. Resultados Foram acessados laudos de angiotomografias de 92.883 pacientes, dos quais 2.597 (2,795%) apresentavam aneurismas intra-abdominais, sendo 937 (1,063%) viscerais, incluindo 158 (0,171%) aneurismas viscerais raros, mais frequente entre homens e nos seguintes segmentos: tronco celíaco (0,098%), artéria mesentérica superior (0,033%), artéria gástrica esquerda (0,010%), artéria pancreático-duodenal (0,009%), artéria gastroduodenal (0,005%) e arco pancreático (0,004%). Prevalências menores foram encontradas em outros diferentes segmentos. Achados adicionais revelaram taxa de concomitância de aneurismas viscerais raros com outros aneurismas intra-abdominais entre 11,11% e 66,67%. Conclusões A prevalência de aneurismas viscerais raros em ampla população submetida a angiotomografias foi de 0,171%, com maior comprometimento nos pacientes do sexo masculino.","PeriodicalId":14814,"journal":{"name":"Jornal Vascular Brasileiro","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136304342","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of this study is to consolidate all information related to venous drainage of the thyroid gland as a ready reference for vascular and endocrine surgeons. The study was conducted at the Department of Anatomy and the literature search was carried out using the Pubmed, Scielo, Researchgate, Medline, and Scopus databases. Various terms related to the thyroid gland and its venous drainage were used to explore the literature. The literature review revealed that the superior and middle thyroid veins have the fewest variations in terms of course and termination while the inferior thyroid vein has the most variations in terms of course and termination. Detailed knowledge of normal and variant anatomy of the thyroid veins is of utmost use for vascular surgeons performing anterolateral neck surgery, especially tracheostomy, a lifesaving procedure, minimizing intraoperative and postoperative complications and morbidity and mortality.
","PeriodicalId":14814,"journal":{"name":"Jornal Vascular Brasileiro","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10153794/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"9767273","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The grayscale median (GSM) value is one of the most widespread methods of studying atherosclerotic plaques and it is therefore important to comprehensively standardize image post-processing. Post-processing was performed using Photoshop 23.1.1.202. Images were standardized by adjusting the grayscale histogram curves, setting the darkest point of the vascular lumen (blood) to zero and the distal adventitia to 190. Prior knowledge of the quality of the vein can guide choice of therapeutic strategy, avoiding surgery that is doomed to failure. Discrepancies between intraoperative findings of the quality of the great saphenous vein and imaging tests are also frequently observed.

Objectives: To evaluate the diameter of the great saphenous vein using two imaging methods (Duplex Ultrasound and Computed Tomography) and the gold-standard (intraoperative direct measurement of the vein), comparing the results.

Methods: Prospective, observational study of data obtained during routine medical procedures performed by the Vascular Surgery team.

Results: 41 patients were evaluated, with a 12-month follow-up. 27 (65.85%) were male and mean age was 65.37 years. 19 (46.34%) patients had femoropopliteal grafts and 22 (53.66%) had distal grafts. Preoperative saphenous vein internal diameters measured with the patient supine were on average 16.4% smaller on CT and 33.8% smaller on US than the external diameters measured after intraoperative hydrostatic dilatation. There were no statistical differences in measurements when sex, weight, and height were considered.

Conclusions: Saphenous vein diameters were underestimated by preoperative US and CT scans when compared to intraoperative measurements. They are classified according to the extent of damage and computed tomography angiography has the highest sensitivity and specificity for identifying the degree of injury and potential associated lesions. The patient's degree of stability can also help to define the choice of treatment, which can be conventional or endovascular surgery (EVAR) or even conservative management in selected cases. Among patients with adequate vascular anatomy, endovascular surgery is associated with better survival and fewer risks. The objective of this article is to describe a series of four cases followed up at a tertiary service in a Brazilian state that has few centers that provide high complexity care. This study was conducted in response to the vast clinical implications associated with this vein. The aim of this review is to highlight and describe the prevalence, varied morphology, and morphometry of the mastoid emissary vein, how these varied parameters cause clinical complications, and how these can be rectified and avoided. A literature survey was conducted using various databases and different terms related to mastoid emissary vein were used to search the literature. Pitfalls related to surgery in the vicinity of this vein and their remedies were elucidated. The literature search revealed that the prevalence, morphology, and morphometry of mastoid emissary veins vary immensely and are responsible for morbidity and mortality. These aneurysms have varied presentations depending on size and proximity. Both open surgical and endovascular modalities can be used for treatment depending upon presentation, aneurysmal anatomy, and patient condition. Preservation of at least one internal iliac artery is important to prevent post-repair hypogastric ischemia. There are no previous reports of IgG4-related disease (IgG4-RD) as etiology of these aneurysms. A 32-year-old male patient presented with a left lower abdominal lump and was found to have a left external iliac artery aneurysm on computed tomography angiography. The patient underwent iliofemoral bypass with an 8 mm polyester graft. Histopathological examination of the aneurysm wall suggested IgG4-RD. The patient fulfilled the 2020 Revised Comprehensive Diagnostic Criteria for IgG4-RD.
