São classificadas de acordo com a extensão do dano, e a angiotomografia computadorizada tem as maiores sensibilidade e especificidade para identificar o grau de lesão e potenciais lesões associadas. As estratégias terapêuticas para LTA são baseadas no tipo de lesão, na extensão e nas lesões associadas. Pode auxiliar na definição de conduta também o grau de estabilidade do paciente, podendo ser manejo cirúrgico convencional, endovascular (TEVAR) ou conservador em casos selecionados. Entre os pacientes com anatomia vascular adequada, a cirurgia endovascular está associada a melhor sobrevida e a menos riscos. O objetivo deste artigo foi descrever uma série de quatro casos acompanhados em serviço terciário, em um estado com poucos serviços de alta complexidade. A terapêutica endovascular foi empregada como método preferencial. Possui prevalência em mulheres e normalmente está associada a outras variações anatômicas, como o nervo laríngeo não recorrente, presente em 86,7% dos casos. Em sua maioria, a artéria subclávia direita aberrante não apresenta sintomas. Descrevemos essa alteração em uma paciente de 82 anos, hipertensa e assintomática, que havia sido submetida a uma angiotomografia toracoabdominal para a avaliação de uma dissecção crônica tipo III (DeBakey) associada à dilatação de aorta descendente. No achado, a artéria subclávia direita aberrante apresentava percurso retroesofágico associado a um divertículo de Kommerell. The grayscale median (GSM) value is one of the most widespread methods of studying atherosclerotic plaques and it is therefore important to comprehensively standardize image post-processing. Post-processing was performed using Photoshop 23.1.1.202. Images were standardized by adjusting the grayscale histogram curves, setting the darkest point of the vascular lumen (blood) to zero and the distal adventitia to 190. Posterization and color mapping were performed. A methodology that presents the current state of the art in an accessible and illustrative way should contribute to the dissemination of GSM analysis. Current practice involves use of pre-test probability prediction rules. Several strategies to optimize this process have been explored.</p><p><strong>Objectives: </strong>To explore whether application of the pulmonary embolism rule-out criteria (PERC rule) and age-adjusted D-dimer (DD) would have reduced the number of computed tomography pulmonary angiography (CTPA) examinations performed in patients with suspected PE.</p><p><strong>Methods: </strong>A retrospective cross-sectional study of adult patients taken for CTPA under suspicion of PE in 2018 and 2020. The PERC rule and age-adjusted DD were applied. The number of cases without indications for imaging studies was estimated and the operational characteristics for diagnosis of PE were calculated.</p><p><strong>Results: </strong>302 patients were included. PE was diagnosed in 29.8%. Only 27.2% of 'not probable' cases according to the Wells criteria had D-dimer assays. Age adjustment would have reduced tomography use by 11.1%, with an AUC of 0.5. No Brasil, dados acerca da incidência dessas complicações e das variáveis associadas com esse desfecho são escassos. Objetivos Determinar a incidência de complicações mecânicas e de falhas de acessos venosos centrais de curta permanência realizados no serviço de Cirurgia Vascular de um hospital de ensino, além de identificar as variáveis associadas com a sua ocorrência. Métodos Tratou-se de uma coorte prospectiva com 73 tentativas de acesso venoso central realizadas em pacientes assistidos pelo serviço de Cirurgia Vascular de um hospital de ensino entre julho e outubro de 2022. Resultados Complicações mecânicas ocorreram em 12 tentativas de acessos (16,44%), e falhas, em 10 tentativas (13,70%). Os fatores associados às complicações mecânicas foram menor experiência do operador (p < 0,001), menor grau de especialização do operador (p = 0,014), falha na tentativa de acesso precedente à solicitação de auxílio ao serviço de Cirurgia Vascular (p = 0,008) e presença de pelo menos dois critérios de dificuldade (p = 0,007). Conclusões A incidência local de complicações mecânicas e de falhas dos acessos venosos centrais foi semelhante à descrita na literatura internacional, mas foi superior à descrita em trabalhos brasileiros. Given the proximity of the ruptured site to the superior mesenteric artery (SMA) and renal arteries, a ChEVAR was indicated. Catheterization of the target visceral vessels was a challenging procedural step because of an intensely tortuous thoracic aorta. This hostile aortic anatomy also inhibited exchange for a super stiff guide-wire and selective cannulation with the diagnostic catheter was repeatedly lost when guidewire exchange was attempted. To overcome this issue, a 5 x 40 mm balloon catheter was placed 3cm into the target arteries. The balloon was then inflated below the nominal pressure limit enabling safe exchange for a super stiff guidewire and placement of three 90-cm long 7Fr guiding sheaths. Objectives In this study, our objective was to investigate the presence of seasonal variation in deep vein thrombosis (DVT) and assess the location of the thrombus. Methods Out of 8177 patients admitted to two university hospitals and referred to outpatient clinics, we included a total of 611 consecutive patients (316 females, 295 males) diagnosed with acute deep vein thrombosis in this retrospective study. The mean age of the patients was 59.35±18.49 years, ranging from 1 to 96 years. Patients were categorized into four groups by age. Results DVT was found to be more frequent in the summer (n = 190 or 31%, p = 0.003) and specifically in August (n = 65 or 10.6%, p = 0.014), while it was least frequent in the spring (n = 128 or 20.9%) and in May (n = 40 or 6.5%). However, when comparing seasons, no significant differences were observed in terms of seasonal variation (p = 0.062) or monthly variation (p = 0.143). Conclusions Contrary to previous studies, this study demonstrated a higher occurrence of DVT during the summer, particularly in August. However, it did not reveal a clear seasonal pattern. This condition has a low incidence, being more common in thin young women. Nutcracker syndrome is compression of the left renal vein between the superior mesenteric artery and the aorta. Both entities are rare, and their coexistence has been reported in a few cases. Conservative treatment targeting weight gain is sufficient in most cases. An association between the superior mesenteric artery syndrome and acute pancreatitis has rarely been reported. We intend to describe the case of an 18-year-old girl who was admitted to the emergency room with epigastric pain and emesis. Our investigation revealed acute acalculous pancreatitis. During work-up, we discovered superior mesenteric artery syndrome and a compressed left renal vein. All patients have undergone open surgical interventions on the arteries of the aorto-iliac and femoral-popliteal segments. During these interventions, intraoperative specimens were obtained from the vascular wall with atherosclerotic lesions. The following values were evaluated: VEGF А165, PDGF BB, and sFas. Samples of normal vascular wall were obtained from post-mortem donors and used as a control group.</p><p><strong>Results: </strong>The levels of Bax and p53 were increased (p<0.001) in samples from arterial wall with atherosclerotic plaque, while sFas values were reduced (p<0.001), compared to their levels in control samples. Values of PDGF BB and VEGF A165 were 1.9 and 1.7 times higher in atherosclerotic lesion samples (p=0.001), in comparison with the control group.
