The incidence of anxiety was 28.9%, and that of depression 21.1%. The total scores of uncertainty in illness, uncertainty, complexity, lack of information, and unpredictability were positively correlated with the scores of anxiety and depression (P<0.01). The total scores of uncertainty in illness, complexity, and lack of information were negatively correlated with confronce (P<0.01). The total scores of uncertainty in illness, uncertainty, lack of information, and unpredictability were positively correlated with avoidance (P<0.01). Haishan","doi":"10.3760/CMA.J.ISSN.1007-1245.2019.21.022","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1007-1245.2019.21.022","url":null,"abstract":"Objective \u0000To explore the efficacies and onset times of high frequency repetitive transcranial magnetic stimulation combined with fluoxetine versus fluoxetine alone for post-infarction depression. \u0000 \u0000 \u0000Methods \u0000Forty patients with post-infarction depression who met the inclusion criteria were selected from April, 2017 to March, 2019 in the Department of Rehabilitation Medicine of our hospital. They were randomly divided into a treatment group and a control group, with 20 cases in each group. On the basis of routine treatment, the control group was treated with fluoxetine, while the treatment group with high frequency rTMS and fluoxetine. Repeated transcranial magnetic stimulation therapy was given for 4 weeks, 5 times a week. Hamilton Inhibition Scale (HAMD) and ability of daily life (ADL) were used to evaluate the depressive symptoms and life dependence before and after the treatment in the two groups. \u0000 \u0000 \u0000Results \u0000After the treatment, the HAMD scores of both groups decreased significantly, and decreased more in the treatment group than in the control group (all P<0.001). The ADL of the treatment group was improved from the 2nd week, while that the control group from the 4th week (P<0.001). The observation group took laryngeal mask anesthesia before the operation, and the control group tracheal intubation anesthesia. The anesthetic effects, recovery times, extubation times, and adverse reactions of the two groups were compared and analyzed. The levels of HR and MAP in each anesthesia control point of the two groups were compared. \u0000 \u0000 \u0000Results \u0000There were no statistical differences in MAP between the two groups after entering the operating room and after anesthesia intubation (both P>0.05). The MAP of the observation group after anesthesia intubation and at the end of the operation were (80.5±5.9) mmHg and (81.1±5.9) mmHg, which were significantly lower than those of the control group; and was (80.4±5.5) mmHg during the operation, which was higher than that of the control group; with statistical differences (all P 0.05). The HR of the observation group when the operation was over and after extubation were (147.5±10.7) beats/min and (144.8±9.6) beats/min, which were lower than those of the control group; and was (152.6±18.1) beats/min after anesthesia intubation, which was higher than that of the control group; with statistical differences (all P<0.05). The total anesthesia effective rate was 95.00% in the observation group, and 70.00% in the control group (P<0.05). The recovery time and extubation time of the observation group were (12.5±3.8) min and (2.1±0.7) min, which were significantly shorter than those of the control group (both P<0.05). In the control group, the traditional check-up method was used to verify the appointment. The experimental group paid attention to the WeChat public number \"X Micro Health\" to make an appointment. The patients' good psychological nursing was maintained before and after operation; health education and humanized and individualized clinical nursing during operation were adopted; and the patients' treatment compliance was enhanced. The effect of pre-, intra- and post-treatment nursing on clinical efficacy was analyzed. \u0000 \u0000 \u0000Results \u0000In the 100 patients, the total recovery rate was 95%; the effective rate was 98%; and the recurrence rate was 8%. \u0000 \u0000 \u0000Conclusion \u0000Ira photodynamic therapy for condyloma acuminatum is a new method with good curative effect and a low recurrence rate. The experimental group were treated with radiotherapy and zoledronic acid, and the control group with radiotherapy. The effective rate, adverse reactions, quality of life assessment, and life span were observed in both groups. \u0000 \u0000 \u0000Results \u0000The effective rate of the experimental group was significantly higher than that of the control group (90.3% vs. 67.7%). The pain scores of the experimental group was significantly lower than those of the control group 7 and 28 days after the treatment (both P<0.05). The pain score was significantly lower 7, 14, and 28 days after than before the treatment in the experimental group; while the pain score was lower 14 and 28 days after than before the treatment in the control group (all P<0.05). The scores in the field of psychology were significantly higher after than before the treatment (P<0.05). The physical, psychological, social relationship, spiritual pillar/religion/personal belief, and quality of life assessment scores were higher after than before the treatment in the experimental group; the Physiology, psychological, social relationship and quality of life assessment score were higher after than before the treatment in the control group (all P<0.05). The control group was treated with levofloxacin, and the study group was treated with heparin on the basis of the control group. The incidence of adverse reactions, WBC and CRP levels, PCT and ESR levels, and inflammatory factors levels were compared between the two groups. \u0000 \u0000 \u0000Results \u0000There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). There were no statistically significant differences in WBC and CRP levels between the two groups before treatment (P>0.05). After treatment, the WBC and CRP levels in the study group [(4.1±2.2)×109/L, (5.1±2.3)mg/L] were lower than those in the control group (P 0.05). After treatment, the PCT and ESR levels in the study group [(0.8±0.2)μg/L, (19.3±3.2)mm/h] were lower than those in the control group (P 0.05). After treatment, the TNF-α, IL-6, and TG levels in the study group [(16.1±6.3)μg/L, (35.1±8.5)ng/L, (4.5±2.3)mmol/L] were significantly lower than those in the control group (P<0.05). \u0000 \u0000 \u0000Conclusion \u0000Levofloxacin combined with heparin can significantly improve WBC and CRP levels, PCT and ESR levels in hemodialysis patients with catheter infection, which can reduce inflammatory factors levels, improve safety, and improve patients’ prognosis. Hemodialysis is the main alternative treatment after CKD patients enter the end stage. Although hemodialysis prolongs the patients life span, it prones psychological disorders as well. In order to understand the mental health level of hemodialysis patients, we investigated the mental health status of anxiety and depression of hemodialysis patients in the hemodialysis room of our hospital, and the correlation of anxiety and depression with coping styles. \u0000 \u0000 \u0000Methods \u0000255 hemodialysis patients were investigated with general information questionnaire, GAD-7 item evaluation form, PHQ-9 item evaluation table, and medical coping style questionnaire. \u0000 \u0000 \u0000Results \u0000The incidences of anxiety and depression were 27.06% and 40.00% in the hemodialysis patients, respectively; and were higher in the patients undergoing hemodialysis< 1 year than in those ≥1 year. It was found that there was a negative correlation of facing the situation with anxiety and depression in the hemodialysis patients (P<0.05), and a positive correlation of avoiding and yielding with anxiety and depression (P<0.05). \u0000 \u0000 \u0000Conclusion \u0000The incidences of anxiety and depression in hemodialysis patients in our center are high, and highly relates to dialysis time. Yufeng","doi":"10.3760/CMA.J.ISSN.1007-1245.2019.12.026","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1007-1245.2019.12.026","url":null,"abstract":"Objective \u0000To explore the clinical characteristics of giant cell tumor (GCT) in hands and feet, and to evaluate the effect of surgical treatment. \u0000 \u0000 \u0000Methods \u0000The clinical data of 11 patients with GCT in hands and feet treated at our hospital from 1998 to 2012 and followed up, 5 male and 6 female, were retrospectively analyzed. They were 15-41 years old, with an average of 23.5. Among the 11 cases, 4 had the tumor in hands, and 7 in feet. All the patients had pain, swelling, movement restriction on the lesion part, and came to hospital 3-14 months (average 8 months) after the sighs of the symptoms. No case got Campanacci’s grade I, 6 grade Ⅱ, and 5 gradeⅢ. One case had toe excision, and the rest were treated with extensive curettage and bone graft. \u0000 \u0000 \u0000Results \u0000The patients were followed up 16-120 months (average 49 months) after surgery. One of them had local recurrence after the first resection, and the metacarpal bone was resected and followed up 3 years, and found no recurrence; the rest patients had no pain and other complications after the surgery. The postoperative pathological results showed that all the patients were GCT with no malignancy, multiple center lesion, and lung metastasis. \u0000 \u0000 \u0000Conclusion \u0000GCT in hands and feet is rare, and tend to be misdiagnosed. According to time, the control group was from July, 2016 to May, 2017 and took traditional training and assessment methods; and the observation group was from June, 2017 to March, 2018. In the observation group, a WeChat combining Wenjuanxing platform management team was established; the training team arranged the content of monthly business learning, and uploaded the content to the WeChat platform after each business study; and each clinically responsible nurse could uploaded the problems discovered or existed at work to the WeChat platform through videos, pictures, etc. for learning and discussion. The team leader gave a solution after the WeChat consultation. At the end of each month, the team used the design questionnaire to conduct assessment, summary, and analysis. \u0000 \u0000 \u0000Results \u0000Compared with those in the control group, the first passing rate was higher and the theoretical examine time was significantly shorter in the observation group (both P<0.001). The first passing rates of theoretical examination were significantly higher in N0 (P<0.001) and N1 level (P < 0.001) nurses in the observation group than in the control group. \u0000 \u0000 \u0000Conclusions \u0000As new network tools, Wechat and Wenjuanxing platform are convenient and fast, and can save nurses’ time. The new nursing training tools stimulate nurses enthusiasm for professional study.
