According to the clinical diagnosis, they were divided into a bacterial infection group (44 cases) and a non-bacterial infection group (44 cases). 44 healthy people who underwent physical examination in our hospital at the same time were selected as a control group. The three groups were detected by electrochemical sandwich method. The sputum of the bacterial infection group was collected for bacterial culture. The PCT levels and positive rates of the three groups were compared; and the distribution of bacteria and the detection rate of bacterial culture in the bacterial culture group were counted. \u0000 \u0000 \u0000Results \u0000The PCT level of the bacterial infection group was higher than those of the non-bacterial infection group and the control group, with statistical differences (both P<0.05); the PCT level of the non-bacterial infection group was higher than that of control group, with a statistical difference (P<0.05). The positive rate of the bacterial infection group was higher than those of the non-bacterial infection group and the control group, with statistical differences (both P<0.05); the positive rate of the non-bacterial infection group was higher than that of the control group (P<0.05). They were treated with letrozole, 1.5 mg/m2 per day. And 30 healthy boys in the early and middle youth stage taking physical examination during the same time period were selected as a control group, and took no drug intervention. The growth characteristics and linear growth changes of the two groups before treatment and after 1 year’s treatment were compared. \u0000 \u0000 \u0000Results \u0000There were no statistical differences in bone age, age, BMI, testicular volume, and height between the study group and the control group before the intervention(all P>0.05). The control group was treated with conventional western medicine alprostadil, while the treatment group was treated with oral Dengzhan Shengmai capsule on the basis of alprostadil, 2 capsules each time, three times a day. The two groups were treated for 14 days as a cycle and 4 cycles as a treatment course. The treatment effects were compared between these two groups. \u0000 \u0000 \u0000Results \u0000The total effective rate of the treatment group was 96.7%, and that of the control group 80.0% (P<0.05). After the treatment, the ABI and TBI increased significantly in both groups (all P<0.05), and the changes of ABI and TBI in the treatment group were more significant than those in the control group (both P<0.05). After the treatment, the internal diameters of the posterior femoral artery, the posterior tibial artery and the dorsal pedal artery of the two groups increased significantly (all P<0.05). All the patients were subjected to topical anesthesia with lidocaine aerosol. In group A, dexmedetomidine was infused intravenously at 0.5 μg/kg for 15 minutes, maintained at 0.5 μg/(kg·h); and butorphanol was infused intravenously at 0.02 mg/kg 5 minutes before examination. In group B, 0.02 mg/kg butorphanol was injected intravenously 5 minutes before the examination, and plasma target-controlled propofol was given 3 μg/ml. The hemodynamic parameters, such as MAP, HR, and SPO2, as well as anesthesia effect and adverse reactions were recorded at the times of entry (T1), arrival of glottis (T2), passage of tracheal carina (T3), diagnosis and treatment (T4), and withdrawal from fiberoptic bronchoscopy (T5). \u0000 \u0000 \u0000Results \u0000At T2, T3, and T4, the mean blood pressures and the heart rates were lower in group A than in group B [(81±10) mmHg vs. (101±12) mmHg, (86±12) mmHg vs. (98±9) mmHg, (85 ±9) mmHg vs. (92±13) mmHg, (68±8) times/min vs. (98±12) times/min, (72±8) times/min vs. (88±11) times/min, and (70±9) times/min vs. (85±12) times/min; all P 0.05). Simao","doi":"10.3760/CMA.J.ISSN.1007-1245.2019.23.052","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1007-1245.2019.23.052","url":null,"abstract":"Objective \u0000To explore the application of hazard vulnerability analysis (HVA) in hospital emergency management, and to improve hospital emergency prevention capabilities. \u0000 \u0000 \u0000Methods \u0000Using the commonly used risk assessment matrix in China, 255 different positions and different personnel levels were selected to identify the potential hazards faced by the hospital; risk assessment and sequencing were carried out; and various hazard risk levels were identified. \u0000 \u0000 \u0000Results \u0000Through the hospital HVA, the top six risk factors were typhoon (46.00%), elevator accident (37.20%), public health emergency (30.00%), information system failure (27.00%), fire (26.00%), and power failure (25.50%). We identified the sudden risk events that the hospital needed to respond to and the key management projects. \u0000 \u0000 \u0000Conclusion \u0000HVA is an important basis for hospitals to identify and reduce risks. The weakest links in hospitals are identified, and the major emergencies that hospitals need to deal with are identified. Therefore, appropriate prevention and response measures are adopted in an organized, planned, and focused manner. The GEPIA database was used to analyze the relationship between the expression of NR3C1 and the survival rate of ccRCC patients in TCGA data. Protein interaction on one hand and upstream and downstream regulation of NR3C1 gene on the other hand were analyzed by the String database. \u0000 \u0000 \u0000Results \u0000The NR3C1 expression in ccRCC tissue was statistically different from that in normal renal tissue (P<0.05). The overall survival rate of the patients with a high NR3C1 expression was significantly higher than that of ones with a low NR3C1 expression (P<0.05). The instrument and equipment in ICU ward, the surface of bed unit, the diagnosis and treatment instrument, the sanitary ware, hands of medical staff and so on, were sampled. The root cause analysis method was used to find the source of infection and the route of transmission. The root causes of the outbreak of multi-drug resistant Acinetobacter baumannii were summarized and analyzed, and the intervention measures were taken. \u0000 \u0000 \u0000Results \u0000A total of 48 ICU environmental specimens were collected from 6 patients, of which 30 samples were positive showed by etiological examination, and the isolated strains were basically consistent with their drug resistance results (resistance to more than three types of antibiotics). It was inferred that the nosocomial infection of Acinetobacter baumannii in this outbreak was caused by the same pathogen contamination. At the same time, the detection results of Acinetobacter baumannii in ICU environment showed that the contamination of bed units and most of the surface of objects in the ward (the detection rate was 66%-75%), the disinfection and isolation measures not in place, and the non-standard hand hygiene of the staff were the main causes of the outbreak of nosocomial infection. \u0000 \u0000 \u0000Conclusions \u0000The main causes of the spread of multi-drug resistant Acinetobacter baumannii infection of ICU were found by root cause analysis, and the standard manual hygiene and contact isolation standards should be formulated and strictly implemented. It is an effective way to reduce the incidence of nosocomial infection of MDRAB by disinfection and sterilization measures on the surface of objects and the environment in the ward. The incidence of infection during PICC was recorded and the risk factors associated with infection were analyzed. \u0000 \u0000 \u0000Results \u0000Among 285 patients, there were 33 cases of PICC infection, with the infection rate of 11.58%. {"title":"Advances in the study of ASXL1 mutations in myeloid neoplasms","authors":"Fan Lirui","doi":"10.3760/CMA.J.ISSN.1007-1245.2019.21.052","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1007-1245.2019.21.052","url":null,"abstract":"In recent years, many studies on myeloid neoplasms have reported the occurrence of ASXL1 mutations, including myeloproliferative neoplasms (5%-35%), myelodysplastic syndrome (15%-50%), acute myelogenous leukemia (5%-15%), and so on. ASXL1 mutations include three types: frame-shift mutation, nonsense mutation, and missense mutation; among which, the first two have a great impact on the overall survival rate and disease-free survival rate of patients with myeloid neoplasms. At the same time, myeloid neoplasms patients with ASXL1 mutations have obvious clinical characteristics, and have significant differences in response to treatment and in prognosis compared with wild-type patients. The universality of ASXL1 mutation indicates that it is closely related to the occurrence and progress of myeloid neoplasms.
