In this study, we aimed to present the functional and anatomical results of 182 patients who underwent PF repair surgery and to reveal the risk factors for ED and penile curvature in a relatively long follow-up period. Thirty (16.5%) patients suffered from ED and 18 (9.9%) patients had penile curvature, respectively in a median follow-up period of 66 (39-111) months. Presence of bilateral fractures (OR = 7.806; 95% CI = 2.083-29.261; p = 0.002), larger tunical defect size (OR = 1.099; 95% CI = 1.036-1.164; p = 0.002) and longer time between fracture and surgery (OR = 1.095; 95% CI = 1.036-1.157; p = 0.001) were stated as the independent risk factors for ED. Only larger tunical defect size was found as an independent risk factor for penile curvature (OR = 1.14; 95% CI = 1.063-1.221; p < 0.001). The cut-off values for predicting ED were 18.5 mm for tunical defect size, 14.5 h for time between fracture and surgery, and 41 years for age. The cut-off value for tunical defect size was 18.5 mm for penile curvature. Risk of ED increases among patients with extended tunical rupture and delayed surgical repair. While rare, IPP reservoir migration can be significant. This systematic review aims to characterize the incidence of reservoir migration, assess migration outcomes across various placement techniques, and propose a clinical decision-making algorithm for reservoir placement. A comprehensive search of PubMed and the Cochrane Library was conducted on April 7, 2025, without filters. Eligible English-language studies reporting migration-related outcomes of functional IPP reservoirs were included. Data extraction and risk-of-bias assessments were performed using validated tools. Thirty-five studies were included: 19 case reports, 10 case series, and 6 cohort studies. A meta-analysis of proportions using a random-effects model was performed on 9 case series and 6 cohort studies (5783 patients), yielding a pooled migration rate of 0.2% (95% CI: 0.0%-0.6%) with moderate heterogeneity (I² = 54.2%). Sensitivity analyses yielded consistent results. The evidence is limited by small sample sizes, retrospective designs, and reporting variability. Migration risk is low but clinically significant and especially relevant in patients with altered pelvic anatomy. Although commonly found in the skin and soft tissues, its occurrence in the penis is extremely rare. We report the case of a 51-year-old male who presented with a painless, progressively enlarging penile mass. Imaging suggested a benign etiology, and the patient underwent surgical excision. Histopathological analysis confirmed the diagnosis of Masson’s tumor, revealing papillary endothelial proliferation within a thrombotic background. The postoperative course was uneventful, with satisfactory wound healing and no recurrence at follow-up. While rare, penile Masson’s tumor should be considered in the differential diagnosis of vascular penile lesions to avoid misdiagnosis and overtreatment. Since the late 20th century, continuous advancements in inflatable penile prostheses have been introduced by various manufacturers, focusing on enhancing usability, minimizing complications, and improving patient satisfaction. These refinements have particularly emphasized the importance of pumps, critical to device functionality and overall patient experience. Recent innovations in prosthesis technology underscore the progress made in surgical techniques and the integration of advanced materials, leading to better clinical outcomes and heightened user satisfaction. This paper reviews the historical development, technological advancements, and current trends in penile prosthesis pumps. We aimed to develop a novel rat model suitable for investigating erectile dysfunction (ED) following nerve-sparing radical prostatectomy (NSRP) and to validate its applicability. Thirty-two male Sprague-Dawley rats were randomly assigned to undergo either a novel NSRP modeling procedure or sham surgery. The surgical protocol for the NSRP group included identification and dissection of the major pelvic ganglia and cavernous nerve, prostatectomy, and reconstruction of the lower urinary tract. Mean arterial pressure (MAP) and intracavernous pressure (ICP) were assessed at 1 and 2 weeks postoperatively. Subsequently, the penis tissues were collected for histopathological analyses. The results showed that rats in the NSRP group exhibited significantly lower maximum ICP/MAP ratios (0.39 ± 0.17 vs 0.77 ± 0.08, P < 0.0001) and areas under the ICP curve (2090.13 ± 1050.15 vs 4326.08 ± 1042.08, P = 0.0001) compared to the sham-operated group at 1 week postoperatively. By 2 weeks postoperatively, the NSRP group showed a significant improvement in both the maximum ICP/MAP ratio (0.73 ± 0.09 vs 0.39 ± 0.17, P < 0.0001) and the area under the ICP curve (3654.40 ± 679.28 vs 2090.13 ± 1050.15, P = 0.005) compared to the 1-week post-NSRP group. However, the area under the ICP curve in the NSRP group remained significantly lower than that in the sham-operated group at 2 weeks postoperatively (3654.40 ± 679.28 vs 5259.10 ± 951.48, P = 0.003). Similarly, histological staining suggested that smooth muscle atrophy and collagen deposition in the penis gradually improved over time post-NSRP. However, significant differences in the ratio of smooth muscle to collagen remained between the NSRP and sham-operated groups at 2 weeks postoperatively (0.22 ± 0.03 vs 0.27 ± 0.05, P = 0.011). In conclusion, this study established a novel rat model for investigating post-NSRP ED and validated its utility through erectile function monitoring and histological analysis. There is a paucity of published data focusing on long term outcomes and quality of life reporting for patients in this setting. We contacted patients who had a post-ischaemic priapism penile prosthesis inserted in our department via telephone and conducted the previously validated Quality of Life and Sexuality with Penile Prosthesis (QoLSPP) questionnaire. Question items were answered on a Likert scale from 0-5 with satisfactory scores ≥3. Two-tailed Z-tests were used to determine satisfactory scores at significant levels (p < 0.05). We chose to add two additional questions on regret and feelings towards living the rest of their lives with a penile prosthesis. 167 patients had penile prostheses inserted post- ischaemic priapism between 2002-2022. Of these, 39 (23.4%; implants between 2007-2021) completed our questionnaire. The median age was 56 years (IQR 50-63) with a median time to questionnaire post-ischaemic priapism of 9 years (IQR 3-11). Analysing the QoLSPP mean item responses, the penile prosthesis resulted in satisfactory scores in 7 of 16 QoLSPP questionnaire items: device adequacy, device rapidity, device duration, meeting expectations, contentment with life, general well-being and sexual experience. The pooled mean score by domain was 3.7 ± 1.2 for functional, 3.3 ± 1.4 for relational, 3.2 ± 1 for social and 3.4 ± 1.1 for personal. Sub-group analysis demonstrated no significant difference in mean score by prosthesis type (inflatable versus malleable). Ten respondents cited specific reasons for how the penile prosthesis caused dissatisfaction. All 39 respondents (100%) answered 'yes', they did not regret penile prosthesis insertion as a treatment option and that they would be satisfied living the remainder of their lives with a device in situ. We aim to explore and compare socio-demographic and clinical characteristics of men with self-reported DE and primary premature ejaculation (PE). Data from 555 consecutive men seeking first medical help for DE or primary PE were retrospectively analyzed. Socio-demographic, clinical, laboratory data, and self-reported symptoms of anxiety and depression were collected. Patients completed International Index of Erectile Function (IIEF) and Beck Depression Inventory (BDI). We excluded men who had undergone pelvic surgery. Descriptive statistics compared clinical and socio-demographic characteristics. Linear regression models tested the association between depressive symptoms and baseline IIEF domain scores among men with DE. Of 555 patients, 479(86.3%) and 76(13.7%) had PE and DE, respectively. Men with DE were significantly older than those with PE [47(34-63.5) vs. 44(33-53), P = 0.01]. Conversely, men with PE depicted a CCI ≥ 1 more frequently compared to those with DE [17(22.4%) vs. 53(11.1%); P = 0.01]. The two groups did not differ regarding relational status, BMI, waist circumference, and hormonal milieu. Men with DE reported more frequently symptoms referable to anxiety and depression (P = 0.001) along with higher median (IQR) baseline BDI scores [8(4.8-14.3) vs. 6(2-10); P = 0.01], lower median IIEF-orgasmic function (OF) [6(5-9) vs. 9(6-10); P = 0.003] and IIEF-sexual desire (SD) [7(5.8-8) vs. 8(6-9); P = 0.001] domain scores compared to men with PE. At linear regression analysis, in men with DE, the higher the BDI score the lower the IIEF-overall satisfaction [β = -1.57, CI(-2.60, -0.53), P = 0.004], and the lower the IIEF-intercourse satisfaction [β = -0.76, CI(-1.29, -0.16), P = 0.01] domain scores. Conversely, no association between BDI and IIEF-OF, -SD, and -erectile function domain scores were found. Of men complaining of ejaculatory disorders, one out of ten reports DE.
