The presence of symptoms and paraneoplastic syndromes, such as myasthenia gravis, along with the histological subtype and stage of the tumor, are important factors that guide the treatment approach and prognosis for patients with thymomas. The prognosis for thymoma varies depending on several factors, including the stage and type of the tumor, as well as the presence of associated autoimmune diseases. Generally, early-stage thymomas have a better prognosis compared to advanced-stage tumors. Mastoidectomy and tympanoplasty are common surgeries in the head and neck area, in which profound and long-term analgesia is essential. Objectives: The present study investigated the effect of the combination of topical morphine, triamcinolone, and lidocaine compared to the control group. Patients and Methods: In the current clinical trial, 68 patients’ candidates for mastoidectomy-tympanoplasty surgery were included and randomly divided into intervention and control groups. The variables of demographic, pain, hemodynamics, extubation time, and received opioids were measured. Results: The results of the current study indicated a significant reduction in the patient’s pain scores in both groups (P=0.001), while the patients of group 1 had significantly lower pain scores than group 2 after 8, 12, and 24 hours in the ward (P<0.05). Moreover, a significant reduction was observed in systolic blood pressure (SBP), diastolic blood pressure (DBP), and mean arterial pressure (MAP) in all patients during surgery (P<0.001). However, in patients who received morphine, triamcinolone, and lidocaine (group 1), SBP and MAP were significantly lower than group 2 after 40, 80, 100, 120, 140, and 160 minutes during surgery (P<0.05). Conclusion: The present study indicated that the combination of morphine, triamcinolone, and lidocaine compared to the control group could lead to better pain control, further reduction of SBP and MAP, reduction of post-operative opioid use, and delay during the first opioid administration. SSc affects various organs, and one of the most important of these organs is the cardiovascular system, which is one of the complications of this syndrome, including pulmonary arterial hypertension (PAH). Early diagnosis of this complication is very important due to the importance of the effect of PAH on mortality and morbidity of patients. For diagnosis of PAH, two methods of computed tomography (CT) scan and echocardiography are conducted. Objectives: In this study, we aimed to evaluate the agreement coefficient of chest CT scan in comparison with echocardiography in the diagnosis of PAH in patients with SSc. Patients and Methods: In a diagnostic study, which was conducted in Loghman hospital (Tehran-Iran), patients with a diagnosis of SSc were evaluated for pulmonary artery pressure (PAP) by echocardiography. For patients, a CT scan of the lungs was conducted and PAP was assessed. The results that obtained from the lungs CT scan were compared with the echocardiography results. Results: Fifty patients with SSc were evaluated. Eighty-four percent of patients were female and the mean age of all patients was 48.94 ± 11.02 years. About 16% of all patients had high PAP based on echocardiography and 28% of patients based on CT scan. Kappa’s agreement coefficient was 0.428 and Spearman’s correlation coefficient was 0.457. In other words, chest CT scan has a partly agreement with the echocardiography method (P value = 0.001). Materials and Methods: A cross-sectional study was conducted from April 2019 to April 2020, including all patients who required sterile culture at AL Zahra and Kashani hospitals. Specimens were cultured in BACTEC and subjected to standard phenotypic methods. Subsequently, 16S rRNA gene sequencing was performed to detect gram-negative non-fermenting Bacillus bacteria at the genus and species level. A comparative evaluation was then conducted. Results: The study identified 30 bacteria. The 16S rRNA gene sequencing method observed that 83.3% of the diagnoses were Acinetobacter baumannii, while the phenotypic approach identified 86.7% as Acinetobacter. The specimens were injected into BACTEC™. Positive cultures identified as non-fermenting gram-negative bacteria by the phenotypic method were included in the study and then evaluated using the Phoenix method to determine the genus and species of non-fermenting gram-negative bacteria. A comparison was then conducted. Results: The study identified 30 non-fermenting gram-negative bacteria. The presentation of these markers can guide the treatment in breast cancer patients because of their predictive value in hormonal therapy responsiveness and prognosis. Objectives: This study investigated the correlation between estrogen and PR expression with prognostic parameters of breast cancer, including age, grading, staging, tumor size, and lymph node spread. Patients and Methods: This cross-sectional study included 245 cases of breast tumors. Characteristics of patients, including age, the status of ER and PR biomarkers, tumor type, pathological features of cancer, and staging, were documented from case files. Accordingly, SPSS version 27 software was conducted for data analysis. The collected data were analyzed using the Spearman’s correlation, Mann-Whitney U and chi-squared tests. A P value < 0.05 is considered statistically significant. Results: High ER and PR expression (>50%) were found in 56.7% and 40.4% of cases, respectively. Most of the tumors in patients belonged to grade 2 (56.3%) and staging IIA, IIB, and IIIA. We found a significant correlation between ER expression, breast cancer grading, and patients’ age. PR expression was significantly associated with breast cancer staging, grading, size, and lymph node spread. Objectives: The aim of the current study was to assess the effect of hemoperfusion in COVID-19 patients with respiratory failure. Patients and Methods: In this prospective study, a total of 98 patients over the age of 18 with the positive COVID-19 polymerase chain reaction (PCR) test were investigated. The patients were divided into two groups; a control group consisting of 47 patients who did not receive hemoperfusion, and an exposed group consisting of 51 patients who met the criteria for hemoperfusion. Various parameters including complete blood cell counts, serum bilirubin, creatinine, C-reactive protein (CRP), and interleukin 6 (IL-6) levels were evaluated in all patients. Results: The results of our study revealed a statistically significant difference in intensive care unit (ICU) admission between the two groups. Hospitalization time (19.941±1.75 versus 14.615±1.39, P=0.021) and ICU time (14.98±1.30 versus 9.62±1.15; P=0.003) were significantly higher in patients who received hemoperfusion. Regarding the mortality rate, only 36.7% of the patients survived; however, there was no significant difference observed between the two groups (P=0.34). Conclusion: In conclusion, the findings of our study indicate that hemoperfusion in COVID-19 patients with respiratory failure led to a significant increase in hospital stay and ICU stay compared to those without hemoperfusion. In leukemia, ferritin levels increase with disease stages and can predict survival after transplantation. Leukemia-induced dysregulation of the ferroportin-hepcidin regulatory axis results in intracellular iron accumulation, activation of the reactive oxygen species (ROS)-related signaling pathway, and inhibition of hematopoiesis, leading to anemia and increased dependence on red blood cell transfusions, thereby disrupting the physiological cycle. Additionally, ferritin levels rise in other types of cancer, indicating its potential as a target for both diagnosis and treatment. HC can be found anywhere in the body, however it is a rare finding in the heart, and its recurrence in the heart and pericardium is very rare. A 54-year-old woman who had undergone surgery for the removal of HC in another country 11 years before presented with shortness of breath and heaviness in the chest. The examinations and computerized tomography (CT) scan revealed a large mass measuring 60×50 mm in the pericardium cavity originating from the right atrial wall. Infection with Echinococcus granulosus was confirmed with serological enzyme-linked immunosorbent assay (ELISA) test and the patient underwent cardiac surgery. The patient underwent cardiopulmonary bypass, nevertheless did not suffer cardiac arrest, and aortic cross-clamping was not performed for her. The removed mass was filled with daughter cysts. The endocysts were removed and intramural cysts were drained. Following the surgery, the patient received treatment with albendazole for six months. Therefore, placental ultrasound findings influence the diagnosis of fetal growth restriction (FGR). Objectives: This study examines the relationship between placental thickness, grading and heterogeneity during ultrasound and placental pathology in third trimester FGR fetuses and its effect on neonatal outcome. Patients and Methods: This prospective observational study included 67 pregnant women with FGR fetuses in the third trimester of pregnancy with the mean gestational age of 34.52 ± 2.65 weeks and possible termination of pregnancy within the next seven days. The placental thickness, grading, and heterogeneity in ultrasonography were examined and compared with thickness and grading. Their effect was also considered in neonatal outcome and the results were analyzed. Results: In this study, there is a significant relationship between the thickness of the placenta before and after birth with the weight of the placenta, weight and Apgar score, neonatal arterial pH at birth and fetal growth restriction grading in ultrasound, and neonatal outcome. Conclusion: This study shows that ultrasonic placental findings can be effective in determining the final weight of the placenta and fetus and also the final outcome of the baby at birth.
