Clinical manifestations include painful polyneuropathy with autonomic and orthostatic symptoms. Corneal confocal microscopy (CCM) is one of the promising non-invasive methods for DN evaluation and tracking the effectiveness of treatment. Objectives: To perform the early diagnosis of DN in T1DM patients and describe the morphological changes in nerve fibers during the treatment process. Patients and Methods: This prospective study included 77 T1DM patients and 42 healthy controls. All patients underwent CCM and electroneuromyography (ENMG). Twenty-five patients with T1DM and HbA1c 7-10% underwent strict glycemic control with the correction of insulin therapy and the subsequent CCM and ENMG investigations for the second time. Results: In T1DM patients a decrease in the main nerve trunks with age was revealed, as well as an increase in corneal nerve tortuosity (P<0.05). The number of branches of nerve fibers did not change with the increasing age (P=0.07). After reaching T1DM compensation, patients underwent a course of alpha-lipoic acid therapy. Confocal microscopy of the cornea parameters did not undergo significant changes during therapy since significant positive dynamics were noted only in the tortuosity of nerve fibers (P<0.05), which decreased by 11.8%. Almost all patients with Hashimoto’s thyroiditis have high levels anti-thyroid peroxidase (anti-TPO) antibodies. Objectives: This study aimed to evaluate the relationship between ultrasonography and fine needle aspiration (FNA) results in anti-TPO antibodies positive and negative patients with nodular goiters. Patients and Methods: This cross-sectional study included 128 patients with nodular goiters, referred to endocrinology clinic of imam Reza hospital, Tabriz, Iran. Anti-TPO levels above and below 16 IU/mL were considered as positive and negative, respectively. All patients underwent thyroid ultrasonography, and eligible nodules were subjected to FNA. Results: Of 128 patients, 33.6% and 66.4% were anti-TPO positive and negative, respectively. FNA was conducted on 196 nodules. A significant relationship was observed between sonographic and FNA results in low and intermediate-suspicion nodules. Chronic lymphocytic thyroiditis (CLT) was more frequently reported in low-suspicion nodules of the anti-TPO positive group (P≤0.0001). In addition, in intermediate-suspicion nodules, CLT was reported in 33.3% of patients in the anti-TPO positive group (P=0.026). No significant difference was observed between other nodules. This complication was called post-COVID-19 syndrome and was recognized by the World Health Organization, which was reflected in the Delphi Consensus on October 6, 2021. The clinical characteristics of this disease include a significant number of immunological manifestations, including fever, arthralgia, myalgia, flu-like symptoms, as well as autoimmune diseases manifestation. These symptoms may allude the autoimmune genesis of the COVID-19 complications. Confocal microscopy of the cornea (CCM) may be a promising method for early neuropathy verification. To study the possibility of the small fiber neuropathy evaluation using the CCM in a patient with the post-COVID-19 condition. The patient is a female of 61 years old, which suffered from a COVID-19 infection in October 2020. After a month, she noted the presence of myalgia and polyneuropathy. A year after the onset of the disease, in addition to myalgia, headaches, neuropathy pain, the patient also experienced bowel disorders and stomach aches, which cannot be explained with an alternative diagnosis. The patient underwent CCM to assess the structure of nerve fibers to prove the presence of the small fiber neuropathy as a complication of the COVID-19 infection. The presence of the small fiber neuropathy was evaluated in the patient as well as enlarged Langerhans cells in the cornea. Considering the pivotal role of these cells in the immunological processes, the \"activated\" dendritic cells in post-COVID-19 patients may serve as another evidence of the autoimmune nature of this complication and possibly, a method for the monitoring treatment effectiveness of this disease. Recently, a few studies noted that inadvertent incorrect injection of COVID-19 vaccines may lead to the development of serious adverse events. Herein, we report a case of inadvertent intravenous COVID-19 vaccine injection who developed severe anaphylactic shock following receiving the COVID-19 vaccine. Cigarette smoke is produced by burning of complex organic compounds that include highly toxic components. Passive smokers are non-smoker individuals that are exposed to second-hand smoke (SHS) or environmental tobacco smoke (ETS). Passive or involuntary smoking is currently a major public health dilemma worldwide. Considering the increasing number of passive smokers in today’s world, it is imperative to assess the potential adverse effects of passive smoking on oral health and periodontal tissue. Objectives: This study aimed to assess the correlation between exposure to ETS and periodontitis in passive smokers. Methods: In this systematic review, the Google Scholar, PubMed and Scopus databases were searched for relevant articles published from 1990 to 2020. The extracted data were tabulated. Results: Search of the literature yielded 77 articles based on the searched keywords. After assessing the abstracts and excluding the duplicates, 68 articles remained. After accurate assessment by the examiners, 21 articles were selected for the full-text review. Finally, seven eligible articles remained in the study for the final review. Conclusion: Cigarette smoke affects the immune system and aggravates periodontal destruction as such. We describe successful benralizumab monotherapy in asthma-dominant EGPA. Sixteen years ago, a 38-year-old male was diagnosed with multisystemic illness with asthma, arthralgias, Bell’s palsy and nephrotic syndrome, with 20.3% eosinophilia (absolute count 1160 K/µL), ESR 50 mm/hour, albumin 2.3 g/dL, serum creatinine 1.1 mg/dL, slightly above baseline and negative immunology work. Kidney biopsy demonstrated diffuse podocyte foot process effacement with minimal change disease, without vasculitis and EGPA was diagnosed. He achieved early remission with corticosteroids and intravenous cyclophosphamide and was maintained on prednisone and mycophenolate mofetil. Nevertheless, he repeatedly experienced severe asthma exacerbations, falls in FEV-1% and recurrent relapsing eosinophilia. He was successfully started on benralizumab and was weaned off mycophenolate mofetil and prednisone. Eosinophilia was corrected and FEV-1% normalized. It has a diverse clinical course and unpredictable prognosis. Traditionally, clinical and pathological features are conducted to prognosticate the outcome of the disease. Recently, attention has been directed towards other indices that may have prognostic implications for IgAN progression. The Ki-67 index have been extensively detected as a proliferation marker of malignant cells for several years. Objectives: In this study, we aimed to determine the prognostic impact of Ki-67 expression in IgAN biopsies. Patients and Methods: This cross-sectional study was conducted on 18 biopsy-proven IgAN patients. IgAN was diagnosed by significant IgA deposits and according to the morphologic variables of Oxford (MEST) classification for this disease. The expression of Ki-67 in the glomeruli, interstitium and tubules was described as the intensity percentage of the immunohistochemical staining. Results: In all of the eighteen cases with biopsy-proven IgAN, the mean ± SD of age, degree of proteinuria and serum creatinine were 36.3±17 years, 1600±733 mg/day and 1.2±0.4 mg/dL, respectively. In this study, we found a significant correlation between glomerular Ki-67 immunohistochemical staining and MEST scores (r: 0.780, P< 0.001). We also detected a significant correlation between interstitial Ki-67 staining and MEST scores (r: 0.700, P=0.001). Moreover, Ki-67 immunohistochemical staining of the interstitium had a significant relationship with IgM deposits (r=0.544, P=0.02). Conclusion: In this pilot study, we found that interstitial and glomerular Ki-67 immunohistochemical staining had positive and significant associations with MEST scores. Our data also showed that interstitial Ki-67 staining had a significant relationship with IgM deposits. It has been demonstrated that supplemental oxygen used in the time of exercise testing has a considerable positive effect in cases with COPD. Objectives: Present study was aimed to investigate the possible effects of supplemental oxygen on the outcomes of 6-minute walk test (6MWT). Patients and Methods: COPD cases were randomly divided into two groups (n= 50/each). Group1; patients who received nasal oxygen during the 6MWT, and group two were patients who did not receive supplemental oxygen during the test. Results: The Shapiro-Wilk test showed that the distribution of all parameters in two groups followed the normal distribution. The total distance walked was 431.54±40.76 m in the intervention group and 399.08±49.94 m in the control group, with a significant difference between the two groups (P= 0.001). After 6MWT, the mean of SpO2 in the intervention group was significantly higher than the control group (P= 0.002) and the degree of dyspnea was significantly lower than the control group (P= 0.031). Conclusion: Overall, supplemental oxygen has significant positive effects in COPD patients, but definitive commentary is needed for further studies.
