Identifying the risk factors for patients with a higher risk of vision loss is crucial.</p><p><strong>Objective: </strong>To identify the risk factors for urgent referrals in diabetic patients through an ophthalmic examination program.</p><p><strong>Material and methods: </strong>Specific criteria for urgent referral were established to facilitate attendance. Risk factors related to urgent referral were evaluated in a multiple logistic regression model.</p><p><strong>Results: </strong>A total of 1,265 patients were screened. 678 (54%) had DR, 245 (19.4%) had diabetic macular edema, and 124 (10%) had visually significant cataracts. 399 (31.4%) patients needed urgent referral. Significant risk factors for urgent referral were 10 to 20 years from diabetes diagnosis (OR = 5.71, p < 0.001), more than 20 years from diabetes diagnosis (OR = 15.36, p < 0.001), high systolic blood pressure (OR = 1.66 per mmHg, p = 0.003), and high HbA1c (OR = 1.15, p = 0.002), after adjusting for sex and age.</p><p><strong>Conclusions: </strong>A third of the patients require urgent referral. Main causes of urgent referral were DME, severe or worse DR, cataract and high intraocular pressure. Results from a nationwide survey among Mexican adults.","authors":"Wrzecionkowska Dagmara, Stephens R Christopher, Gutiérrez-Reyes Juan Pablo","doi":"10.24875/GMM.24000279","DOIUrl":"https://doi.org/10.24875/GMM.24000279","url":null,"abstract":"<p><strong>Background: </strong>Mexico is one of the countries with the highest case-fatality ratio due to COVID-19.</p><p><strong>Objective: </strong>Explore the reasons for not getting vaccinated against COVID-19.</p><p><strong>Material and methods: </strong>A nationwide telephone survey, comprising all 32 states of Mexico, was conducted to examine associations between inoculation against COVID-19 and other characteristics, the chi-square test and logistic regression analysis were applied.</p><p><strong>Results: </strong>Out of 3 126 adults, 68% reported complete vaccination, 21% the first dose, 11% remained unvaccinated. Using logistic regression models: place of residence -Other central state and State of Mexico-, being single, lower education level, younger age, lower BMI, not getting tested for COVID-19, and the perception of COVID-19 as unreal and not severe, were all significantly associated with non-vaccination, i.e. not receiving any dose of anti-COVID vaccine. The predominant reasons for not getting vaccinated were: 63% \"external barriers\" (e.g., not being able to attend an appointment), and 37% \"internal motives\" (e.g., considering that the vaccine does not work).</p><p><strong>Conclusions: </strong>The causes of non-vaccination against COVID-19 are related to social and geographical determinants. Addressing external barriers is necessary to promote equity in vaccination. A cascade of access to the public health system was constructed. Multivariate regression models were performed to identify related factors.</p><p><strong>Results: </strong>43.33%, 40.85%, and 43.79% of older adults had access to the public health system in 2018, 2021, and 2022, respectively. In 2018, frailty increased 2.419 times the probability of having access. While, being married or in union, being literate, and living in an urban residency increased access in 2021 and 2022.</p><p><strong>Conclusions: </strong>There are persistently low levels of public healthcare access among older Mexican adults. Frailty elders had more probability of having access in 2018. Seguro Popular might have promoted access by overcoming organizational obstacles from the public system and surpassing sociodemographic barriers. {"title":"Burden of disease in older adults in Mexico, 1990-2022: time trends and challenges for the health system.","authors":"Christian Razo, Rafael Lozano, Luis M Gutiérrez-Robledo","doi":"10.24875/GMM.M24000901","DOIUrl":"10.24875/GMM.M24000901","url":null,"abstract":"<p><strong>Background: </strong>Aging, urbanization, and lifestyle changes have fueled the non-communicable diseases (NCDs) epidemic.</p><p><strong>Objective: </strong>Assess mortality, disability, risk factors and life expectancy in people aged ≥60 years (LE-60) in Mexico from 1990-2022 using estimates from the Global Burden of Diseases, Injuries and Risk factors study.</p><p><strong>Material and methods: </strong>Health loss/gain in terms of disability-adjusted life years, risk factors, LE-60 and healthy life expectancy (HALE-60) by sex and state were analyzed.</p><p><strong>Results: </strong>NCDs and metabolic risk factors were the main contributors of health loss, noting a high prevalence of non-lethal conditions related to functional decline (vision and hearing), oral conditions, and chronic pain (back pain and osteoarthritis). All-cause mortality and disability rates decreased among men and women while LE-60 and HALE-60 increased from 1990-2022, with state-level variations. In Mexico, there are currently almost 20,000 patients waiting for a solid organ or tissue transplant.</p><p><strong>Objective: </strong>To summarize the rates of organ donation and transplants in the northeast of Mexico.</p><p><strong>Material and methods: </strong>We performed an observational, retrospective, epidemiological study using the collected data of solid organ and tissue transplants performed in 27 hospitals licensed for organ donation and transplantation in Nuevo Leon between 1996 and 2022. This study was approved by the clinical and epidemiological research committee of the Secretaria de Salud de Nuevo Leon.</p><p><strong>Results: </strong>From 1996 to 2022, 14,834 transplants were performed in Nuevo Leon. 4,185 (28.2%) were solid organ transplants and 10,649 (71.8%) were tissue transplants. Kidney transplants represented 84.3% (n = 3,528) of solid organ transplants, while corneal transplants represented 66% (n = 7047) of tissue transplants. From 2015 to 2022, organ and tissue donations reported in the state of Nuevo Leon represented an average of 5% of the national reported donations.</p><p><strong>Conclusions: </strong>Nuevo Leon has made significant advances in donation and transplantation, achieving good coverage for its population.
