{"title":"Knowledge and use of pericranial nerve blocks in headache treatment in Mexico.","authors":"Rogelio Dominguez-Moreno, Alejandro Marfil, Luis R Partida-Medina, Miguel Osorno-Guerra","doi":"10.24875/GMM.24000149","DOIUrl":"10.24875/GMM.24000149","url":null,"abstract":"<p><strong>Background: </strong>In developed countries, most of the neurologists use pericranial nerve blocks to treat headache patients, nevertheless, the knowledge and use patterns of this technique in developing countries are unknown.</p><p><strong>Objective: </strong>Evaluate the knowledge and use patterns of pericranial nerve blocks in headache treatment by Mexican neurologists.</p><p><strong>Material and methods: </strong>We did a cross-sectional study, 90 Mexican neurologists completed a 26-question survey including data about sociodemographics, knowledge and patterns of use of pericranial nerve blocks. All statistical analyses were performed with R (version 4.2.0).</p><p><strong>Results: </strong>The mean age was 43.2 + 9.9 years, 55(61.1%) were men. In terms of knowledge, migraine was correctly identified by 74 participants (82.2%), while cluster headache by 65 participants (72.2%) as indications for pericranial nerve blocks. Regarding the use patterns of pericranial nerve blocks, 87(96.7%) of the neurologists had heard about the technique, but only 12(13.3%) use it in their clinical practice. In terms of education, 19(21.1%) received training during the residency and 27(30%) during the post-residency period.</p><p><strong>Conclusions: </strong>The knowledge and use of pericranial nerve blocks for the treatment of headaches in Mexico is low. {"title":"Recurrence of cervical intraepithelial neoplasia stratified by age.","authors":"González-Rodríguez C Julio, Barquet-Muñoz A Salim, Mejorada-Pulido Emmanuel, Ugarte-Julio A Luz, Zepeda-Fong Mariel, Sánchez-Rodríguez Anahí","doi":"10.24875/GMM.24000131","DOIUrl":"https://doi.org/10.24875/GMM.24000131","url":null,"abstract":"<p><strong>Background: </strong>Cervical cancer is one of the leading causes of death among women in countries with middle to high income levels.</p><p><strong>Objective: </strong>Stratifying the predictors of recurrence/persistence of grade 2 or 3 CIN after primary conization in different age groups, is the object of analysis of our study.</p><p><strong>Material and methods: </strong>A total of 432 women with cervical intraepithelial neoplasia (CIN) attended from January 2017 to May 2020 were included. Information regarding age at diagnosis, immunosuppression and cytologic, colposcopy, LEEP or cervical cone biopsy and histopathological results were acquired. Recurrence incidence was calculated over follow-up time in months. To evaluate the association between patient's characteristics and recurrence, we calculated hazard ratio (HR) with 95% CI.</p><p><strong>Results: </strong>We observed 25 recurrences from 432 women and 4,869 months of follow-up (5.13 incidence per 1,000 months). Median age was 38 years old. Median follow-up was 6.8 months. The recurrence-free period at 1, 2 and 3 years was 95.47%, 82.18% and 44.09% respectively. Hospital mortality was the dependent variable. The area under the ROC curve (AUC) was calculated to determine the discriminative ability of qSOFA to predict in-hospital mortality.</p><p><strong>Results: </strong>A total of 587 cancer patients were included. A qSOFA score higher than 1 obtained a sensitivity of 57.2%, specificity of 78.5%, a positive predictive value of 55.4% and negative predictive value of 79.7%. The AUC of qSOFA for predicting in-hospital mortality was 0.70. In-hospital mortality of patients with qSOFA scores of 2 and 3 points was 52.7 and 64.4%, respectively. Certain health conditions, such as obesity and diabetes mellitus, have been described to be related to COVID-19 unfavorable outcomes.</p><p><strong>Objective: </strong>To identify factors associated with mortality in patients with COVID-19.</p><p><strong>Material and methods: </strong>Retrospective cohort of 998,639 patients. Patient sociodemographic and clinical characteristics were analyzed, with survivors being compared with the deceased individuals. Cox proportional hazards model was used to identify variables predictive of COVID-19-associated mortality.</p><p><strong>Results: </strong>Among the deceased patients, men accounted for 64.3%, and women, for 35.7%, with the difference being statistically significant. Subjects older than 80 years had a 13-fold higher risk of dying from COVID-19 (95% CI = 12,469, 13,586), while chronic kidney disease entailed a risk 1.5 times higher (95% CI = 1,341, 1,798), and diabetes mellitus involved a risk 1.25 times higher (95% CI = 1.238,1.276).</p><p><strong>Conclusions: </strong>Age, sex, diabetes mellitus and obesity were found to be predictors of COVID-19 mortality. The certification exams of medical specializations correspond to criterial summative exams with high consequences. Therefore, it is imperative to maintain the highest quality standards in all the processes involved in preparing the exam. At present, it has been detected that some councils of medical specialties conduct the certification exams with tests that contain serial case reports and the psychometric analysis that they conduct is supported by the TCT; however, the structure of this type of test violates the fundamental assumptions of the TCT. The violation of the fundamental assumptions of the TCT in the tests constructed through serial case reports can lead to biases or misinterpretations of the results. This document presents a reflection on the discrimination that older people face not only because they are older (ageism), but also because of other characteristics that place them in a situation of greater vulnerability and discrimination, such as belonging to the LGBT+ community, being in prisons or having HIV. It urges reflection on the role and sensitivity with which public institutions, mainly health institutions, must have to guarantee a fundamental right such as health, not only in the physical aspect but also in the mental one. The health challenges that the country faces are presented, and a call is made to leave no one behind, encouraging the most vulnerable populations to have immediate attention free of discrimination. Therefore, it is necessary to detect them with validated and easy-to-administer instruments.</p><p><strong>Objective: </strong>Validation of the Steps to Better Health Questionnaire (STBH-Q) on social determinants of health in the Peruvian population.</p><p><strong>Material and methods: </strong>Instrumental research in 420 adult participants. The STBH-Q was used and underwent translation, content validation and piloting. Construct validity and reliability were explored.</p><p><strong>Results: </strong>A version of 16 items grouped into five factors that explained 70.03 % of the total variance and an acceptable model fit (χ2/gl = 1.30, NFI = 0.996, CFI = 0.997, SRMR = 0.049, RMSEA = 0.027 [90 % CI = 0.10-0.40]) was corroborated. Discriminant, correlation, tautological and convergent validity with non-communicable diseases were adequate, as well as internal consistency.</p><p><strong>Conclusions: </strong>The STBH-Q is valid and reliable for use in the Peruvian population. Data collected included age, sex, subtype, year of diagnosis and death, disease progression, severity index (PASI), and comorbidities. Qualitative variables were described in frequencies and percentages; quantitative variables with mean and standard deviation. Logistic regression was performed to evaluate associations.</p><p><strong>Results: </strong>The study included 221 patients (116 men) with an average age of 55 years. Plaque psoriasis was the most frequent subtype (85%), and the main comorbidities were dyslipidemia (33%) and hypertension (33%). Men showed a higher proportion of plaque psoriasis (97% versus 75%), while women had more palmoplantar psoriasis (9.5% versus 0.9%).</p><p><strong>Conclusions: </strong>The association with metabolic and cardiovascular comorbidities highlights the need for multidisciplinary management and early detection.
