CV. Wijaya Teknik is a company engaged in the field of plastic injection molding. Based on interview data, it was found that in the production process of pot cendo products, around 315,000 units produced product defects, which reached 2-5% each month. Product defects are caused by the number of defective products found and complaints from consumers to the company. To overcome quality problems regarding product defects that occur in the company, it is necessary to identify related problems that occur and the need for improvement proposals to reduce consumer returns. This study aims to identify and plan improvement proposals using the Seven Quality Control Tools and the 5W+1H method. The Seven Tools used consist of checksheets, stratification, histograms, pareto diagrams, and causal diagrams. The research data taken is the company's historical data for 2020–2022. The results showed that there were product defects caused by several factors, such as humans, machines, methods, materials, and the environment. Criteria for defects that often occur in companies are damaged products due to broken packaging, corrugated packaging, and cracked packaging. Judging from the criteria for these defects, the proposed improvements are in the form of improving the quality of the work of operators, providing training to operators, which is carried out alternately once a month, and carrying out scheduled periodic maintenance on the machine. \u0000Abstrak. CV. Wijaya Tehnik merupakan perusahan yang bergerak di bidang injeksi molding plastik. Berdasarkan data wawancara yang didapatkan bahwa pada proses produksi produk pot cendo sekitar 315.000 unit menghasilkan cacat produk yang mencapai 2-5% setiap bulannya. Kecacatan produk disebabkan karena banyaknya produk cacat yang ditemukan serta komplain dari konsumen kepada perusahaan. Untuk mengatasi permasalahan kualitas mengenai cacat produk yang terjadi di perusahaan perlu dilakukannya identifikasi terkait dengan permasalahan yang terjadi dan perlu adanya usulan perbaikan untuk mengurangi return konsumen. Penelitian ini bertujuan untuk mengidentifikasi dan merencanakan usulan perbaikan menggunakan alat bantu Seven Quality Control Tools dan metode 5W+1H. Seven Tools yang digunakan terdiri dari checksheet, stratifikasi, histogram, diagram pareto dan diagram sebab akibat. Data penelitian yang diambil merupakan data historis perusahaan tahun 2020-2022. Hasil penelitian menunjukan adanya cacat produk yang disebabkan karena beberapa faktor seperti manusia, mesin, metode, material dan lingkungan. Kriteria cacat yang sering terjadi di perusahaan yaitu produk rusak karena kemasan yang pecah, kemasan bergelombang, dan kemasan retak. Efforts to maintain product quality to meet the requirements are an important preventive step in quality control. Constraints in quality control can lead to losses for the company. PT Helmindo Pratama, a helmet manufacturing company, faced challenges in quality control, especially dealing with product defects during the year 2022, with an average rate of 7.10% of its production. This research aims to identify the causes of product defects and provide recommendations for quality control improvement in the helmet production process. The method used in this research employs Failure Mode and Effect Analysis (FMEA) to analyze and address the defects, using the 5W+1H approach. The study identified 5 types of defects in the helmets, namely bubbles on the surface, imprecise visor positioning, low helmet durability, loose helmet strap, and detached helmet edge rubber. The defect with the highest Risk Priority Number (RPN) scored 392. Based on the research findings, several improvements are proposed for quality control in the helmet production process. These improvements include increased supervision during helmet production activities, replacement of damaged production facilities, scheduling maintenance for production facilities, and the implementation of standard operating procedures tailored to production needs. It is expected that these quality control improvements will reduce product defects in helmets, enhance product quality, and prevent losses for the company. \u0000Abstrak. Upaya menjaga kualitas produk agar sesuai ketentuan merupakan suatu langkah preventif yang penting dalam pengendalian kualitas. Kendala pada pengendalian kualitas dapat menyebabkan kerugian bagi perusahaan. PT Helmindo Pratama, perusahaan yang memproduksi helm, mengalami kendala dalam pengendalian kualitas, terutama dalam menghadapi kecacatan produk selama tahun 2022 dengan tingkat rata-rata 7,10% dari hasil produksinya. Penelitian ini bertujuan untuk mengidentifikasi penyebab kecacatan produk dan memberikan rekomendasi perbaikan dalam pengendalian kualitas pada proses produksi helm. Metode yang digunakan dalam penelitian ini menggunakan Failure Mode and Effect Analysis (FMEA) untuk menganalisis kecacatan dan perbaikan kecacatan menggunakan pendekatan 5W+1H. Hasil penelitian ini mengidentifikasi 5 jenis kecacatan pada helm, yaitu bubble pada permukaan helm, posisi visor tidak presisi, durabilitas helm rendah, tali strap kendur, dan karet pinggiran helm lepas. Initially catering to students, Cabut Kost provides a convenient solution for their moving needs, eliminating the hassle of packing and shipping. However, to enhance efficiency and expand their market reach, the company aims to transition from a conventional to a digital business model.The primary focus of this paper is to develop a prototype mobile application for Cabut Kost and implement new digital business processes. To achieve this, a Problem Solving Framework will be employed to identify and analyze the company's existing business flows. The research will progress through various stages, including inputting the company's information system, designing business processes, and creating an information system design using the waterfall method.By adopting this digital approach, Cabut Kost aims to facilitate faster communication with customers, ultimately leading to increased customer satisfaction. The analysis and design of the application and website will utilize the System Development Life Cycle (SDLC) approach. UKM Roti Citepus is a company that produces various types of plain bread products. These implement Make-To-Stock and Make-To-Order market response strategies. The number of product sales isn’t fixed because consumer and company are not bound by a contract, so incoming orders cannot be ascertained. This affects the procurement of raw materials where excess or shortages of materials occurs. Excess materials causes a longer shelf life so that quality of material affects quality of bread. While material shortages can cause production delays so orders are processed the next day. This can cause consumers to cancel orders. Both of these conditions can lead to a decrease in revenue. Based on the problems occurred, the proposal made is to improve inventory control with minimum inventory costs. Due to uncertain demand, the proposed is given by applying the probabilistic inventory methods, namely Q method and P method. The result of q method, order quantity is 186sacks, reorder point is 70sacks, and safety stock is 14sacks. While the result of p method, time interval is 11days, expected maximum inventory amount is 211sacks, and safety stock is 3sacks. There are difference in the two methods, but the proposed policy with the smallest total inventory cost expectation is fulfilled by the P method. In the proposed policy, total fee of Rp1.081.008.117 is obtained, which is smaller than the current policy of Rp1.092.638.559. (Reduction in cost with a percentage of 1,06% from total cost of the current policy inventory). \u0000Abstrak. UKM Roti Citepus merupakan perusahaan yang memproduksi berbagai jenis produk roti tawar. UKM ini menerapkan strategi respon pasar Make To Stock (MTS) dan Make To Order (MTO). Jumlah penjualan produk tidak tetap karena konsumen dengan perusahaan tidak terikat kontrak, sehingga pesanan yang masuk tidak dapat dipastikan. Ketidakpastian permintaan ini mempengaruhi pengadaan bahan baku yang sering terjadi kelebihan maupun kekurangan bahan baku. Kelebihan bahan baku menyebabkan masa simpan lebih lama sehingga kualitas bahan baku akan mempengaruhi kualitas roti. Apabila terjadi kekurangan bahan baku dapat menyebabkan penundaan produksi sehingga pesanan konsumen akan diproses keesokan harinya. Hal ini dapat menyebabkan konsumen membatalkan pesanan. Kedua kondisi tersebut dapat menyebabkan penurunan pendapatan perusahaan. Untuk mengatasi permasalahan diatas, dilakukan perbaikan pengendalian persediaan bahan baku dengan biaya persediaan minimum. Dikarenakan permintaan yang tidak pasti sehingga pengendalian persediaan di UKM Roti Citepus dilakukan dengan menerapkan metode persediaan probabilistik yaitu metode Q dan metode P. Hasil metode Q, kuantitas pemesanan (q) sebesar 186 sak, reorder point sebesar (r) 70 sak, dan safety stock (ss) sebesar 14 sak. Work that is done manually with a heavy workload, wrong work postures, and high repetition of work without adequate tools can increase the risk of musculoskeletal disorders. PT. Gradient is a company engaged in the manufacture of plastic products. The problem that occurs is that the moving and lifting activities are carried out manually by the operator, with the weight of the load being lifted up to 30 kg. The moving and lifting activities should be carried out 10–12 times per day. At the time of observation, it can only be done 8–9 times; this is because the operator takes a break during working hours. Based on interviews with operators, rest is used to unwind and reduce soreness. Therefore, this study aims to identify and measure work risks using the Key Indicator Method with Respect to manual Lifting, Holding, and carrying of Loads and the Key Indicator Method for Awkward Body Postures. Occupational risk assessment using the KIM LHC method shows a score in the range of 55–79; this score is in the classification of increasing substantially. The results of KIM ABP's work risk measurement showed a score of 41 with a slightly increased classification. \u0000Abstrak. Manual material handling merupakan aktivitas yang banyak dilakukan oleh manusia, didalamnya terdapat aktivitas seperti mengangkat, mendorong, dan penanganan material lainnya yang dilakukan tanpa alat bantu. Pekerjaan yang dilakukan secara manual dengan beban kerja yang berat berat, postur kerja salah dan pengulangan pekerjaan yang tinggi tanpa alat bantu yang memadai dapat menyebabkan terjadinya risiko musculoskeletal disorder. PT. Gradient adalah perusahaan yang bergerak dibidang pembuatan produk plastik. Permasalahan yang terjadi terdapat aktivitas pemindahan dan pengangkatan yang dilakukan secara manual oleh operator dengan berat beban yang diangkat mencapai 30 kg, aktivitas pemindahan dan pengangkatan seharusnya dapat dilakukan 10-12 kali per hari. Pada saat pengamatan hanya dapat dilakukan 8 – 9 kali, hal ini dikarenakan operator melakukan istirahat pada jam kerja. Berdasarkan wawancara dengan operator, istirahat digunakan untuk melepas lelah dan mengurangi rasa pegal. Oleh karena itu penelitian ini bertujuan untuk mengidentifikasi dan mengukur risiko kerja menggunakan Key Indicator Method with Respect to manual Lifting, Holding, Carrying of Loads dan Key Indicator Method – Awkward Body Postures. Cicalengka Dreamland is a natural tourist attraction located in Tanjungwangi Village, Cicalengka and has a land area of 69 hectares and 24 rides in it. Based on the data obtained from the observations, it is known that the number of visitors to this natural tourist spot has not increased. This also affects the amount of company revenue and the process of providing incentives to employees. The cause of this phenomenon is because the company does not innovate or add much to the rides in tourist areas. Apart from that, competitor tourist attractions have also started to appear in the area so that the competitiveness of natural tourism in Cicalengka Dreamland has also decreased. A performance measurement approach using the SMART (Strategic Management Analytic and Reporting Technique) System model is used to solve these problems and integrate the company's needs from both financial and non-financial aspects. The identification process between the target and the actual condition of the company is carried out in accordance with the steps of the SMART System, namely the identification stage of objective strategies and key performance indicators (KPI), the KPI structuring stage based on 9 SMART System perspectives, the KPI weighting stage using Expert Choice 11 software, and the performance appraisal stage using the Objective Matrix (OMAX) score calculation system. By measuring performance using the SMART System, companies can identify aspects that need improvement in order to increase employee competitiveness and performance. \u0000Abstrak. Cicalengka Dreamland merupakan tempat wisata alam yang berada di Desa Tanjungwangi, Cicalengka dan memiliki luas lahan 69 hektar serta 24 wahana didalamnya. Berdasarkan data yang didapat dari hasil observasi diketahui bahwa jumlah pengunjung di tempat wisata alam ini tidak kunjung mengalami peningkatan. Hal tersebut juga mempengaruhi jumlah pendapatan perusahaan serta proses pemberian insentif pada karyawan. Penyebab terjadinya fenomena tersebut dikarenakan pihak perusahaan yang tidak banyak melakukan inovasi maupun penambahan pada wahana di kawasan wisata. Selain itu juga mulai bermunculan tempat wisata pesaing yang berada di kawasan tersebut sehingga daya saing wisata alam Cicalengka Dreamland inipun ikut menurun. Pendekatan pengukuran kinerja dengan model SMART (Strategic Management Analytic and Reporting Technique) System digunakan untuk menyelesaikan permasalahan tersebut dan mengintegrasikan antara kebutuhan perusahaan baik dari aspek finansial maupun non-finansial. Proses identifikasi antara target dan kondisi aktual perusahaan ini dilakukan sesuai dengan langkah-langkah SMART System yaitu tahap identifikasi strategi objektif dan key performance indicator (KPI), tahap penstrukturan KPI berdasarkan 9 perspektif SMART System, tahap pembobotan KPI dengan menggunakan bantuan software Expert Choice 11, dan tahap penilaian kinerja dengan menggunakan sistem perhitungan skor Objective Matrix (OMAX). Shift workers often experience complaints such as back pain, muscle aches, neck pain, fever, colds, drowsiness and often lose focus in doing their jobs. CV. Kezka Diastama is a company that produces headscarves by implementing a 6-6-6 work pattern (6 working days in the morning shift, 6 working days in the afternoon shift and 6 working days at night shift). Companies experience 6.22% of defective products, while company standards set 1%–2% defects. The highest defects occurred at the laser cut work station by 4.12%, during the night shift by 2.9%. The night shift can reduce the ability to work because working at night is very easy to get tired because the rest time is used for work. Another cause, the job demands that require operators to pursue product targets to finish on time. This condition causes complaints conveyed by operators such as back pain, muscle and neck pain, fever, colds, drowsiness and loss of focus. The aim of the study is to propose improvements to work patterns at laser cut workstations. The research method used is the quantitative method using the 10 pulse method. The results of the measurements are used to calculate energy consumption and %.Cardiovascular (CVL). The results of energy consumption measurements in the morning shift and evening shift show the operator's workload in the moderate category and for the night shift in the heavy category. The percentage of %CVL operators is in the range of 30-60% which states that all operators need improvement. Proposed improvements change the 6-6-6 work pattern to a 2-2-2 work pattern. This change in work shift patterns is expected to reduce the number of product defects produced. \u0000Abstrak. Pekerja shift sering mengalami keluhan seperti, sakit punggung, nyeri otot, nyeri leher, demam, masuk angin, mengantuk dan sering kehilangan fokus dalam melakukan pekerjaannya. CV. Kezka Diastama perusahaan yang memproduksi kerudung dengan memberlakukan pola kerja 6-6-6 (6 hari kerja shift pagi, 6 hari kerja shift sore dan 6 hari kerja shift malam). Perusahaan mengalami produk cacat sebanyak 6,22%, sedangkan standar perusahaan menetapkan kecacatan 1%–2%. Kecacatan tertinggi terjadi pada stasiun kerja laser cut Sebesar 4,12%, pada shift malam sebesar 2,9%. Shift malam dapat menurunkan kemampuan bekerja karena bekerja pada malam hari sangat mudah lelah karena waktu istirahat digunakan untuk bekerja.  Penyebab lain, tuntutan pekerjaan yang mengharuskan operator mengejar target produk selesai tepat waktu. Kondisi ini menyebabkan keluhan yang disampaikan oleh operator seperti, sakit punggung, nyeri otot dan leher, demam, masuk angin, mengantuk dan kehilangan focus. X was founded in 1994, the company is a family business which is now being run by the second generation. The family system in the internal hierarchy and professionalism towards external parties is one of the company's fundamental approaches. CV. X is a manufacturing company that focuses on production services for the manufacture of plastic injection molds and die casting. Running a business in the manufacturing sector often has problems, one of which is work accidents which are often experienced by workers. Starting from serious accidents that require to be taken to the hospital or minor accidents where action is only carried out using first aid. Factors that become these problems are often caused by negligence of workers who do not comply with existing regulations in the company, causing harm to the workers themselves and the company. Efforts made by the company in reducing work accidents by creating a system that can tighten company regulations and Occupational Safety and Health (K3) standards used in Indonesia. One of the standards used in Indonesia is to implement an Occupational Safety and Health Management System (SMK3) based on the International Organization for Standardization (ISO) 45001: 2018 using this system it is necessary to go through 6 stages in designing a work safety management system, namely starting with policy making, planning, implementation and operation, checking, management review and continuous improvement. The final results obtained from the research conducted are in the form of Guidelines for the Occupational Safety and Health Management System Manual, which are in accordance with the International Organization for Standardization (ISO) 45001: 2018 standards. \u0000Abstrak. CV. X berdiri sejak tahun 1994, perusahaan tersebut merupakan bisnis keluarga yang kini dijalankan oleh generasi kedua. Sistem kekeluargaan dalam hierarki internal dan profesionalitas terhadap pihak eksternal merupakan salah satu pendekatan fundamental perusahaan. CV. X merupakan perusahaan manufaktur yang berfokus terhadap bidang jasa produksi pembuatan mold injection plastic and die casting. Menjalankan usaha dalam bidang manufaktur tentu sering memiliki permasalahan, salah satunya yaitu kecelaakan kerja yang sering didalami oleh parapekerja. Mulai dari kecelakaan berat yang mengharuskan dibawa ke rumah sakit atau kecelakaan ringan yang hanya dilakukan tindakaan dengan menggunakan P3K. CV X is a company engaged in the pottery industry with products such as jars, flower vases, ashtrays. Based on the research that has been done, the company is experiencing problems, namely the existence of defective products which result in losses. Based on the data in CV X, it is known that the percentage of defects month is 6.17% this shows the high number of defects that occur. The types of defects that occur include faded paint, asymmetrical shapes, cracks in the product, dirty black spots, and stains on the product. Efforts that can be made to reduce these problems is to identify the factors that cause defects using the Fault Tree Analysis and Failure Mode And Effect Analysis methods. Based on the results of data processing using the FTA method, it can be seen that the root cause of the problem is influenced by several factors, namely worker fatigue, lack of worker skills, materials used are not according to standarrs the burning process is too long and the temperature used is too hot, the glaze composition is not suitable, the amount of dust from production. As for the results of the FMEA analysis, it can be seen that the highest RPN is 252 for the type of faded paint defect, the potential cause is worker fatigue, while the lowest RPN is with a value of 40, the type of defect is dirty black spots, the potential cause is workers not diligently cleaning storage shelves. Proposed improvements are made by giving a break of 5 minutes every 1 hour on the sidelines of work, increasing supervision, conducting training schedules, making displays of the duration of burning and the temperature of the fire used. \u0000Abstrak. CV.X merupakan perusahaan yang bergerak di bidang industri kerajinan gerabah dengan produk yang dihasilkan berupa guci, vas bunga, asbak. Berdasarkan penelitian yang telah dilakukan, perusahaan mengalami permasalahan yaitu adanya produk cacat yang mengakibatkan kerugian. Berdasarkan data yang ada pada CV.X diketahui persentase cacat perbulan sebesar 6,17%, hal tersebut menunjukkan tingginya kecacatan yang terjadi. Jenis cacat yang terjadi diantaranya cat luntur, bentuk tidak simetris, retakan pada produk, bintik hitam kotor, dan coakan pada produk. Upaya yang dapat dilakukan untuk mengurangi permasalahan tersebut yaitu dengan melakukan identifikasi faktor-faktor yang menyebabkan kecacatan menggunakan metode Fault Tree Analysis dan Failure Mode and Effect Analysis. Berdasarkan hasil pengolahan data menggunakan metode FTA dapat diketahui akar penyebab masalah dipengaruhi oleh beberapa faktor yaitu pekerja kelelahan, skill pekerja yang kurang, bahan yang digunakan tidak sesuai standar, proses pembakaran yang terlalu lama dan suhu yang digunakan terlalu panas, komposisi pengglasiran tidak sesuai, banyaknya debu dari hasil produksi. Remaja Rosdakarya is a publishing and printing company that publishes textbooks for elementary, junior high, and high school, as well as reading books for early childhood. Currently, the company is always faced with a surge in demand in a certain month when it is about to enter the new school year. This surge in demand causes problems in the form of an increase in the number of defective products above the tolerance limit that the company has set at 3% due to inefficient employees or production processes. The method used to solve this research problem is to compile a solution for improvement using the 5W+1H method. This study recommends improvement proposals, in the form of providing adequate rest time for workers, making maintenance schedules regularly and periodically, checking the type of ink and paper to be used before being inserted into the printing machine, making SOPs for black and white, color, and prepares printing machines briefly and, cleaning the printing machine drum routinely 2-3 times a week, and providing fans at prepares work stations. \u0000Abstrak. PT. Remaja Rosdakarya suatu perusahaan penerbitan dan percetakan yang menerbitkan buku pelajaran SD SMP SMA, perguruan tinggi, serta buku bacaan untuk anak usia dini. Saat ini perusahaan selalu dihadapi dengan lonjakan permintaan pada bulan tertentu ketika hendak memasuki tahun ajaran baru. Lonjakan permintaan ini menimbulkan permasalahan berupa meningkatnya jumlah produk cacat diatas batas toleransi yang telah perusahaan tetapkan sebesar 3%, dikarenakan karyawan atau pun proses produksi yang berjalan tidak efisien. Metode yang digunakan untuk menyelesaikan masalah penelitian ini ialah dengan menyusun solusi perbaikan menggunakan metode 5W+1H.
