Headache (HA) is the predominant symptom, often mimicking primary cephalalgias, complicating diagnosis and optimal surgical strategy selection.</p><p><strong>Aim of the study: </strong>To analyze the characteristics of headache in CMI, identify the pathophysiological mechanisms of typical (tHA) and atypical headaches (aHA), and assess the effectiveness of different decompressive surgical techniques in postoperative pain relief.</p><p><strong>Materials and methods: </strong>The study included 102 patients (58 females, 44 males, aged 2-69) with isolated or complicated CMI who underwent decompressive surgery at the Neurosurgery Department of Azerbaijan Medical University and the Republican Neurosurgical Center (2016-2024). Patients were divided into two groups based on headache type: typical HA (tHA, n=82) and atypical HA (aHA, n=20). Headache characteristics and dynamics were assessed using neurological and neuroimaging monitoring.</p><p><strong>Results: </strong>Decompression surgery significantly reduced headache severity, particularly in tHA patients. Our study aims to investigate the effect of bariatric surgery on diabetes remission.</p><p><strong>Methods: </strong>We studied 91 patients with type 2 diabetes mellitus (T2DM) and morbid obesity who underwent bariatric surgery-sleeve gastrectomy-at the \"David Abuladze Georgian-Italian Clinic\" from 2015-2022. The patients' ages ranged from 23 to 67 years (46.67±8.60), including 50 women and 41 men.</p><p><strong>Results: </strong>After the surgery, all patients reported weight loss (kg): before the surgery 146.80±25.81, after the surgery 97.87±17.56. t= 23.30, p <0.0001 the relative chance of remission after bariatric surgery increases Weight (kg) - OR=1.03(95% CI:1,02-1.05) and reduce Glycemia - Fasting >125 mg /dL - OR=0.06(95% CI:0.01-0.60); Sulfonylurea - OR=0.07(95% CI:0.02-0.27); Insulin - OR=0.06(95% CI:0.01-0.56) Conclusions: • Bariatric surgery in patients with type 2 diabetes promotes long-term remission of diabetes. • Positive predictor of complete remission is the patient's initial weight before bariatric surgery. {"title":"DIAGNOSTIC CHALLENGE: COEXISTING MULTIPLE MYELOMA AND EXTRAMEDULLARY PLASMACYTOMA WITH RENAL AND HEPATIC INVOLVEMENT.","authors":"K Tsanava, L Khurtsia, E Shengelia, G Qvariani, L Dangadze","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>Both plasmacytoma and multiple myeloma (MM) are plasma cell dyscrasias, though their concurrence, especially with the unusual extramedullary presentation of plasmacytoma, is exceedingly rare. Distinguishing between them is crucial for management, especially in the presence of atypical findings that complicate the diagnosis.</p><p><strong>Case report: </strong>Herein, we describe a rare instance of synchronous plasmacytoma and multiple myeloma (MM) in a 65-year-old woman. The patient did not have any preceding symptoms or complaints and was living her life with no limitations until the day she fell and fractured. Preoperative examinations by a traumatologist demonstrated anemia, increased rates of nitrogen metabolism, and a mass in the right kidney. The above findings led to the cancellation of the surgery and encouraged our team to go into more detail. Computed tomography (CT) scans showed a renal mass and liver lesions. During the diagnostic procedure, special attention was given to the acutely high level of total protein in the blood plasma, for which investigations were directed in another avenue. Electrophoresis of blood and urine and bone marrow biopsy established the diagnosis of MM, although the typical CRAB criteria were absent. Laboratory findings present conflicting evidence, with some results suggesting MM and others suggesting plasmacytoma. Clarification was especially important since the renal mass was initially thought to be renal cell carcinoma and needed to be distinguished carefully. Due to the uncommon coexistence of these diseases, we examined both literature and clinical cases in order to investigate their simultaneous occurrence.</p><p><strong>Conclusions: </strong>This case illustrates the diagnostic dilemma of simultaneous MM and plasmacytoma, highlighting the importance of thorough assessment, including histology and modern imaging. However, the fragmentation of information systems in Peruvian blood banks limits the traceability and efficiency of donor data management. This study aims to evaluate the perception of healthcare professionals regarding the need for an interoperable information system for blood banks.</p><p><strong>Methods: </strong>A cross-sectional study was conducted in public (MINSA), social security (ESSALUD), and private sector blood banks in Lima, Peru. A structured survey was applied to 65 professionals to assess the current status of their information systems, perceived limitations, and their acceptance of a new interoperable platform. The data were analyzed using descriptive and inferential statistics.</p><p><strong>Results: </strong>Most participants (85%) reported having an information system, but only 22% confirmed its interconnection with other blood banks. Furthermore, 97% supported the implementation of a new system with a centralized database for donor traceability. The main barriers identified were insufficient network infrastructure (43%) and budget constraints (31%).</p><p><strong>Conclusion: </strong>The results highlight the urgent need for an interoperable information system in Peruvian blood banks. Its implementation could enhance efficiency, reduce redundant testing, and improve transfusion safety. And trend analysis, from long-term studies, are even scarcer.</p><p><strong>Objective: </strong>we sought to report changes in revascularization, in-hospital treatments, and outcomes of patients with STEMI admitted in the last ten years, included in the REgistro de Sindromes Coronarios AgUdos (RESCUE) database, from a middle-income scenario without coronary intervention Methods: The RESCUE database (2014 to December 2023) was queried to identify all STEMI hospitalizations. Temporal trends, outcomes, demographics, revascularization, in-hospital and discharge medications in these patients were determined. Also, overall defect-free care, a composite measure capturing use of guideline-directed medical therapy (GDMT) at admission and at discharge was reported.</p><p><strong>Results: </strong>Among 1456 patients with STEMI, there was significant increase in fibrinolytic administration (58.2% in 2014-2016 vs 70.3% in 2021-2023, P≤0.01). Overall defect free care composite at admission and at discharge was only achieved in 37.4% and 74.3% of patients, respectively. Administration of Aspirin, Clopidogrel, statin, Angiotensin Converter Enzyme Inhibitor (ACEI), Beta-blocker (BB) was 98.6%, 98%, 98.1%, 92.4%, 61.7%. At discharge, it was 97.9%, 96.1%, and 95%, 96.7% and 80.7%. Unadjusted in-hospital mortality decreased from 2014-2016 to 2021-2023 (14-7.7%, P=0.012). However, risk-adjusted mortality rate increased significantly (5-8.9%, P<0.001).</p><p><strong>Conclusion: </strong>There has been a steady improvement in process measures and a high adherence to GDMT in this population. In-hospital mortality has remained stable, despite not having defect-free care in most patients. The results revealed that a significant proportion of children (41%) had BLLs exceeding 5 μg/dL. The aim of this study is to evaluate the health risks associated with the potential environmental impacts of technogenic chemical agents in the Bolnisi-Dmanisi region. Additionally, the research seeks to develop recommendations for mitigating these risks through appropriate preventative and corrective measures.</p><p><strong>Methodology: </strong>A cross-sectional method was used for this survey and biomonitoring to assess sources of lead exposure. Three target groups (TGs) were selected for the study based on potential exposure levels, randomly interviewed about their health status, and subjects were randomly selected to undergo venous blood testing for lead. For the correlation defining the Statistical analysis of crossover interference using the chi-square model was used. The results obtained determine the relative risks of various nosologies among the TGs, the correlations with various risk factors, and their potential cause-and-effect relationships.</p><p><strong>Results: </strong>Our results revealed a significant reduction in BLLs among residents of the study region when compared to the MICS results. In 2023, our findings indicated a decrease in BLLs among populations in potentially exposed areas of Kvemo Kartli. Specifically, the average BLL in the Kvemo Kartli region decreased by approximately 50%, from 4.2 μg/dL in 2018 to 1.8 μg/dL in 2023. Demographic risk factors did not exhibit significant variation across the TGs. Notably, statistically significant relative risks (RR) for diseases of the eye and adnexa were observed in the first and second TGs, while no such significant associations were found for other diseases.</p><p><strong>Conclusion: </strong>Although the MICS study conducted in the country five years ago showed a significant reduction (by two times or more) in the high average biomonitoring results recorded in children, our study still revealed certain low, yet credible, health risks in populations living 'near production/mining' sites and 'potentially living on ore,' according to some nosological classes. The biomonitoring results also recorded isolated cases of relatively high BLLs, particularly in the exposed groups compared to the control group. Based on these findings, we conclude that the measures implemented over the past five years have yielded positive results, significantly reducing lead concentrations in humans. However, certain low but credible health risks persist in the presumably exposed areas. Axial Diffusion-weighted Imaging (DWI) without contrast is used for echo planar imaging.</p><p><strong>Method: </strong>Acute stroke MRI pleasant discrimination has been examined using routine brain MRI with various b-values.</p><p><strong>Result: </strong>Different B- Values were tested for diagnosis of acute stroke. Best result was when the b Value Between 200 to 1000 s/mm2. Assume that acute stroke is evaluated using just one b variable. Then, instead of raising the b value to 1500 s/mm2, it is better to maintain the overall performance of the b factor at 1000 s/mm2.</p><p><strong>Conclusion: </strong>To get a better understanding of acute stroke, several b-values have been employed. Microscopy and fungal culture are the gold standard techniques for onychomycosis diagnosis. At the same time, the culture method is long-term and requires significant costs. In this regard, a promising direction in laboratory diagnostics of onychomycosis is the detection of genetic markers of onychomycosis pathogens using polymerase chain reaction. The goal of current work is optimization of diagnostic care for patients with onychomycosis through the use of PCR, epiluminescent diagnostics and therapeutic agents that improve the structure of nail plates.</p><p><strong>Material and methods: </strong>The work is based on the results of observations of 343 patients with fungal lesions of the nails of the hands and feet, which were on inpatient and outpatient treatment, as well as data from laboratory and instrumental studies. Mycological examination of patients included microscopic examination of pathological material (pieces of nail fragments) and cultural study. PCR was performed using a reagent kit with primers to determine the presence of DNA specific to Trichophyton rubrum and PCR with panfungal primers. Epiluminescence surface microscopy of affected nail plates was performed.</p><p><strong>Results: </strong>After calculations using standard methods, it was determined that the sensitivity for PCR is 91.9%, for microscopy - 75.9%, for culture - only 44.3%. The specificity for PCR is 71.4%, for microscopy - 83.3%, for culture - 100.0%. The diagnostic accuracy rate for PCR was 90.3%, for microscopy - 76.3%, and for culture - 47.3%. The higher the sensitivity of the method, the more often pathological changes are detected with their help, and accordingly, the more effective it is. Utilizing molecular docking simulations, we evaluated the binding affinities of synthesized compounds, including compound 1, compound 5, and the reference drug salbutamol, against β2AR. The docking studies were conducted using GOLD software, and binding interactions were analyzed to identify key residues responsible for ligand binding and receptor activation. The results revealed that all tested compounds, particularly compound 1 and compound 5, demonstrated strong binding to the β2AR, with binding energies comparable to salbutamol. Key residues such as SER 207, PHE 289, LYS 305, and ASP 192 played significant roles in stabilizing the receptor-ligand interactions. The presence of functional groups like NO2 and NC in the synthesized compounds enhanced their affinity, suggesting that structural modifications could optimize β2AR binding. These findings provide valuable insights into the molecular mechanisms underlying β2AR-ligand interactions and highlight the potential of compounds 1 and 5 as promising candidates for further development into β2 agonists for asthma treatment. The General Health Questionnaire-20 (GHQ-20), Type A Behavior Pattern (TABP), University Student Employment Stress Scale, Medical Student Employment Stress Questionnaire, and Employment Intentions Survey for Medical University Students were used to assess the participants. Based on the GHQ-20 scores, the residents were divided into a positive group and a negative group to analyze the relationship between Type A personality, employment stress, and mental health.</p><p><strong>Results: </strong>The results of this study did not show a significant correlation between Type A personality, employment stress, and mental health among resident physicians in tertiary hospitals in Nanchong (P>0.05).</p><p><strong>Conclusion: </strong>This study did not confirm a significant relationship between Type A personality, employment stress, and mental health among resident physicians in tertiary hospitals in Nanchong. This may be due to the small sample size and insufficient assessment methods.
