This research is particularly relevant because ASD is a neurodevelopmental disorder, and one of its most challenging issues is sleep disorders. In Georgia, the number of children and adolescents with ASD has increased significantly over the past two decades, yet sleep disorders among this population have not been studied at all to date.</p><p><strong>Material and methods: </strong>The parents (volunteers) of 500 ASD children/adolescents (without comorbid conditions and medication-free) participated in this study. Children and adolescents with ASD who had sleep disorders were identified based on subjective data collected from their parents using a modified version of the Children's Sleep Habits Questionnaire, as well as the Simonds & Paraga modified version. The control group consisted of typically developing, age-matched peers. Results treated statistically by ANOVA, with Student's t criteria.</p><p><strong>Results and conclusion: </strong>For the first time, we discovered serious sleep disorders in 61% of Georgian children and adolescents with ASD characterized by: a) difficulties falling asleep, significant increases in sleep latency, and heightened sleep resistance; b) frequent awakenings and challenges in maintaining nighttime sleep; and c) a notable increase in anxiety and nightmares. All of these considerably worsen sleep quality, which can, in turn, have further consequences on the behavioral symptoms of ASD. {"title":"APPLICATION OF SHELL TECHNIQUE IN C-TIRADS COMBINED WITH STE IN DIAGNOSIS OF C-TIRADS GRADE 4-5 NODULES.","authors":"Bi-Yun Sun, Wei Peng","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To analyze the diagnostic value of the Shell technique in the Chinese Thyroid Imaging Reporting and Data System (C-TIRADS) combined with Shear Wave Elastography (STE) for differentiating benign and malignant C-TIRADS grade 4-5 nodules.</p><p><strong>Methods: </strong>A retrospective analysis was performed on 279 patients (with a total of 286 C-TIRADS grade 4-5 nodules) who underwent thyroid fine-needle aspiration biopsy (FNA) or surgery. Nodules were differentiated as benign or malignant using C-TIRADS classification, STE, and their combination. Taking cytopathological or postoperative pathological diagnosis as the gold standard, Receiver Operating Characteristic (ROC) curves were plotted, and the Area Under the Curve (AUC), sensitivity, specificity, accuracy, positive predictive value (PPV), and negative predictive value (NPV) were calculated to compare the diagnostic efficacy.</p><p><strong>Results: </strong>Pathological results showed that among 286 nodules, 94 were benign and 192 were malignant. C-TIRADS diagnosed 122 benign and 164 malignant nodules; STE diagnosed 115 benign and 171 malignant nodules; the combination diagnosed 108 benign and 178 malignant nodules. The AUC of the combined diagnosis for C-TIRADS grade 4-5 nodules was significantly larger than that of C-TIRADS classification and STE alone (Z=5.195, P<0.001; Z=3.602, P<0.001). The sensitivity of the combined diagnosis was higher than that of C-TIRADS classification and STE alone (χ²=8.649, P=0.003; χ²=5.488, P=0.019). The accuracy of the combined diagnosis was higher than that of C-TIRADS classification and STE alone (χ²=11.457, P<0.001; χ²=9.527, P=0.002). Most metal components, including cobalt, zinc, manganese, and germanium, significantly reduced bacterial viability, whereas magnesium showed no suppressive effect. Bactericidal efficacy was confirmed both by diffusion assays and morphological disruption observed under high-resolution microscopy. However, awareness and attitudes towards prostate cancer screening vary widely. This study aimed to assess the knowledge, attitudes, and practices (KAP) regarding prostate cancer and its screening methods in the Qassim region of Saudi Arabia.</p><p><strong>Methods: </strong>This descriptive cross-sectional study involved 384 male participants aged 20 years and above. Data were collected using an online Arabic questionnaire. Knowledge scores were calculated, and participants were classified as having good or poor knowledge. The Chi-squared test and Spearman's correlation analysis were used for statistical analysis.</p><p><strong>Results: </strong>Most participants (42.8%) were aged 20-29 years, and 90.3% were Saudi nationals. Only 61.6% had heard of prostate cancer, and 30.4% were aware of both the prostate-specific antigen (PSA) blood test and rectal exam. Knowledge about risk factors was poor, with smoking identified by 42.3% and obesity by 23.9%. Symptoms were better recognized, with 67.9% identifying blood in urine or semen. Only 10.9% had ever had a prostate exam, and 6.3% had a PSA test. Knowledge was significantly associated with age, educational level, occupational status, and working in the health-care field.</p><p><strong>Conclusion: </strong>The study revealed gaps in knowledge, attitudes, and practices regarding prostate cancer and its screening methods. The study included 70 newborns with pathology. Depending on the method of surgical intervention, the children were divided into 2 groups. The 1st group consisted of 44 newborns with congenital diaphragmatic hernia, operated on using the traditional open method. The 2nd main group consisted of 26 newborns with similar pathology, operated using a minimally invasive new type of surgical technique - endosurgery. Before and after the operation, both the parent and the doctor filled out a questionnaire based on the QUALIN questionnaire. According to the results of the questionnaire, an improvement in all indicators in the postoperative period was noted in the main and control groups. Also, in the main group, all parameters were high according to both parents and doctors. However, statistical reliability was achieved for the parameters \"Ability to live independently\" and \"Neurological and mental development\". Although no single study specifically addressed reduction assessment in low-resource environments, relevant articles discussing imaging-based assessments (e.g. fluoroscopy, 3D imaging), direct visualization approaches (e.g. arthrotomy), and tactile reduction techniques were included based on clinical relevance and applicability.</p><p><strong>Results: </strong>Conventional 2D fluoroscopy remains the most widely used intraoperative imaging modality, despite its limited sensitivity for subtle articular incongruities. Direct visualization techniques, including arthrotomy, provide reliable tactile and visual assessment without requiring advanced imaging. Emerging innovations such as smartphone-assisted tools and 3D printing hold promise but currently lack validation in low-resource settings. There remains a notable gap in literature addressing tailored intraoperative assessment protocols for resource-constrained environments.</p><p><strong>Conclusions: </strong>In the absence of advanced intraoperative imaging, reliance on fundamental principles-such as thorough preoperative planning, direct visual and tactile assessment, and structured surgical technique-remains essential. Unlike other melanoma types, UV exposure is not considered as significant etiological factor. Instead, mechanical stress, particularly traumatic injury, is recognized as a potential contributor to ALM development, especially in weight-bearing areas such as the sole. The presence of pre-existing pigmented lesions may serve as precursors. While diagnostic and therapeutic approaches for skin cancer are well-established in existing guidelines - such as those proposed by the European Joint Committee (EJC) and the American Joint Committee on Cancer (AJCC) - certain limitations are evident. These standardized protocols may not be fully beneficial for each patient, which highlights the need for a more personalized, patient-focused approach. In response, alternative methodologies, including the personalized One Step Melanoma Surgery (OSMS), have been introduced. OSMS offers a single-stage surgical intervention that minimizes both financial and psychological burdens compared to the conventional two-step approach protocol by the EJC and AJCC guidelines. We present a 57-year-old woman with a tumor-like lesion on the right hallux. A prior injury 30-years earlier was reported, after which a pigmented lesion developed and remains stable for decades. Following a recent domestic trauma, the lesion underwent rapid malignant transformation into an aggressive subungual acral nodular melanoma with subsequent bone infiltration, staged IIC T4bN0M0, Clark level IIC, Breslow thickness >4mm. Amputation of the right distal and proximal phalanx of digitus I (hallux) was performed. In response, we developed a medical brochure designed to be accessible, easy to understand, and practical. We believe it will be effective, as a similar guide created previously for pregnant women proved successful. This topic was also discussed in an article I published in your journal one year ago. The primary outcomes were TTR, BER, and INR stability. The databases search was performed on Scopus, Web of Science, PubMed, and Cochrane.</p><p><strong>Results: </strong>This systematic review highlights the effectiveness of genotype-stratified warfarin dosing after cardiac surgery. Bayesian models showed an improvement in TTR, with NextDose achieving 63% versus 56% with standard dosing. Genotype-guided approaches reduced bleeding events from 34 to 16 and increased INR stability from 83.1% to 86.1%, improving dosing precision and achieving a TTR of 77.76% compared to 57.43%.</p><p><strong>Conclusion: </strong>These findings reinforce the clinical importance of the use of genotype data for more precise warfarin dosing in improving TTR, INR control, and bleeding risk. Various fetal parameters like fetal anthropometry are used for the calculation of Gestational Age (The present study aimed to study the correlation between fetal anthropometric measurements like Crown-Rump length, Femur length, Head Circumference (HC), Bi-Parietal diameter (BPD) and Abdominal Circumference (AC) and Gestational age to derive regression equations for the same.</p><p><strong>Methods: </strong>This cross-sectional hospital-based observational study was conducted at Alkharama Hospital, Baghdad; TS involved 144 aborted fetuses without congenital anomalies, aged between 12 to 40 weeks of gestational age. As per the standard protocol, the fetal measurements were documented. Regression equations are derived for the anthropometric measurements.</p><p><strong>Results: </strong>In the present study, the correlation coefficients of the equation are 0.937, 0.984, 0.982, 0.987, 0.991, and 0.992, respectively, for birth weight, HC, CRL, BPD, FL, and AC. Gestation has a robust positive association with all other indices, with correlation coefficients ranging from 0.94 to 0.99, all of which are statistically significant at p<0.001.</p><p><strong>Conclusion: </strong>All fetal measures exhibit a positive correlation with gestational age.
