Cengiz","doi":"10.29333/EJGM/81756","DOIUrl":"https://doi.org/10.29333/EJGM/81756","url":null,"abstract":"Introduction:To generate a prediction model for miscarriage in women with a viable single pregnancy from first-trimester ultrasound findings and maternal characteristics.Material and Methods:A prospective, cross-sectional study of 415 singleton pregnancies was performed. The initial ultrasound parameters were crown-rump length (CRL), mean gestational sac diameter (MGSD), yolk sac diameter (YSD), and the sum of the differences between gestational ages and embryonic heart rate (EHR). Potential predictors for spontaneous miscarriage occurring prior to 20 weeks were evaluated.Results:Fifty-three (12.8%) patients had miscarriages and 362 (87.2%) had normal outcomes. Forty-three (81.2%) miscarriages occurred in the first trimester, 5 (9.4%) in the second trimester, and 5 (9.4%) represented fetal anomalies. EHR, CRL, and MGSD were decreased in the miscarriage group (p<0.001); YSD showed no difference (p=0.21). Gestational age by CRL and by MGSD were different between the groups (p<0.001). The proposed sum of differences was higher in the miscarriage group (p<0.001). Maternal age, indication for scan, gestational age by MGSD and CRL, heart rate, and proposed sum of differences were found to be potential predictors. Predictive ability of our proposed model was calculated sensitivity as 0.509, and specificity as 0.975 with a cut-off=0.5. The prediction model’s false positive rate is 0.025, and its false negative rate is 0.491.Conclusions:Miscarriage can be predicted via maternal characteristics and ultrasound findings. Gabapentin, a structural analogue of gamma-amino butyric acid, has been used as an anticonvulsant and antinociceptive drug and is claimed to be more effective in preventing neuropathic component of acute nociceptive pain of surgery.Material and Methods:Fifty patients of ASA grade I and II were assigned to receive oral 600mg Gabapentin or Placebo 2 hours before surgery. Surgeries were conducted under spinal anesthesia. Post operatively pain was assessed by visual analogue score (VAS) at 2, 4, 8, 12 and 24 hrs. Patients were given rescue analgesic on demand. Sedation score and total numbers of analgesics during first 24 hours postoperatively were noted.Results:Gabapentin group resulted in faster onset of motor and sensory block, significantly longer duration of analgesia, substantial reduction in post-operative pain and the rescue analgesics. The clinical records of 1800 patients who underwent surgery in emergency conditions with a preliminary diagnosis of acute appendicitis were reviewed retrospectively. Carcinoid tumor was detected in 7 patients (0.38%) on histopathological examination. Of these patients, 4 were male (57.1%), 3 were female (42.9) and the mean age was found to be 36 (22-49). Appendiceal carcinoid tumors are tumors with a good prognosis. {"title":"Oxidative stress biomarkers among Saudi patients withnon-alcoholic steatohepatitis versus chronic hepatitis C","authors":"O. Al-Jiffri","doi":"10.29333/ejgm/81755","DOIUrl":"https://doi.org/10.29333/ejgm/81755","url":null,"abstract":"Introduction:As oxidative status may be an influential factor for increasing the progress of the disease and decreasing the effectiveness of the treatment, the aim of this study was to measure the oxidative stress and anti-oxidative markers among Saudi patients with chronic hepatitis C (CHC) versus nonalcoholic steatohepatitis (NASH).Material and Methods:This study was carried out on a sample of 150 consecutive patients (75 with CHC and 75 with NASH), selected from gastroenterology outpatient clinic, King Abdulaziz University Hospital, Jeddah, Saudi Arabia. Also, this study included 75 healthy volunteers, mostly, relatives of the patients who came with them to the clinic. Measurements of oxidative stress and anti-oxidative stress markers were measured for all participants.Results:The mean values of oxidative stress markers included conjugated dienes (CD), malondialdehyde (MDA), nitric oxide (NO) and myeloperoxidase (MPO) were significantly elevated in patients with CHC and patients with NASH when compared with Healthy controls group. Also, these parameters were significantly elevated in patients with NASH than patients with CHC. However, the mean values of anti-oxidative stress markers include glutathione peroxidase (GPx), superoxide dismutase (SOD), glutathione (GSH), arylesterase (AE) and paraoxonase (PON) were significantly reduced in patients with CHC and patients with NASH when compared with Healthy controls group. PSB can lead to morbidity via inhibiting the minimally invasive features of laparoscopic surgery such as the increase in the incision size. PSB can be controlled by Foley catheter. PSB has been observed in four patients with body mass index >35 and the bleeding cannot be prevented with different hemostasis techniques (for instance cauterization, etc.). Bleeding can be controlled upon placing the Foley catheter and then the balloon was inflated and putting it to the traction in the port site. Umbilical cord clamp was used instead of traditional methods for traction. In addition to the hemostatic effect of traction, the effect of compression of the umbilical clamp contributed to hemostasis. Catheter was removed after 24-36 hours. Bleeding did not occur again upon the usage of Foley catheter in these patients. However, vitamin D deficiency may play a role in mediating low-grade inflammation and insulin resistance among type 2 diabetic patients.Material and Methods:One hundred obese Saudi patients with T2DM (60 males and 40 females). Their age was46.38 ± 7.53 year, and a control group included one hundred healthy volunteers, who was gender and age matched.Results:Obese T2DM patients showed significantly higher glucose, insulin, glycosylated hemoglobin (HBA1c), Homeostasis Model Assessment-Insulin Resistance (HOMA-IR) index, interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α) and C-reactive protein(CRP) in addition to significantly lower values of the quantitative insulin-sensitivity check index (QUICKI) and 25(OH) vitamin D levels in comparison to controls. The clinical symptoms vary from femoral neuropathy to fatal hypovolemic shock with increased used of anticoagulant agents; femoral nerve palsy subsequent to hemorrhage within the iliopsoas muscle has become a frequent clinical problem. In our case, a 73-year-old male patient presented to hospital with left leg pain and weakness. Lumbar magnetic resonance imaging (MRI) and abdomen computed tomography (CT) showed a left-sided iliopsoas hematoma. An extraperitoneal inguinal incision (Gibson incision) was made. The hematoma was superior of the external iliac artery in the iliopsoas muscle. It was nearly 8 cm deep and was drained and evacuated. The aim of this brief report was to compare clinical assessment of obesity with measurements obtained using BMI and subscapular skinfold thickness (SST). In this paper, we aimed to present our experience in tracheobronchial foreign body aspirations and rigid bronchoscopy in children.Material and Methods:Between January 1990 and May 2015, 693 patients less than 17 years of age with presumptive diagnosis of tracheobronchial aspiration of foreign bodies were included this study. Patients records were retrospectively analysed according to age, gender, symptoms and physical fndings, foreign body type and localization, radiologic fndings and treatments.Results:Three hundred ffty one of the patients were male (51%) and 342 patients were female (49%). All patients underwent rigid bronchoscopy procedure except six patients. Foreign bodies were found in 589 patients (85%). Mcgill clemp was used for removing of foreign bodies settled on vocal cords in six patients and subsequently, rigid bronchoscopy was performed for checking of tracheobronchial system. Thoracotomy was required in four patients (0.7%). The most common foreign bodies were nuts (n=373). The most frequent localization site of foreign bodies was right bronchial system (n=292). The chief radiologic fnding was obstructive emphysema (n=296). But, there is no clear evidence comparing asthma and COPD. The aim of the present study was to investigate and compare the emotional status of patients with asthma and COPDMaterial and Methods:The study included 132 consecutive patients who were diagnosed with asthma (n=78) and COPD (n=54) who did not receive therapy for pshiciatric disorders. Emotional status were evaluated using the Beck and Hamilton surveys.Results:There was signifcant difference between patients asthma an COPD in terms of anxiety and depression level. The patients with COPD achieved signifcantly worse scores in emotional status. Anxiety and depression was signifcantly greater in patients with uncontrolled asthma.
