Viridans group streptococci are the most common bacterial isolates along with many other aerobic and anaerobic bacteria. S. salivarius is the most common viridians group streptococcus followed by S. mitis, S. sanguis, S. milleri, S. gordonii and S. mutans. The diverse habitat unique to the oral cavity together with a variety of nutrients is supportive to mixed population. These members of the resident oral flora, including viridians group streptococci, are responsible for colonization resistance, i.e., they prevent colonization / invasion by more pathogenic exogenous bacterial pathogens. Disruption of the oral ecosystem may lead to impairment of colonization resistance leading to higher rates of colonization by more pathogenic exogenous bacterial pathogens. Herein we report a case of pyelonephritis caused by Paecilomyces variotti in a 14-month-old child following an uriner tract operation. He had operation in 3.5-month-old for reason of ureteropelvic junction obstruction. Approximately two months after the operation a tissue fragments ejected from urinary tract. The patient admitted to our hospital because of bimonthly recurrence was occured. This tissue fragment was sent for fungal studies to our department. Direct microscopy revealed hyphae. P. variotii was isolated from discarded tissue fragments. To this end, the correlation between chronic obstructive pulmonary disease and inflammation was planned to be utilized and whether neutrophil-to-lymphocyte ratio can be used as a valid tool in the diagnosis of acute exacerbations of chronic obstructive pulmonary disease was investigated.Method:We retrospectively enrolled the 467 patients. Control group included sex and age-matched healthy people.C-Reactive protein, forced expiratory volume-1, forced vital capacity, complete blood count and clinical data A receiver-operating characteristic curve analysis was performed to determine the best cut-off value of N/L ratio and C-Reaktif protein to predict the exacerbation. The present study aimed to demonstrate one-year activity-based costing of services provided by service-providing centers of Yedikule Chest Diseases and Thoracic Surgery Training and Research Hospital, which provides services for a wide region, and to present the share of COPD in this cost.Method:Income and expense cost sheets of the year 2011 of the hospital was reviewed. The activity-based costing system was used for calculations. After the identification of the main service centers's expenses of the hospital,the annual expenditure of each unit was calculated. The cost of COPD patients was calculated based on the expenditures of the units caring for COPD patients. The cost per unit of services like radiography, laboratory and pulmonary function tests was calculated and added to the cost of COPD patients.Results:Annual cost according to the activity-based costing system was\u0000found to be 3,839,788.32 US Dollars ($) for COPD patients examined and\u0000treated as inpatient (2,531,290.9 $ ) , or at emergency room (586,170.7\u0000$) , or at respiratory intensive care unit (419,282.11$) , or at polyclinics\u0000(303,044.5 . It accounted for 19.5% of one-year hospital cost (total\u0000annual hospital cost was 20,055,400.8 US Dollars).Conclusion:The rising economic burden of COPD corralates with acute exacerbations. Hypopituitarism may occur in old age with fatigue, decrease in libido, weight loss and musculoskeletal symptoms as a major cause of the overall condition impairment. As one gets older, the signs of these two diseases may progress even without any significant disease. The natural process which comes with old age, and pseudogout and hypopituitarism can mask one to another clinically and the symptoms of three situ- ation may be overlapping. This can lead to difficulty and delay in diagnosis. We present a 59 years old female patient with a polypoid lesion in nasal cavity, together with clinical com¬plaints as recurrent epistaxis and stuffiness. Histological and immunohistochemical examination of the surgically excised le¬sion revealed a diagnosis of epithelialmyoepithelial carcino¬ma with high grade transformation. Epithelial-myoepithelial carcinoma originates from salivary glands, mainly parotid gland. Other affected sites are submandibular gland and minor sali¬vary glands. Epithelialmyoepithelial carcinoma is characterized by double layered duct-like structures. Greater cytologi¬cal atypia, higher mitotic rate, presence of necrosis and loss of the biphasic ductlike structures are features of high grade transformation. Ultrasound (US)-guided insertion of catheters has been used widely, and its safety and efficacy have been demonstrated in several studies. However, this technique is not free from complications, such as carotid artery puncture, hemothorax, pneumothorax and infection. Öztürk","doi":"10.15197/EJGM.01416","DOIUrl":"https://doi.org/10.15197/EJGM.01416","url":null,"abstract":"Objective:To investigate the value of grey scale and Doppler ultrasonography in the assessment of metastatic and lymphomatous cervical lymph nodes.Methods:One hundred and nineteen malignant cervical lymph nodes in 119 patients diagnosed as malignant according to grey scale, color Doppler and spectral Doppler ultrasonography features were included in this study. The sizes, shape, echo pattern, echogenic hilus, nodal border, cystic necrosis and coagulation necrosis, vascular distribution pattern, resistivity and pulsatility index values of lymph nodes were noted. All patients had histopathological diagnosis.Results:The final diagnosis of the 42 lymph nodes was lymphoma, and of the 77 lymph nodes was carcinoma metastasis (33 thyroid carcinoma metastasis and 44 squamous cell carcinoma metastasis). While a hyperechoic pattern and calcification were detected in the lymph nodes originating from thyroid carcinoma as 39% and 33%, respectively, a reticular pattern (26%) and low resistive and pulsatility indices were detected in lymph nodes originating from lymphoma. These criteria were statistically significant for the differential diagnosis (p<0.05). Size, shape, hypoechoic echo pattern, absence of echogenic hilus, nodal border, cystic necrosis, coagulation necrosis and vascular distribution pattern were not found to be significant for the detection of a primary tumour.Conclusion:Hyperechoic echo pattern, reticular pattern, calcification, low resistivity and pulsatility indices are useful parameters for the differential diagnosis of malignant cervical lymph nodes.
