The article presents the results which received during the investigation and treatment of 166 patients with acute ethanol in- toxication on the background of alcoholic fatty liver disease Patients were assessed by the severity scale APACHE-Il. Were studied the dynamics of clinical, laboratory, biochemical parameters, the state of the antioxidant system activity and lipid peroxidation. The study found the effect of the combination hepatoprotective drug remaxol on the many links of metabolism, which was confirmed by the dynamics of biochemical parameters. There was a quick correction of hyperlactatemia, an effective reduction of ALT activity, the absence of reduction of albumin and urea concentration in the somatic period of ethanol poisoning. To demonstrate morphological and genetic features of these tumors and to determine their clinical management.</p><p><strong>Key points: </strong>Described case shows diagnostics of serrated colon adenocarcinoma. A 67-year-old woman presented for colonoscopy because of long constipation besides abdominal pain and distention have appeared recently. Colonoscopy was performed and she was found to have a 3.0 x 3.0-cm tumor next to hepatic flexure and pedunculated 1.2x0.7 cm polyp in sigmoid colon (type lp according to Paris classification). Endoscopic biopsies of these lesions were consistent with serrated adenocarcinoma and tubulovillous adenoma with foci of severe dysplasia. Additional diagnostic methods revealed no distant metastases. Patient was under went right hemicolectomy with D3 lymphodissection, lesion in sigmoid colon was removed using snare with electrocautery. Final pathology revealed no residual tissue in margins and no malignant cells in removed lymph nodes. Genetic diagnostics by PCR found high level of microsatellite instability and positive CpG island methylator phenotype (CIMP+) in tumor tissue.</p><p><strong>Conclusions: </strong>According to WHO classification serrated lesion are considered to have malignant potential. They serve as the precursors for approximately 10% to 15% of sporadic colorectal cancer developed through the serrated pathway. These tumors were named serrated adenocarcinoma and had special morphological criteria that allow us to differentiate them with colorectal adenocarcinomas developed through the traditional pathway. Serrated adenocarcinoma has high level of microsatellite instability and positive CpG island methylator phenotype (CIMP+). Presence of microsatellite instability in tumor determines its more favorable prognosis because of tumor immunogenicity. The first one was 12 animals with 50% wrap-retirement stenosis of the celiac trunk. In the second series of experiments consisted of 8 animals with 50% compression stenosis of the cranial brizee Noi artery. {"title":"ELECTROPHYSIOLOGICAL ANALYSIS OF EXPERIMENTAL ULCERATION STAGES IN RATS.","authors":"I L Privalova,&nbsp;E T Kamal","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Research objective - to conduct electrophysiological analysis for experimental ulceration stages in rats studying changes in the electrical activity (EA) frequency characteristics (FCH) (frequency response) of the stomach and duodenum.</p><p><strong>Materials and methods: </strong>The study was conducted in 5 experimental groups of outbred albino rats weighing 250-300 g with acetate stomach ulcerative injury (SUl). Using the method of direct multi-channel electrocardiography, we recorded the EA of gastric corpus (SB), its pyloric (PO) and the entry part of duodenum. We analyzed the frequency response of EA smooth muscles of stomach and duodenum during the period of seven days after the simulation SUI. The obtained by Shapiro-Wilk test data was distributed in accordance with statistically significant differences between samples - using the Student's t test. Correlation analysis was performed basing on the calculation of Pearson correlation coefficient.</p><p><strong>Results: </strong>The average value of the frequency EA and TJ PO increased on the second day of the experiment 55.7% (p <0.01) and 53.0% (p <0.01), respectively, were down on the fourth day and then increased from 5 to 7 day of the experiment by 87.6% (p <0.01) in TJ and 89.8% (p <0.01) in the PO. The frequency EA LDK increased on the seventh day of the experiment 32,4% (p <0.05). The frequency EA LDK increased on the seventh day of the experiment 32.4% (p <0.05). The aim of our study was to examine the effectiveness of transplantation of allogeneic mesenchymal bone marrow stromal cells for the healing of surgical wounds the glandular stomach in rats.</p><p><strong>Material and methods: </strong>Using white Wistar rats. Producing cell transplantation mononuclear fraction derived from rat bone marrow aspirate. Injected cells 8 and 9 th passage. The dose of cells administered to 3-th and / or 7-th days 3,5h106 cells / ml twice or 5,0h106 cells / ml dose. Autopsy on day 10-th and 17-th. The serum ELISA determined the content of the pro- and anti-inflammatory cytokines IL1β, TNFα, IFNy, IL-4.</p><p><strong>Results and discussion: </strong>Introduction MSCs contributed to the decline of the immune mediators of inflammation IL1P, TNFa, IFNy, increase anti-inflammatory IL4. Quality improved healing.</p><p><strong>Conclusion: </strong>Rapid curative effect of stem cells may be associated with the formation of blood immune cells (macrophages) that produce substances that restore damaged tissue. {"title":"[[Clinical Relevance Helicobacter Pylori Genotypes in Patients with Chronic Pancreatitis and Concomitant Infections Helicobacter Pylori].]","authors":"A S Sarsenbaeva,&nbsp;E V Domracheva,&nbsp;M N Rustamov","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Relevance: </strong>Currently, there is a need to study the genetic diversity of H.pylori in patients with variety of acid-related dis- eases to develop new strategies for the treatment of patients with H.pylori to predict high efficiency of treatment.</p><p><strong>Objective: </strong>To assess the effectiveness of schemes of eradication therapy in patients with chronic pancreatitis and concom- itant H.pylori infection.</p><p><strong>Materials and methods: </strong>The study included 108 patients with H.pylori infection: 63 patients had chronic pancreatitis and were concomitant with H.pylori-infection and 45 patients were without chronic pancreatitis and had H.pylori infection with a chronic gastritis. All patients were determined by factors of pathogenicity of H.pylori by immunoblotting. After forming the group randomized patients received eradication therapy scheme I and the scheme I with inclusion of bismuth tripotassium dicitrate.</p><p><strong>Conclusions: </strong>The effectiveness of H.pylori eradication therapy is dependent on the genetic component of H.pylori.
