Paper presents the data review on the drug activity mechanisms, effectiveness, safety, and major drug interactions of NSAIDs at co-morbid conditions in different age groups, as well as measures of prevention of NSAID-gastropathy, which can occur even against the use of small doses of NSAIDs. {"title":"URGENT COLONOSCOPY IN LOWER INTESTINAL BLEEDING.","authors":"A G Korotkevich, E V Zakharchenko, A S Leont'ev, R A Plyusnin","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The aim of the study was to compare the diagnostic efficacy and safety of emergency colonoscopy without colon preparation at patients with intestinal bleeding.</p><p><strong>Material and methods: </strong>In a comparative cohort study included 252 patients admitted due to intestinal bleeding in 2006- 2015. 118 men, women 134. The average age was 60.15 ± 15,7 years. Patients with explicit anal / hemorrhoidal bleeding at admission or manifestations of gastroesophageal bleeding were excluded. Randomization was performed by alternating days of hospitalization. In Hospital N~ 29 colonoscopy was performed at admission without prior colon preparation, patients of Hospital N~ 1 performed lavage or enema preparation prior to colonoscopy within 24-48hours of hospitalization.</p><p><strong>Results: </strong>Significant differences in the structure of intestinal bleeding source were identified. In patients without bowel preparation most frequently detected bleeding diverticula and cancer - 17%, ulcerative colitis - 1096, intestinal bleeding - 1696, upper bleeding - 16%. At preliminary colon preparation most often as likely causes bleeding source detected cancers - 22%, ulcerative colitis - 15%, angiodysplasia - 13%. Analysis of the dependence of bleeding sources structure from bowel preparation revealed differences only in women. The number of endoscopic findings at colon preparation was 1.5 times higher versus emergency colonoscopy without colon preparation. The number and structure of the observed changes were significantly different between groups, and depending on the sex of patients. {"title":"NEW OPPORTUNITIES OF THE QUALIFYING ENDOSCOPIC DIAGNOSTICS OF PANCREAS NEOPLASMS.","authors":"P V Pavlov, S S Pirogov, E S Karpov, V V Sokolov, S O Stepanov","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>the aim of study was testing and introduction in clinical practice combination of EUS with sonoelastography, fine-needle biopsy and needle-based confocal laser endomicroscopy in advanced diagnostic of pancreatic disease.</p><p><strong>Materials and methods: </strong>in the period from February 2014 to December 2015, we accumulated experience of EUS-FNA in 72 patients mostly with tumor pathology of the pancreas. In 16 cases we used 19G, in 49-22G, and in 7 patients - 25G needles. In 5 patients we used COOK Echotip Procore needle. In 8 cases we performed confocal laser endomicroscopy through the 19G needle (nCLE) in 3 patients with cystic and in 5 - with solid tumors. In 20 patients, there we used.HITA-CHI-PENTAX sonoelastography was performed.</p><p><strong>Results: </strong>There were no complications in EUS-FNA. In all cases we received pathology verification of disease. nCLE results always confirmed by morphological examination of material. In elastography examination normal pancreatic tissue had a equable green-yellow color, in chronic pancreatitis on the same background there were areas of blue seal parenchyma, in the cases pancreatic adenocarcinoma it was marked predominance of blue color scale, in neuroendocrine tumors - diffuse distribution of green and blue areas. Using nCLE in patients with adenocarcinoma revealed the destruction of certain glands, polymorphic nuclei of epithelial cells, with their enlargement, deformation, loss of polarity. Appliance of Procore needle has its technical features, but allowed us to obtain more material with less bloody - due to fewer number of passes.</p><p><strong>Conclusion: </strong>Thus, our study confirms the effectiveness of EUS-FNA with sonoelastography and nCLE. Thoracic involvement is the most common presentation; however, sarcoidosis can involve almost any other organ. Liver is one of the most frequently affected organs after lungs and lymph nodes. Microscopic hepatic involvement is common in these patients but is mostly clinically silent. specific symptoms include jaundice and pruritus, mostly from chronic cholestasis. In a significant portion of these patients, macroscopic findings can be detected by ultrasound examination ,abdominal CT or magnetic resonance imaging and these findings may easily be confused with other benign and malignant conditions of the liver. Liver biopsy is usually required to confirm the diagnosis. Not all cases of hepatic sarcoidosis require treatment. {"title":"THE LONG - TERM RESULTS OF USEFUL ERADICAION PATIENTS WITH PEPTIC ULSERS (a five-year monitoring).","authors":"I V Zverkov, O N Minushkin","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>There was held an useful eradication of Helicobacter Pylon in 20 patients with peptic ulcers (Omeprazole, Clarithromycin and Amoxicillin in normal doses). A five - year monitoring period revealed clinical manifestations of relapse in 50% of cases and endoscopic manifestatiQns in 60% of cases (15% - ulcers and 45% - gastral erosions). Relapse of peptic ulcers was accompanied with change of clinical course, it became oligo- or asymptomatic in 83% of patients.
