The attributable risk of mortality due to physical inactivity is 6 % and the preventive fraction is 15 %. Studies have demonstrated with a high level of scientific evidence that regular physical activity and sport can prevent and treat most chronic non-communicable diseases: cardiovascular disease, type 2 diabetes, colon, breast and endometrial cancer. Physical activity also contributes to the prevention of the main risk factors of these diseases. In France, regardless of age group, PA is considered insufficient in relation to WHO recommendations and sedentary times reach 12<!--> <!-->h/day. Children are the most impacted by this association of “inactivity and sedentary lifestyle”. Due to a lack of evidence based data, this management is based on published data for severe respiratory infections such as SARS and expert opinion. While there is no effective treatment to date, new promising therapeutic strategies and targets are being investigated. Innovative cell therapies are reaching clinical trials, and the most recent progress in our understanding of the pathology is opening new routes for gene therapy. In the long term, the development of new biofabrication tools such as 3D bioprinting might pave the way for the use of personalized mini-joint models that would allow clinicians to screen drugs and personalize treatments. Nevertheless, international guidelines are not unanimous as to their use in the knee, hip or hand osteoarthritis. This discrepancy is explained by the results of meta-analyses of randomized controlled trials, showing a modest effect size and a disputable clinical relevance. Indeed, clinical trials do not take into account the contextual effect associated with the intra-articular procedure. This effect contributes to the greater placebo effect observed with intra-articular therapies than with oral or transdermal treatments, and the difficulty in demonstrating a clinically relevant effect of intra-articular drugs. Intra-articular injections have an advantageous safety profile compared to other therapies available for the management of osteoarthritis pain. The long-term tolerance of intra-articular corticosteroids remains uncertain, as they have been associated with accelerated structural progression, especially if they are repeated multiple times. Finally, the efficacy of platelet-rich plasma is debated. Its medical management is currently based on European recommendations and includes pharmacological (topical NSAIDs, and infiltrations) and non-pharmacological measures (orthotics, and ergonomics). Conventional synthetic or biological treatments used in rheumatoid arthritis have not demonstrated any efficacy. The only positive trial in recent years is low-dose corticosteroid therapy, which has demonstrated a suspensive effect. However, new therapeutic options such as botulinum toxin infiltrations or vagus nerve stimulation are currently being evaluated. As far as surgical treatment is concerned, arthrodesis remains the intervention of choice for distal interphalangeal OA and new fixation methods allow immediate mobilization. At the proximal interphalangeal level, joint denervation is an interesting conservative solution, while the best functional result is obtained with a prosthesis. The new models of resurfacing implants give higher amplitudes than hinged silicone implants, but we do not yet have the hindsight we have for the latter. For thumb base OA, trapezectomy with or without suspension ligamentoplasty has been confronted in recent years with severe competition from trapeziometacarpal prostheses, which allow faster functional recovery despite a higher complication rate and a long-term survival that is still poorly known. It can be adapted to a person's health status and have the objective of maintaining or improving health status. The link between sedentary lifestyle and the severity of symptoms in osteoarthritis is now well established, making adapted physical activity (APA) a first-line targeted treatment in osteoarthritis, in order to fight against the effects of a sedentary lifestyle, regardless of the phenotype or stage of the disease. The latest recommendations from EULAR, ACR and OARSI place APA, in the form of structured exercise programs for muscle strengthening, joint mobility, proprioception and aerobic work, as the core treatment for people with osteoarthritis. The benefits of APA in reducing pain and activity limitations, in the short, mid and long terms, in osteoarthritis, especially of the lower limbs, has been demonstrated in high-level clinical trials and meta-analyses, with effect-sizes comparable to those observed with pharmacological treatments. In clinical practice, the prescription of APA in people with osteoarthritis should still be encouraged. Novel therapeutic strategies that aim at eliminating senescent cells, called senotherapies, have been launched during the past few years. A number of therapeutic targets and drugs have been identified and are being evaluated in pre-clinical and clinical phases. The main chronic conditions found in osteoarthritic patients include high blood pressure, cardiovascular diseases (coronary artery disease, heart failure, stroke/transient ischemic attack, peripheral artery disease, and cardio-vascular diseases (CVD)-related hospitalization or mortality), peptic ulcer, type II diabetes, obesity. Metabolic syndrome is more common. Comorbidities have aggravating consequences for osteoarthritis. They will be able to change the perception of pain or influence the course of the disease. This link seems to vary according to different locations of osteoarthritis. The technical improvements over the past two decades have not brought the expected benefits, and it is the changes in perioperative management that are responsible for the most obvious changes, with an enhanced patient experience. Thus, rapid recovery after surgery includes effective multimodal analgesia allowing early mobilization of patients with reduced risk of thromboembolism, a reduction in the risk of bleeding and transfusion by the introduction of tranexamic acid intraoperatively, even allowing the management of some selected patients on an outpatient basis.
