Abstract
The number of total hip and knee replacements in France continues to increase due to the prevalence of osteoarthritis, excellent clinical results and long-term survival of the implants. The technical improvements over the past two decades have not brought the expected benefits, and it is the changes in perioperative management that are responsible for the most obvious changes, with an enhanced patient experience. Thus, rapid recovery after surgery includes effective multimodal analgesia allowing early mobilization of patients with reduced risk of thromboembolism, a reduction in the risk of bleeding and transfusion by the introduction of tranexamic acid intraoperatively, even allowing the management of some selected patients on an outpatient basis. The identification of preoperative risk factors makes it possible to screen patients at risk of complications and correct some of these risk factors before performing the prosthesis.
