In patients with a history of vascular event, and in those under primary prevention and presenting with associated risk factors, the benefit of a statin treatment is indubitable, whatever the initial level of LDL-cholesterol. These studies provide also new evidence on beneficial results of statin therapy in diabetic patients, in women, and in elderly patients with high vascular risk. In the future, statin indication in any patient should depend primarily on the degree of his vascular risk. Orphan nuclear receptors do not have obvious naturally occurring ligand. Nuclear receptors display a modular structure with similar characteristics: transactivation domain, ligand binding domain (LBD) and DNA binding domain (DBD). They are partly responsible for the increased cardiovascular risk observed in this population, particularly in type 2 diabetic subjects. Type 2 diabetic patients present both quantitative and qualitative abnormalities of lipoproteins, when type 1 diabetic patients in good (or fairly good) glycemic control have only qualitative abnormalities. Quantitative abnormalities of lipoproteins are elevated triglycerides and decreased HDL-cholesterol level. Main qualitative abnormalities are large triglyceride-rich VLDL (VLDL<sub>1</sub>), small dense triglyceride-rich LDL, increased oxidation of LDL, triglyceride-rich HDL and glycation of apolipoproteins. Both quantitative and qualitative abnormalities are atherogenic. In type 2 diabetes, insulin resistance and insulin \"relative\" failure play a crucial role in the physiopathology of lipid disorders. Treatment of diabetic hyperlipidemia, which is important to reduce cardiovascular risk, is based on diet, physical activity, glycemic control and hypolipidemic drugs (fibrates, statins). In addition to hyperglycemia, smoking is a potent risk factor for the development of arteriopathy. In diabetic patients, clinical signs of the disease are not specific, but arteriopathy is rarely isolated and often combined with neuropathy and infection, leading to the aspect known as the “diabetic foot”. First-line non-invasive vascular investigations are always carried out, but lower limb arteriography is often required; this procedure should be cautiously used in diabetic patients. Therapeutic management requires at first assessment and treatment of infection. Then arterial restoration can be considered, using either percutaneous angioplasty or by-pass, but amputation can occur at any step.
