Guo, Mengling Zhao, Yun Feng, Fen-Fen Chu, Miaomiao Zhang, Qian Wang","doi":"10.3760/CMA.J.ISSN.1674-4756.2020.05.003","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1674-4756.2020.05.003","url":null,"abstract":"Objective \u0000To compare the curative effects on high-grade squamous intraepithelial lesion (HSIL) by performing total hysterectomy (LTH) immediately after fast frozen section examination (FSE) and performing total hysterectomy after fast FSE frozen section examination and conventional pathological determination. \u0000 \u0000 \u0000Methods \u0000Eighty-four patients with HSIL diagnosed in Pathology Department of the First Affiliated Hospital of Zhengzhou University from January 2016 to April 2017 who were going to have total hysterectomy operation were divided into two groups by their own willingness on operation. Patients in one-step operation group were given cervical cold knife conization (CKC) for FSE; if patients were diagnosed as non-cancer by FSE, patients underwent total hysterectomy immediately. Patients in two-step group underwent CKC for FSE first; if patients were diagnosed as non-cancer by FSE, clinicians ended the surgery; then, patients experienced routine pathological examination; if the results of pathological examination proved not to be cancer, patients underwent total hysterectomy; if the FSE showed a cancer result, patients underwent the radical operation of cervical cancer. The hospital stay, treatment coast, intraoperative bleeding volume, postoperative complication, anxiety degree and inappropriate rate of operation were compared between the two groups. \u0000 \u0000 \u0000Results \u0000Compared to two-step group, one-step group had shorter hospital stay, less hospital coast, lower anxiety degree and shorter time for the first anal exhaust (P 0.05). \u0000 \u0000 \u0000Conclusions \u0000Performing CKC and total hysterectomy at the same time has more advantages, however, it also has limited accuracy. The curative effects were analyzed. The pregnancy outcomes were compared among patients with different cervical dilatation degrees, and the related factors influencing prognosis were analyzed. \u0000 \u0000 \u0000Results \u0000After ECC, gestational weeks of 36 CIC patients were prolonged by (8.55±4.86)weeks, and the neonatal survival rate was 83.33%(30/36). Compared to cervical dilatation degree>6.0 cm group, cervical dilatation degree≤6.0 cm group had longer prolongation time of gestational week, less abortion cases and better neonatal weight, and the difference between the two groups was significant (P 2.5 cm group had longer prolongation time of gestational week, less abortion cases, better neonatal weight, and the difference between the two groups was significant (P 6.0 cm were independent risk factors influencing pregnancy outcomes (P<0.05). \u0000 \u0000 \u0000Conclusions \u0000The perinatal outcomes are relatively better in CIC patients with cervical dilatation degree ≤6.0 cm and cervical length >2.5 cm. They were divided into study group and control group according to the random number table method. Patients in the control group were treated by conventional chemoradiotherapy and intracavitary brachytherapy, and patients in the study group were treated by hyperthermia on the basis of treatment for control group. The curative effects and the changes of leptin and insulin like growth factor 2 (IGF2) in cervical cancer cells before and after treatment were compared between the two groups. After treatment, patients in the two groups were followed up for 3 years, and the tumor-free survival and death of the two groups were recorded. \u0000 \u0000 \u0000Results \u0000The short-term total effective rate of the study group was 74.00% (148/200), higher than the 53.00% (106/200) of the control group (P 0.05). After treatment, the levels of IGF2 and leptin in the study group were lower than those in the control group (P<0.05). All patients were divided into two groups according to treatment methods, with 36 cases in each group. Patients in the control group were treated with conventional western medicine, and patients in the observation group were treated with Danzhi Xiaoyao powder. The levels of serum sex hormone, improvement of skin rash and clinical therapeutic effect were compared between the two groups. \u0000 \u0000 \u0000Results \u0000Compared with the preoperative indexes, the serum sex hormone levels and global acne grading system (GAGS) scores in the two groups were lower after treatment, and the indexes in observation group were lower than those in the control group, and the difference was statistically significant (P<0.05). Drug sensitivity tests were carried out by proportional method. The genotypes of 120 strains of Mycobacterium tuberculosis were analyzed by polymerase chain reaction (PCR) and spacer oligonucleotide typing (spoligotyping). \u0000 \u0000 \u0000Results \u0000Of the 120 strains of Mycobacterium tuberculosis, 78 strains were sensitive to isoniazid, rifampicin, ethambutol and streptomycin, and 42 strains were resistant to these drugs, with the drug resistance rate of 35.00%. Eighty-three strains (69.17%) were Beijing genotype, 37 strains (30.83%) were non-Beijing genotype. The control group was treated with conventional chemical mutilation, while the observation group was treated with radiofrequency thermocoagulation of lumbar sympathetic nerve. Preoperative and postoperative pain scores, postoperative quality of life scores and preoperative and postoperative skin temperature changes of lower limbs were compared between the two groups. \u0000 \u0000 \u0000Results \u0000There was no significant difference in skin temperature of lower limbs between the two groups before treatment (P>0.05); at 1, 2 and 3 months after treatment, there was a significant difference in skin temperature of lower limbs (P 0.05). Twenty-two cases in control group were treated by posterior pubic approach, and 15 cases in observation group were treated by laparoscopy. The perioperative indexes such as operation time, indwelling catheter time, hospitalization time, intraoperative bleeding volume and pain score were recorded and compared between the two groups. The patients in group A underwent CE examination within 3 day stoping bleeding, while the patients in group B 3 d after stoping bleeding. \u0000 \u0000 \u0000Results \u0000The total positive rate of CE in 60 patients was 90%. The proportion of vascular malformation, multiple ulcers or erosion were higher than other types (P<0.05). The results of MRA were 2 cases, 10 cases and 3 cases, which were consistent with arthroscopic results. The relationship between EGFR gene mutation and clinicopathological characteristics including sex, age, smoking history, tumor markers, adenocarcinoma pathological classification, extent of differentiation in patients with lung adenocarcinoma were analyzed, further searched for factors that predicted the mutations of EGFR. \u0000 \u0000 \u0000Results \u0000In the 194 patients with lung adenocarcinoma, there were 120 cases (61.86%) with EGFR gene mutation, the mutation rate of female was higher than that of male (χ2=10.746, P=0.001), the mutation rate of EGFR gene in patients of age≥60 was higher than that in patients of age 0.05). Among the main pathological types of lung adenocarcinoma, the proportion of acinar type was the highest in EGFR mutation group(63.9%, 124/194), the others were solid type, papillary type, adherent type, mucinous adenocarcinoma, the differences between the main pathological types and EGFR gene mutation, the differentiation degree of each excised lesion were statistically significant (χ2=10.275, 17.131; P=0.001, 0.001). There were more exons 19 and exon 21 in EGFR mutants, further analysis showed that there was significant difference between different pathological sub-types and mutations of exon 19 and exon 21(χ2=18.613, P=0.001). \u0000 \u0000 \u0000Conclusions \u0000EGFR gene mutation is related to sex, age, smoking history, main pathological types and tumor differentiation degrees, but it is not related to tumor marker.
