{"title":"Lékové interakce - rizika u pacientů s polypragmazií.","authors":"Karolína Russ, Tereza Vaňková","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Polypharmacy, a phenomenon of today's medicine, brings a great risk of drug interactions. Some of them can lead to serious side effects or treatment failure, knowledge in this field is dramatically developing. Pharmacokinetic drug interactions can occur not only when drugs are metabolized by the cytochrome P450 isoenzyme system, but also at the level of transporters, a number of which have already been described. In order to assess their clinical significance, it is necessary to orient oneself in the pharmacodynamics and pharmacokinetics of drugs, but also to consider other factors that will influence the impact of drug interactions. The physician should be familiar with commonly prescribed drugs with a high interaction potential, be aware of strong inducers and inhibitors of CYP450, as well as the possible influence of genetic polymorphism of some of its isoforms. However, one should not rely on the interpretation of drug interactions only using available interaction databases. No software is yet capable of evaluating interactions comprehensively and in the context of a specific patient, their health status and comorbidities. Also, the information available from clinical studies and observations needs to be properly understood and translated into real practice. Nowadays, it is a great advantage a doctor can consult with a clinical pharmacist or pharmacologist on the issue of drug interactions in a specific patient. {"title":"Ancient sanctuary of Asclepius, god of medicine, on the island of Paros (Greece).","authors":"Tomáš Alušík, Pavla Alušíková Dostalíková","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Asclepius was the main healing deity of the Classical Antiquity. After his gradual establishment, his cult expanded throughout the Greek world, especially in the 5th and 4th centuries BC. Asclepius was worshipped in sacred precincts labelled asklepieia (singular asklepieion), which served both as religious sites and as medical facilities where the sick came for healing. One of the smaller asklepieia is located on the island of Paros in the Cyclades, on two terraces about 3 km southwest of the centre of Parikia. The site was partially excavated in 1898-1899 by the German archaeologist Otto Rubensohn (1867-1964) and incompletely published in 1902. Over the last few years, the authors have been working on a project to (re)identify all surviving finds based on original descriptions and original excavation documentation, and to better understand the healing practices at the site. From the 6th century BC at the latest, the cult of Apollo, a deity with certain healing powers, is attested here, who was succeeded by his mythological son Asclepius sometime between the end of the 5th and the beginning of the 4th centuries BC. During the 4th and probably also in the 3rd century BC, a large complex is built on both terraces, with a temple, retaining and enclosure walls, a specific circular structure, a building for pilgrims and the performance of healing sleep, an altar and 2 sacred healing springs. Despite all efforts to improve the original procedures and improve the results of care for patients with cardiac arrest, the basic recommendations and rules of modern emergency resuscitation remain practically the same as at the time of its inception. In recent years, in order to increase the number of patients surviving cardiac arrest, urgent resuscitation has been used in combination with the method of extracorporeal cardiopulmonary oxygenation (ECPR). Main symptoms are hot flashes and night sweats. Each symptom could be presented alone or commonly in combination with other symptoms. The acute climacteric syndrome is induced by decrease and fluctuations of estrogen and neurosteroids levels. Therapy could be focused on hormone replacement. Artificial intelligence in medical imaging.","authors":"David Zogala","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The current era witnesses a highly dynamic development of Artificial Intelligence (AI) applications, impacting various human activities. Medical imaging techniques are no exception. AI can find application in image acquisition, image processing and augmentation, as well as in the actual interpretation of images. Moreover, within the domain of radiomics, AI can be instrumental in advanced analysis surpassing the capacities of the human eye and experience. While several certified commercial solutions are available, the validation and accumulation of sufficient evidence regarding their positive impact on healthcare is currently constrained. The role of AI presently leans towards being assistive, yet further evolution is anticipated. Risks and disadvantages encompass dependency on computational power, the quality of input data, and their annotation for learning purposes. The transparency of algorithmic functioning is lacking, and issues pertaining to portability may arise. The integration and utilization of AI introduce entirely new ethical and legislative aspects. {"title":"Changes in contraceptive behavior in Czechia.","authors":"Jiřina Kocourková, Jitka Slabá, Bára Idlbeková","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The rapid increase in the proportion of women using hormonal contraception in the 1990s was positively reflected in a rapid decline in the number of abortions. Czechia was unique not only among Eastern European countries, but also worldwide. At the same time the decline in the prevalence of hormonal contraception from a peak of almost 50 % in 2007 to 30 % in 2021 meant a slowing and gradual halt in the further decline in abortions. The results of the GGP 2020-2022 survey in Czechia showed that the lower use of hormonal contraception among women was only partly offset by the increased use of other reliable methods of protection against unintended pregnancy (e.g. condom use). The largest decline in the use of hormonal contraceptives in the form of the pill occurred among the youngest women aged 18-27 years, from 76 to 37 %, which was partly reflected in the more intensive use of condoms (an increase from 21 to 35% in the 18-27 age group), but is worrying, that this age group saw the largest increase in the use of less reliable methods (withdrawal from 11 to 22 % and an increase in the use of the barren days method from 1 to 6 %) and also the largest increase in the proportion of women using neither method (from 7 to 17 %). The lowest proportion of female hormonal pill users was found among female with higher education. However an important finding is that when less reliable methods are used, there is an effort to combine at least two methods. {"title":"Infertility problems in the context of reproductive ageing.","authors":"Anna Šťastná, Adéla Volejníková","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The article evaluates the prevalence of infertility problems in the Czech population, identifies methods used by individuals or couples attempting to achieve pregnancy and evaluates in more detail the use of assisted reproduction technology (ART) in contemporary Czech society. The results show that 27% of women and men in their forties declare they have experienced a time when they were trying to get pregnant but did not conceive within at least 12 months. In the general population of reproductive age, one in five declares experience with methods helping to get pregnant. Methods that do not require a doctor's visit are the most frequently used (one in ten declare monitoring ovulation), and 5% of the general population have experience of ART. WHO Europe has organized surveillance of childhood obesity (Childhood Obesity Surveillance Initiative, COSI) since 2008, which observed the prevalence of overweight and obesity of 6-9-year-old children is followed during this study and proved this result. The study Children's Health 2016 showed that after a period of the global increase of obesity until 2011, there was in the Czech Republic a period of certain stabilization, in which there weren´t major changes in weight. Unfortunately, the covid pandemic changed this trend and the current data from 2021 showed in the Czech Republic a serious increase in childhood obesity. As with other diseases, there are effective treatments - psychobehavioral interventions and pharmacotherapy to suppress withdrawal symptoms. The aim of this analysis was to evaluate the development of reporting of selected intensive tobacco dependence treatment interventions in the Czech Republic. Data provided by the National Register of Covered Health Services of the Institute of Health Information and Statistics of the Czech Republic for the period 2010-2022, with a distinction per county and anonymized per centers for tobacco dependence and trained physicians. Between 2010 and 2022, an average of 1094 initial intervention procedures were reported annually in the country, indicating the number of smokers treated, and 2584 control visits. The Centre for Tobacco Dependence of the General University Hospital in Prague had the largest share in the total reporting of 25501 procedures (average 35.5 %). Intensive treatment for tobacco dependence should be offered and available as treatment for other diseases. Působí jako zásobárna energie, tepelný izolant a tlumič nárazů a zastává také roli v endokrinním systému. V posledních letech byl objeven její regenerační a protizánětlivý potenciál. Za tyto funkce jsou zodpovědné různé typy progenitorových, kmenových a zralých buněk jiných než adipocyty, které se nacházejí v tukové tkáni a souhrnně se označují jako stromální vaskulární frakce. V retikulární dermis tukové buňky obklopují vlasové folikuly a úzce regulují vlasový cyklus. Intradermální progenitorové adipocyty jsou metabolicky aktivní a vylučují řadu signálních molekul, které jsou zodpovědné za indukci anagenní fáze a růst nových vlasů.
