Examination revealed a large intraocular mass with overlying retinal detachment. B-scan ultrasonography revealed an intraocular mass with foci of intralesional calcification. Systemic workup was unremarkable. MRI of the brain and orbit ruled out extra ocular spread. Based on clinical suspicion of either amelanotic melanoma or adult-onset retinoblastoma, fine needle aspiration cytology (FNAC) was done which revealed retinoblastoma. Aims: To report a novel technique of using ACenSS 23G Vitrectomy cutter for trocar surgery with an external light source in Teaching hospital Rathnapura Sri Lanka from 01/03/2022 to 30/09/2022. Methods: 23G Alcon light probes were used with Retikare (ICON Medicare Tamil Nadu India) external light source for endoillumination. Infusion pressure was set at 65cmH2O (47.81 mmHg) and infusion line connected to eye through a 3 ways tap. ACenSS-GFMS aspiration line connected to aspiration line of vitrectomy probe. Cryopexy used to seal retinal tears as no endolaser facility. Volk surgical corneal lenses with Zeiss Lumera I microscope provided the viewing. ACenSS foot pedal was programmed to 3 modes of vitrectomy with navigation, cutter on/ off function and on/pause of continuous irrigation. A gravity driven another infusion line is used to keep the infusion when endo diathermy is used. Results: When working on maximum cut rate of the ACenSS GFMS (5000 cpm) for long duration machine automatically stops and a restart with reprime needed. Therefore, cut rates were set at a lower rate to work without interruption (core vitrectomy2000 cpm, peripheral trim- 3000cpm and for epinucleus removal 4000 cpm). No retinal traction or ridging of retina was observed at peripheral trim. Total of 210 TPPVs were performed with this technique. Space occupying lesions in the orbit include infections, inflammations, vascular malformations, and malignancies. The significant variation in presentations is due to the complex anatomy of the orbit and the heterogeneous nature of the multiple disease processes that present themselves as orbital inflammatory processes. Additionally, although specific disease entities often show similar patterns of orbital tissue involvement, there is still a spectrum of clinical presentations within disease processes, which furthermore overlap with other inflammatory etiologies. This heterogeneity creates a significant challenge in determining specific diagnoses and subsequently instituting timely medical and surgical management of patients with orbital inflammation. Despite advances in imaging, physical examination, and laboratory tests, a biopsy is often needed for diagnosis and to guide treatment. Unfortunately, the biopsy is too often read as non-specific or idiopathic inflammation, a term that gives minimal guidance to the patient or to the clinician. This study was undertaken to determine the prevalence and antimicrobial susceptibility pattern of microorganisms associated with ocular infections. Methods: A hospital based cross-sectional study was conducted at four (4) tertiary hospitals in Abia State. Ocular specimens were collected from 500 patients. Subsequent identification was done based on morphology and biochemical tests. Susceptibility pattern of the isolates were done using the disk diffusion method. Results: The prevalence of ocular infection was 264(52.8%). Conjunctivitis was the most prevalent ocular infection of 105(39.8%) followed by Blepharitis 76(28.8%). S. aureus was the most prevalent pathogen 63(23.9%) followed by CoNS 36(13.6%). S. aureus was 100% sensitive to vancomycin and chloramphenicol. CoNS were also 100% sensitive to ciprofloxacin, vancomycin and chloramphenicol. K pneumoniae was 100% sensitive to gentamicin and Amoxicillin-clavulanic acid while N. gonorrhoeae was 100% sensitive to gentamicin, ciprofloxacin, ceftriaxone, Amoxicillin-clavulanic acid and cefotaxime. The overall MAR bacteria were 38(16.2%). Conclusion: The prevalence of ocular infection was high with Conjunctivitis being the dominant. The dominant bacteria species were S. aureus and CoNS. The overall MAR bacteria proportion was relatively high. {"title":"SARS-Cov-2 Associated Paracentral Acute Middle Maculopathy: A Case Report with a Challenging Diagnosis","authors":"Salceanu So","doi":"10.23880/oajo-16000251","DOIUrl":"https://doi.org/10.23880/oajo-16000251","url":null,"abstract":"We present an intricate case of Paracentral acute middle maculopathy (PAMM) as part of the COVID-19 complex clinic presentation. 53 years old patient, pediatric nurse developed symptoms specific to SARS-Cov-2 infection (dry cough, headaches, myalgia’s and fever) which prompted her to get tested. 11 days into the disease, she experienced right eye mild loss of vision and received a short course of steroids for her concomitant pulmonary disease with no subjective improvement on vision. After 2 weeks of self-isolation, she had an ophthalmology examination and was misdiagnosed with optic neuritis. As her visual acuity was 6/6 she was monitored closely with perimetry and OCT. Patient showed reduction of central scotoma with no further treatment. Methods: Patients (39 eyes) after implantation of two models of spherical lenses with a different multifocal structure, i.e. PanOptix® by Alcon® and Acunex®VARIOMAX® by Teleon® were examined using the iTrace analyser, the Toric Alignment Check module. Results: In each case, patients undergoing implantation of spherical multifocal lenses were diagnosed with astigmatism generated by these lenses. Astigmatism ranged from 0.15 Dcyl to 1.82 Dcyl and differed in the value and axis from corneal astigmatism. Conclusion: Spherical multifocal lenses generate astigmatism, independently of the structure of the lens. Method: An observational and comparative study was carried out at Chittagong Eye Infirmary and Training Complex from January 2016 to January 2017. Two techniques for IOP measurements using the standard GAT and the non-contact tonometer (NCT) were compared. A total of 400 eyes from 200 patients were included in the study. Results: The mean IOP as measured by GAT in the right eye was 14.50±5.59mmHg and in the left eye was 14.87±7.03mmHg while that as measured by NCT in the right eye was 15.97±6.12mmHg and in the left eye was 15.94±6.98mmHg. The mean difference between the two methods of measurement in the right eye was 1.47±0.53mmHg and the left eye was 1.07±0.05mmHg. The readings obtained by NCT were higher than those obtained by GAT. There was no statistically significant difference found in IOP measurements between GAT and NCT according to patient’s age, gender or laterality of eyes. Conclusion: There was a significant difference in the measurement of IOP between GAT and NCT. Method: Descriptive case report. Case Summary Case 1: 14 years female came with Primary optic atrophy (POA) of both eyes associated with poor intelligence. MRI brain showed disproportionate dilatation of both occipital horn of lateral ventricle suggestive of Colpocephaly. Case 2: 16 years male presented with POA of eyes, horizontal jerky nystagmus and left sided hemiparesis with contracture of left hand since birth. MRI brain suggestive of Open lip Schizencephaly. Case 3: 12 years boy came with alternate exodeviation, left sided hemiparesis since birth. MRI brain suggestive of right sided Procencephalic cyst. Conclusion: Cephalic disorders caused by disturbance in development of fetal nervous system. For most cephalic disorder’s treatment is only symptomatic and supportive.
