Laparoscopic Management of Large Benign Ovarian Cysts 大良性卵巢囊肿的腹腔镜治疗

But its effectiveness when dealing with large cysts is unclear. Our aim is to evaluate the results of laparoscopy in routine management of huge benign ovarian cysts. Materials and Methods: We performed a prospective study from August 2014 to January 2016. We enrolled 42 patients with large benign ovarian cysts laparoscopically managed in the department of Gynecology and Obstetrics \"A\" at Charles Nicolle’s hospital. Results: Eight patients had emergency laparoscopy for suspected adnexal torsion. The laparoscopic exploration revealed a right ovary cyst in 63% of cases and a left ovarian cyst in 35% of patients without any extra-cystic vegetation. A cystectomy was performed in 42% and an adnexectomy in all the other cases. A bilateral adnexectomy was performed in 25% of cases. A laparoconversion was needed in 32.7% of cases. No vascular, digestive or infectious complications were observed. The mean duration of the operative procedure was 115 minutes. The average hospital stay was 3 days. Pathological examination of the surgical specimens revealed a serous cystadenoma in 33 cases. Although the prognostic significance of nutritional status among patients with cancer remains controversial, it is generally accepted that the nutritional support is an important aspect of medical therapy. Objective: Assess the nutritional status of children with acute lymphoblastic leukemia at time of diagnosis of disease, assess the nutritional status of the same children during induction of chemotherapy and after induction by 3-6 weeks. Patients and methods: A prospective study was carried out on thirty children their age ranged from (1-14 year) with newly diagnosed acute lymphoblastic leukemia, (16) of them were males and (14) were females, who were admitted to the oncology pediatric Unit in Basrah Maternity and children Hospital, from the first of January to the end of October -2010). This study has been done to determine the nutritional status of children with newly diagnosed acute lymphoblastic leukemia before, during and after induction by (3-6 weeks). Full history was taken from each patient regarding chief compliant, dietary history, also clinical examination and anthropometric measurement by measure weight, height, mid upper arm circumference and body mass index, in addition investigation in form of Hb, serum albumin, serum cholesterol, and random blood sugar. Results: This study has revealed that among 30 patients with newly diagnosed ALL, 15 (50%) of them are underweight according to weight for age, 8 of them are male and 7 are females. Twelve (40%) of these 30 patients are wasted, 7 of them are male and 5 are female. Fourteen (46.7%) of the total 30 patients are malnourished according to body mass index, 6 of them are male and 8 are female, ten (33.3%) are malnourished according to mid upper arm circumference, five of them are male and 5 are female. The study has revealed that there is a statistically significant increase in mean body weight during induction with P value (0.046) and a statistically increase in body mass index during induction with P value (0.005). During induction, there is decrease in serum calcium with P value (0.001), and a significant decrease in serum albumin with P value (0.005), also in this study there is a significant decrease in serum cholesterol during induction with P value (0.001). There is no significant increase in random blood sugar during induction. Conclusion: Malnutrition exists in a significant proportion of children with acute lymphoblastic leukemia. However, resistance to ET frequently occurs during the course of treatment. Cellular pathways involved in cell proliferation are targets for new drugs that interfere with development of resistance to ET. Cyclin-dependent kinases (CDKs) are a subgroup of serine/ threonine kinases that play a key role in regulating cell cycle progression. Honda","doi":"10.21767/2254-6081.100156","DOIUrl":"https://doi.org/10.21767/2254-6081.100156","url":null,"abstract":"Background: The purpose of this study was to retrospectively investigate the association between 2-[F-18]-fluoro-2-deoxy-D-glucose (18F-FDG) accumulation in lung metastasis (LM) before 131I therapy and refractoriness to 131I therapy for differentiated thyroid carcinoma (DTC) patients. Methods and Findings: Sixty-one DTC patients with LM who underwent Positron emission tomography/computed tomography using 18F-FDG (18F-FDG PET/ CT) before an initial 131I therapy were retrospectively evaluated. Maximum of standardized uptake value (SUVmax) in LM with the highest 18F-FDG accumulation was measured in each patient. The SUVmax was compared between patients with and without 131I-positive LM, and between patients with and without an increased level of thyroglobulin (tumor marker) 12 ± 2 months after 131I therapy using the Wilcoxon test. Discussion: Predictability for the patients with an increased thyroglobulin level was also analyzed by receiver-operating-characteristic (ROC) analysis. SUVmax of LM was significantly greater for patients without 131I-positive LM than for those with 131I-positive LM (5.9 ± 6.0 vs. 1.9 ± 2.0, p 3.8 showed an increased TG level after 131I therapy. Use of the optimal cutoff threshold for SUVmax of 1.6 differentiated patients with an increased level of TG from those without at a sensitivity of 74.2%, a specificity of 94.4%, an accuracy of 81.6% and an AUC of 0.91. MASCC score classifies FN to low and high risk. Methods: A prospective, observational cohort study of consecutive febrile neutropenia episodes were conducted in the Department of Medical oncology, Ramaiah medical college and hospital, Bengaluru, from (October 2014– September 2016). The inclusion criteria were confirmed malignancy and febrile neutropenia secondary to chemotherapy. The neutropenic episodes were stratified into high and minimal risk on MASCC score. Clinical, hematological and biochemical laboratory parameters were collected and compared with low and high MASCC score. Results: Hundred febrile neutropenia episodes were recorded; 85 in solid cancers and 15 in haematological cancers. Breast cancer was the most common cancer (27 out of 100) and E. coli was the major organism which was cultured (9 cases). The association of the MASCC score and the duration of absolute neutrophil count recovery were found to be statistically significant (p<0.001). Presence of medical comorbidities also predicted poor outcome. High grade fever, fever >7 days, tachypnea, hypotension, renal failure, culture positivity, MASCC <21, ANC <50 cells, febrile neutropenia episodes in admitted patients predicted poor outcome (p=0.001). Conclusion: The MASCC score identified patients with febrile neutropenic episodes into elevated risk and low risk. Gram negative bacteremia is the predominant cause of febrile neutropenia in our setup.
