Both diagnoses heavily rely on morphological evaluation because SM is rarely suspected in clinical practise.\u0000Objective: This case highlighted a possible delay in diagnosis due to underlying conditions or diseases.\u0000Case Report: We present a case of a patient with acute myeloid leukaemia (AML), acute myelomonocytic subtype, and concurrent mastocytosis according to World Health Organization (WHO) classification. Due to an extensive accumulation of AML blast cells that obscured the mast cell infiltrates, mastocytosis was not evident at the first diagnosis.\u0000Discussion and conclusion: The diagnosis, in this case, was established only in the third bone marrow biopsy after chemotherapy. A high index of suspicion with morphological and immunohistochemical evaluations for neoplastic mast cell populations should be considered. Haque, Rawa Kamal Abdelrahim","doi":"10.3329/bjms.v22i4.67107","DOIUrl":"https://doi.org/10.3329/bjms.v22i4.67107","url":null,"abstract":"Background: The facial spaces in head and neck are the potential spaces between the various layers of fascia normally filled with loose connective tissue and bounded by anatomical barriers, usually of bone, muscle or facial layers.\u0000Objective: To evaluate the level of knowledge and awareness about space infection among pediatric patients and their parents in Riyadh, KSA which can lead to seeking early help.\u0000Results: A total of 102 children (56 girls, 46 boys; mean age from 2-12 years) visited Dar al Uloom hospital since January 2019. Most of the patients (51 cases) were children aged 6–9 years. It has been observed that majority of 10- 12 years old children noticed pain first (58.3%) followed by abscess drainage (25%) whereas, large number of 2-5 years old children noticed pain as the first option (66.7%) followed by swelling (26.7%). Further, a greater number of 6-9 years old children noticed pain (80.4%) followed by swelling (7.8%). These results suggest that the association of the first symptom observed by the child with different age groups was found to be significant.\u0000Conclusion: In our study, the most common age group of pediatric patient infected with space infection was from 6-9 years, and the most common source of infection was caries. The level of education does not affect the awareness. It was observed that the symptoms of space infection were first noticed by the child, and it was due to the lack of early visits to the dentist. UDT is the majority factor in male adults. Studies on the effects of UDT on hormoneassociated hypogonadism are limited.\u0000Materials and Methods: This study aimed to assess the effects of UDT on fertility and testicular hormonal function in adult men. Data from medical records were gathered, laboratory tests, and retrospective interviews from 2018 to 2020. The independent variable was men with UDT, while the dependent variable was fertility and reproductive hormones that trigger hypogonadism. Data on infertility, reproductive hormones, semen analysis, and symptoms of hypogonadism from 20 patients with and without UDT. ANOVA, Kruskal-Wallis test, Pearson chi-square test, and Fisher’s exact test helped perform the analysis.\u0000Results and Discussion: Undescended testes were associated with infertility so that levels of FSH and LH were considerably greater than men without UDT (p < 0.05). Bilateral UDT men had the highest levels of FSH and LH but not significantly different from unilateral UDT (p > 0.05). Bilateral UDT men had the lowest mean estradiol, testosterone and sperm quality (p < 0.05). All men had prolactin and AMH levels within normal limits (p > 0.05). Men with UDT had more significant hypogonadal symptoms (p < 0.001)\u0000Conclusion: Infertility, FSH, and LH levels were highest in men with bilateral UDT, but testosterone and estradiol levels were lowest. It also has the worst sperm quality and the most hypogonadal symptoms. Anovulation, decreased Oocyte quality and low endometrial receptivity are the cause of infertility in women with PCOS. Anovulation is the consequence of hyperandrogenism, insulin resistance. Furthermore, the reactive oxygen species (ROS) induce oxidative stress which may be responsible for poor Oocyte quality. Melatonin is a documented powerful free radical scavenger and broad-spectrum antioxidant. Current evidence suggests that melatonin involves in ovarian physiology including follicular development, ovulation, and oocyte maturation. Present study was tried to evaluate the effects of melatonin on biochemical parameters as well as outcomes of ovulation induction by letrozole in infertile women with polycystic ovary syndrome.\u0000Method: This is a randomized controlled trial including 74 women of Polycystic Ovary Syndrome (PCOS) with infertility. Intervention group received melatonin 3 mg at bed time for 8 weeks as pretreatment. Serum luteinizing hormone (LH), testosterone, anti mullerian hormone (AMH), fasting insulin, oral glucose tolerance test (OGTT) were measured at baseline and after 8 weeks. Both intervention and controlled group were received ovulation induction for 3 cycles by Letrozole (5 mg from cycle days 2 to 6). Intervention group continued melatonin until mature follicle achieved. The primary outcomes were biochemical changes by serum luteinizing hormone (LH), testosterone, anti mullerian hormone (AMH), fasting insulin, oral glucose tolerance test (OGTT) and ovarian responses by number of mature follicles, endometrial thickness and ovulation rate. Secondary outcome was pregnancy rate.\u0000Result: Melatonin treatment for 8 weeks significantly decreased testosterone (P <0.01) serum luteinizing hormone (0.05). There was significant difference in number of mature follicles (< 0.01), mean endometrial thickness (P<0.01). The risk ratio (RR) of ovulation rate was 1.34(0.09-1.68) and pregnancy rate was 2.55 (.37-3.51). The risk ratio (RR) of pregnancy rate in relation to AMH level was 1.12(0.05-1.79) in ≤8ng/ ml group and 8.65(0.25-9.59) in ≥8ng/ml group which was significant.\u0000Conclusion: After 8 weeks pretreatment and 3 cycle’s co treatment with ovulation induction by letrozole, melatonin seems to provide improved biochemical and ovarian response. Over the past few years, Rothia sp. are frequently identified in a tertiary hospital of northern Bangladesh. Biochemical properties and antibiotic susceptibility pattern of the isolates were determined.\u0000Methods: A total 22 isolates of R. mucilaginosa were studied to observe the fermentation status of carbohydrate and protein substrates for identification by using BD Phoenix M50 system. Antibiotic susceptibility was tested by disk diffusion method.\u0000Results: All isolates of R. mucilaginosa showed variable reaction to almost 50% of the carbohydrate and around 50% of protein- amino acid substrates used. Amikacin, meropenem, imipenem and moxifloxacin showed maximum sensitivity in vitro.\u0000Conclusion: Biochemical characteristics of R. mucilaginosa can reveal the vital information necessary for accurately identifying the organism within a sample. This depends on production of various enzymes by the organism. Meanwhile, while wanting to circumcise their children the pandemic situation reduces the economic capacity of the people. Therefore, it is necessary to conduct community service activities in the form of free circumcision performed en masse to help the poor. However, due to the coronavirus pandemic which has spread rapidly since the beginning of 2020, it is necessary to adhere to health protocols. Mass circumcision during a pandemic requires a health protocol strategy to ensure that activities are performed in a safe, comfortable, and satisfying state. East Jakarta has 2,937,859 people, with 486,110 people being children of age 0-19 years. The majority of residents in this city, about 96%, are Muslims. Meanwhile 3% percent of them, or 91,610 people, are considered poor. There is still a high need for the people of East Jakarta to conduct circumcision for their children during the pandemic, even though there is a decrease in their income. \u0000Methods: A free, safe, and comfortable mass circumcision is required with the implementation of health protocols. Out of 170 participants registered for the circumcision, 163 of them passed the Covid-19 screening, while two did not fulfill the indications for circumcision; hence only 161 children were circumcised using the smart clamp. Furthermore, the youngest, oldest, and most circumcised participants were 1, 15, and 5 – 9 years old (60 %), respectively. \u0000Results: The commonly used smart clamps were 13 in numbers, and the average circumcision time for each participant was 6 minutes. Furthermore, there were mild complications among 7 participants (4%). The participants’ parents were satisfied and advised their relatives to partake in mass circumcision in the future. \u0000Conclusion: Meanwhile, there were no reports of participants, parents, health workers, and committee members being infected with the coronavirus after completing the activity. This study aimed to investigate the ameliorative effect of silk fibroin (SF), which has anti-inflammatory and antioxidant properties, against 5 FU-induced gastrointestinal damage. \u0000Materials and methods: Wistar albino rats were divided into 3 groups; Control group, 5- FU group (30mg/kg/day on days 0, 2, 4), and 5-FU+SF (30 mg/kg/day 5-FU on days 0, 2, 4 + 600 mg/kg/day SF for 14 days). At the end of the experiment, stomach and intestinal tissue samples were collected and immunohistochemically iNOS, caspase-3, CD4+ and CD8+ were measured. Besides, structural damage was assessed with hematoxylin-eosin and caspase-3 and 9 activities were evaluated using western blot analysis. \u0000Results and Discussion: It was determined that iNOS, caspase-3, caspase-9 activities, CD4+ and CD8+ levels and structural damage significantly increased in both stomach and intestinal tissues. Furthermore, elevated markers were regressed almost to control levels in SF administrated group. \u0000Conclusion: SF may have an ameliorative effect, thus reducing the gastrointestinal damage caused by 5 FU. To our knowledge this is the first report of a patient with small patella syndrome who complained of bilateral knee pain with no preceding injury. Magnetic resonance imaging (MRI) revealed early degenerative changes of the knee joints which had not been mentioned in any literature. Furthermore, TB is one of the major public health problems in Pakistan as every year 48,000 Pakistani dies due to this infection. Pakistan ranks fifth among high burden countries worldwide. As the TB has become most threatening because of the epidemics of human immune deficiency virus (HIV), Covid-19 and the emergence of multi-drug resistance (MDR) strains of Mycobacterium tuberculosis (M. tb). This study was aimed to understand the genetic mechanism of drug resistance in local TB isolates.\u0000Methodology: For the genetic studies of INH resistance, KatG (encoding catalase peroxidase) hotspot region was amplified through PCR followed by RFLP and sequencing.\u0000Results: The study of PCR-RFLP showed that forty-five out eighty INH resistant M. tb strains had mutations in KatG (codon 315) which is 56.2% of all cases. Sequencing results revealed that this is substitution mutation; AGC to ACC (Ser315Thr).\u0000Conclusion: It may be concluded that majority of INH resistance is due to the mutation in the codon 315 of KatG in local isolates.
