Ortiz-Pérez","doi":"10.15568/am.2022.815.or03","DOIUrl":"https://doi.org/10.15568/am.2022.815.or03","url":null,"abstract":"Background and objectives: To analyze the effects of COVID-19 in the pattern of use (number of admissions, hospitalizations, hospitalization ratio, and classification by priorities) of Emergency Departments (EDs) of one tertiary referral hospital. Methods: Retrospective, observational and single-center study, carried on in the EDs of Virgen de las Nieves Hospital Complex (Granada, Spain). The information was collected from the three hospitals which belong to the Virgen de las Nieves Hospital complex (i.e. the General Hospital, GH; the Children’s Hospital, CH; and the Neuro-Trauma Hospital, NTH) from February 1st to July 31st, 2019, and 2020. Results: In the period under analysis, there were 71236 admissions compared with 118382 in the same period in 2019, which means an overall reduction of 38.83%. Considering the hospitalizations, there were 6841 compared with 7648 (2020 and 2019, respectively) which gives an overall reduction of 10.55%. The distribution of the priorities did not show a defined pattern; while CH and NTH’s EDs showed clear changes in their distribution, no notable changes were observed in GH’s ED. Conclusion: The Covid-19 Pandemic changed dramatically the pattern of ED referral in our tertiary referral hospital. A noticeable reduction of admissions and hospitalizations were reported, together with a pronounced increase in the hospitalization ratio that in some cases reached almost 50%. This could be explaining for many reasons, such as the fear of contagion and the lockdown that presumably caused a decrease in the less-severe cases attended. Possibly, the effects of COVID-19 may have been indirectly influenced by the health care system of each country. Casas-Maldonado","doi":"10.15568/am.2022.815.rev01","DOIUrl":"https://doi.org/10.15568/am.2022.815.rev01","url":null,"abstract":"The pandemic caused by the SARS-CoV-2 virus has put all public health systems worldwide in check, due to a failure of the detection, alert and control mechanisms of the disease, aggravated by the lack of diagnostic tests. All of this led to community transmission of the virus in most countries, forcing exceptional public health measures to be taken, such as the forced confinement of most of the population to cut the chains of transmission of SARS-CoV-2, unleashing a unprecedented economic and health crisis. Although the diagnosis is microbiological, imaging techniques play an important role in supporting the diagnosis, grading the severity of the disease, guiding treatment, detecting possible complications, and assessing the therapeutic response. Pulmonary function tests are important to evaluate lung sequelae in patients who have suffered from COVID-19. If pulmonary tuberculosis made possible the appearance and development of pneumology based on physiology, the new COVID-19 pandemic has generated radical changes in the way of approaching respiratory pathology, making it necessary to incorporate new tools such as the telemedicine, simplifying the healthcare process and optimizing healthcare resources. COVID-19 has given us a bath of reality and has changed our lives, as doctors and as a society. Respiratory support measures, high-flow systems and devices that combine different modalities of oxygen and Positive End-Expiratory Pressure have been key to avoiding the death of a large number of patients, avoiding many admissions to the Intensive Care Units and helping to decongest these units in a way much faster. We have assessed it by calculating the post surgical air-bone gap from pure tone audiometries which were realised one year after the surgery. In addition, we have also estimated if there are differences between stapedectomy and stapedotomy procedures as well as possible complications such as new hearing loss at high frequencies or percentage of hearing aid needed after surgery.\u0000Methods: A retrospective cohort study was carried out with a total of 45 ears. All of them were diagnosed with otosclerosis and treated at the ENT department of Hospital Universitario de Jaén from 2015 to 2019. In our study, we have considered two cohorts depending on the procedure used, either stapedotomy or stapedectomy. Afterwards, a pure tone audiometry was performed one year after the surgery and the post- surgical air-bone gap was calculated, estimating the pure tone average from both air and bone postsurgical conduction.\u0000Results: Surgery success (air-bone gap <20dB) was achieved in 90.3% of the cases using stapedectomy technique while a 85.7% with stapedotomy technique. Altogether, considering both groups, the success of the surgery was 88.9%. In the medical students´ population, it constitutes the second most common cause of death followed by accidents and is due to great loads of stress, depressión, etc. Objective: To determine the prevalence of suicidal behavior in medical students between 18 and 24 years of the Universidad Popular Autónoma del Estado de Puebla linked to risk factors. Material and Methodology: An observational, descriptive, cross-sectional study with a quantitative approach was conducted in a non-probabilistic sample obtained by a quota of medical students. The questionnaire was self-administered, answered voluntarily and confidentially via email by the students. It included the following psychometric instruments: family Apgar, CAGE scale, Beck questionnaire (BDI-II). and Plutchik. Descriptive statistics, Pearson’s correlation, and t-test (p <0,05) were applied. The instruments were interpreted according to the scores of the questionnaires, in addition to the demographic variables. Results: From a total of 350 students, 72,3% (n = 253) were women and 27,7% (n = 97) men. The mean age was 20,137 years (SD 1,60355) and the suicide prevalence was 22,9%. The students´ t-test showed that suicide risk, and depression have a higher incidence in women. Pearson’s correlation (p<0,05) between Plutchik-BDI-II scores was, r=0,802; between Plutchik- Apgar, r=-0,558 and finally between Beck (BDI-II)-Apgar, r=0,573. According to the Plutchik questionnaire, 24,57% of the sample have thought about committing suicide and 5,14% have tried it. To determine and compare choroidal thickness (CT) in keratoconus (KC) patients using Enhanced Depth Imaging (EDI) OCT.\u0000Methods. A comparative cross-sectional study including 26 patients with KC and 26 healthy controls assessed with Pentacam and EDI-OCT. CT was measured manually with the Caliper function at thirteen locations at 500 µm regular intervals.\u0000Results. The average CT in T6 was 253.96 ± 88.95 μm in the KC group, and of 309.39 ± 94.11 μm in the control group (p <0.041). No significant differences in mean CT were observed in the rest of the points, including the subfoveal CT (M), 351.48 ± 106.3 vs 365.35 ± 114.6 μm, (p = 0.66). No correlation was observed between the mean subfoveal CT (M), the mean CT in the KC group, and the values of K1 (p = 0.977 and p = 0.498 respectively), K2 (p = 0.450 and p = 0.656), corneal asphericity (Q ) (p = 0.986 and p = 0.902), minimal pachymetry (Pachy) (p = 0.408 and p = 0.688), keratoconus index (KI) (p = 0.601 and p = 0.217), vertical asymmetry index (VAT) (p = 0.296 and p = 0.523), staging of KC (TKC) (p = 0.549 and p = 0.08) and corneal apex morphology.\u0000Conclusion. Significant lower CT was found in the KC group in T6 (3000 μm temporal to the fovea).
