Today, whole genome sequencing in infectious diseases has many applications in virology, bacteriology, antibiotic resistance, epidemiology, and public health. With the appearance of SARS-CoV-2, the importance of the analysis and study of genetic sequences has been underlined. Since the initial identification of SARS-CoV-2, to date, more than 414,575 complete genomic sequences have been shared worldwide through public access databases. It is characterized by optic nerve hypoplasia, midline brain malformation, and hypothalamic-pituitary axis hypoplasia. The spectrum of clinical manifestations is very wide, from ophthalmological problems to endocrinological disorders that determine the severity and prognosis of these patients. The diagnosis is fundamentally clinical, based on an anamnesis and systematic clinical examination, supported by complementary tests for the study of hormonal deficits and imaging tests that objectify structural malformations. SOD has no cure; however, close follow-up focused on improving comorbidities through hormone replacement, neuropsychological support, and visual correction is necessary to improve the patient’s quality of life. Its low incidence and the breadth of forms of presentation require the knowledge and multidisciplinary approach of specialists in Pediatrics, Neurology, Endocrinology and Ophthalmology, among others. Alfaro Juárez","doi":"10.15568/am.2022.816.cc02","DOIUrl":"https://doi.org/10.15568/am.2022.816.cc02","url":null,"abstract":"Retinitis Pigmentosa (RP) and Idiopathic Intracranial Hypertension (IIH) are two different pathologies that can have a very similar presentation (headache and loss of visión). The differential diagnosis of both is essential due to their different management and prognosis. We present the case of a young woman with a 2-year history of headache, visual loss and buzzing in the head. In the fundus of both eyes, typical findings of RP (bone spicules in the middle retina and arteriolar thinning) and IIH (bilateral papilledema) stand out, although the latter is also described in cases of RP. After a complete study in Ophthalmology and Neurology, the diagnosis of both pathologies is reached, with symptomatic and visual improvement after starting treatment to reduce intracranial pressure. Callejas Rubio","doi":"10.15568/am.2022.815.or04","DOIUrl":"https://doi.org/10.15568/am.2022.815.or04","url":null,"abstract":"Objective: To evaluate the clinical evolution of outpatients with SARS Cov-2 infection, who received dexamethasone in our Health Center. Methods: The medical records of the patients who were prescribed dexamethasone treatment from Primary Care, in the acute phase of the SARS-CoV-2 infection, were reviewed, without initial criteria of severity. We describe the clinical-demographic characteristics and the radiological and analytical data of the cytokine storm, as well as its final evolution. Results: a total of 9 patients were included, 6 women and 3 men with a mean age of 56.6 (34-74) years. The reason for consultation was fever in all cases, with a mean time of 4 (2-7) days from onset of symptoms to consultation. All patients presented a relationship between oxygen saturation and inspired oxygen fraction greater than 450. Out of these, 5 patients presented radiological findings compatible with COVID and no infiltrates were observed in 4. No patient had laboratory data for cytokine storm syndrome. The recommended dexamethasone dose was 6 mg/d for 7 days. In 3 cases, hospital admission was required, 2 on the third day and one on the fourth day after the start of dexamethasone. These patients resolved the in-hospital process with pulses of corticosteroids and antibiotics, without the need for high-flow oxygen therapy or mechanical ventilation. Conclusions: the use of dexamethasone in patients with SARS-CoV-2 infection without hospital admission criteria does not seem to prevent the risk of progression to more serious forms. Moreover, the different models of differentiation suggested by authors throughout the history just as cellular or physiological markers which have been used to typify defined cells will be described. Finally, it will also be described the translational potential of these cells to corneal endothelium diseases through the techniques which have been used to its implantation or transplant.\u0000Material and methods: Narrative review of publications on the differentiation of mesenchymal stem cells to human corneal endothelium or other applications of these cells as regenerative therapy in models of corneal endothelial damage.\u0000Results: A total of six studies used mesenchymal stem cells in differentiation or regeneration of corneal endothelium, of which four used cells from the umbilical cord. In four of these studies a different differentiation protocol was designed. Few days before, he had odynophagia and high fever. Leucocytosis with lymphocytosis predominance in analysis and splenic rupture with hemoperitoneum in echography and computerized tomography was shown. The treatment consisted in urgent surgery (splenectomy), which was uneventful. The treatment of these inborn errors of metabolism is still under continuous investigation since when they exist, they present limitations trying to increase the synthesis of the enzyme or decrease the deposit of substrate, without reaching the definitive solution, and therefore a cure. The progressive and limiting neurodegeneration that most of them produce from pediatric ages, is one of the greatest challenges. Hematopoietic cell transplantation and enzyme replacement therapy are accepted treatments for some of these diseases; however,its results on the control and progression of neurological symptoms often remain disappointing. Undoubtedly, crossing the blood-brain barrier is one of the obstacles that many clinical trials currently underway aim to overcome. Gene therapy and neurophysiological pathways triggered by the lack of partial or total degradation of molecules also constitute a new avenue of study whose main objective is also based on preventing progression in the deterioration of cognitive function. The emergence of carbapenemases, which are a type of enzimes that hydrolize carbapenems, is the resistance mecanism more frequently involved. The aim of this literature review is to analyse the epidemiology arround the world and ,overall, in Europe; as well as the microbiological aspects of the carbapenem restistant bateria: carbapenemases can be classified into different types and are detected by several laboratory methods as well as its detection methods in laboratory.
