{"title":"[Clinical use of magnetic resonance tomography for assessing adrenal function in patients with arterial hypertension].","authors":"T S Pustovitova","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>99 patients with arterial hypertension (AH) were studied: 17 patients with pheochromocytoma, 37 with aldosteronoma, 4 with corticosteroma, 38 with essential hypertension. Control group included 16 healthy subjects. The investigation was performed by means of a NMR-tomograph BMT-1100 (\"Brucker\", FRG) in axial and frontal planes according to Spin Echo (SE) technique with TR = 2.0 s, TE = 34 ms. Qualitative (visual) as well as quantitative criteria for evaluation of adrenal glands state were developed. Normal size of adrenal glands, relative intensity of their signal and T2 were calculated. The data obtained proved that relative intensity of the signal is a highly informative parameter for differential diagnosis of adrenal gland tumors (its value of less than 1.0 is evidence of cortex lesion, and that of more than 1.4--of medulla lesion). Myocardial biopsy findings, immunologic data and autopsy findings are compared. The data obtained indicate that myocardial lymphocyte infiltration correlates with the rise in titres of antibodies to cytomegalovirus and the virus of herpes simplex as well as increase in serum alpha-interferon, neopterin and beta-2-microglobulin concentrations. Rise in the level of antibodies to cardiolipins is combined with thromboembolic syndrome. {"title":"[Effect of the calcium antagonists foridon and nifedipine on systemic hemodynamics and myocardial blood flow in dogs with acute myocardial ischemia].","authors":"E V Glukhovtsev","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Systemic haemodynamic and myocardial blood flow effects of ryodipine and nifedipine have been studied during acute transitory coronary occlusion in the open-chest anesthetized dogs. Acute ischemia has been done by occlusion of left anterior descending coronary artery for 5 min. Cardiac output and blood flows in the ischemic, lateral border and intact areas of the myocardium were defined with 15 mcm plastic radiolabelled microspheres. Drugs were administered intravenously in equihypotensive doses. Both drugs prevented fall of cardiac index during occlusion, decreased arterial pressure and total peripheral resistance but heart rate remained unchanged. In all the patients 3-6 sessions of plasmapheresis (PA) with plasma exchange up to 30 ml/kg body weight per 1 session were performed. After the course of plasmapheresis BP depression on the average by 24% as well as restoration of sensitivity to antihypertensive drugs and elimination of signs of malignant AH in certain cases were observed. PA was not enough effective in patients with AH combined with signs of chronic renal failure. Some patients had been subjected to the \"nitroprusside test\" prior to medication. The relation between the clinical state, the perilimbal and eye fundus microcirculation and degree of BP decrease were studied using fluorescent angiography. An adequate (for this group of patients) effect was observed when blood pressure decreased by 25-30% as compared to the initial level. For this capoten, up to 150 mg per day, is sufficient. However, when the BP during the \"nitroprusside test\" and capoten \"overdosage\" fell by 35-40% or more as compared to the basal level, undesirable changes were observed. {"title":"[Blood rheologic properties in patients with dilated cardiomyopathy].","authors":"V G Naumov, R A Grigor'iants, A M Al-Shaer, N N Firsov, B G Khodzhakuliev","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Rheologic blood parameters--fluidity limit tau 0, viscosity n alpha 1, erythrocyte cohesion coefficient (A), hematocrit--were studied in 42 patients with dilated cardiomyopathy (DCM) and 30 patients with ischemic heart disease (IHD) and heart failure of stages I-III. Rheologic parameters tended to increase along with progression of heart failure while in IHD an opposite tendency was observed. Individual analysis in patients with DCM as well as those with IHD demonstrated, irrespectively of the stage of heart failure, both hyper- and hyporheological syndromes. Occurrence of thromboembolic complications was in both cases nearly the same (42.9% and 40%, correspondingly). Nifedipine blocked receptor-dependent increase of Ca2+ in thrombocytes in vitro as well as by oral administration, which was accompanied by decrease in vascular tone and BP. The degree of BP decrease correlated with that of depression of receptor-dependent increase of Ca2+ in thrombocytes. Combined therapy including nifedipine, propranolol and a diuretic resulted in more manifest inhibition of receptor-dependent calcium channels than monotherapy with nifedipine.
