In 10-14% of patients it may have arrhythmogenic effect. Positive correlation between duration and degree of the antiarrhythmic effect and dose and plasma ethmozin concentration was found. The character of the effect (antiarrhythmic or arrhythmogenic) did not depend on the dose and concentration of the drug. Three examples--atrial septal defect (ASD), formation in the right atrium, thrombus in the left atrium are discussed, when the TEE was useful in the establishment of correct diagnosis. The possibilities provided by TEE in cardiac monitoring are described. Intravenous infusion of allapinine (0.43 mg/kg) arrested the episodes of SVT in none of 7 patients. Oral administration of the drug for short-term treatment (150 mg/day) prevented the artificial induction or spontaneous occurrence of paroxysms in 10 of 16 patients. The authors proved relationship between GcI-I-genotype and predisposition to EH. Heterozygote type 1-2 of the Gc-system was more resistant to the disease. Patients with EH demonstrated significantly greater decrease of systolic as well as diastolic BP at night compared to those with RH. Circadian variation rates of systolic and mean BP were also higher in patients with EH. Baroreceptor test exhibited higher cardiochronotropic effect in patients with EH. They had also negative correlation between BP variation rates and BP reaction to baroreceptor test which is absent in patients with RH. {"title":"[Intra-abdominal transplantation of the heart in rats].","authors":"R S Akchurin, V Iu Khalatov, A A Shiriaev","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>3 variants of microsurgical intraabdominal heart transplantation technique in rats are described. In the first variant heart transplantation was performed with anastomosis of the transplant aorta and pulmonary artery with the abdominal aorta and v. cava caudalis of the recipient. In the second variant heart was transplanted in complex with the lungs, performing anastomosis of the transplant aorta with the abdominal aorta of the recipient. The lungs acted as a shunt for venous coronary blood flow from the transplanted right heart into the left heart and further into the aorta of the recipient. The third variant included transplantation of the heart with a right lung lobe. A total of 68 intraabdominal heart transplantations was performed with 57 (84%) successful cases. The diagnosis was based on clinical symptoms of chronic alcoholism, roentgenocontrast coronaroangiography and ventriculography (in some cases with endomyocardial biopsy), echocardiography, bicycle exercise test and on exclusion of other cardiovascular diseases. In all the patients intracardiac electrophysiological study was performed. Younger persons systematically abusing alcohol and having no pronounced clinical picture of cardiovascular damage exhibited symptoms of functional conduction disturbances located mainly in ventricular conduction pathways and AV-node and more rarely in the sinus node and His-Purkinje's system.
