{"title":"[Examination of the aorta and major arteries in arterial hypertension using MR tomography].","authors":"I D Fedina","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>184 patients with arterial hypertension (AH) were studied. 63 of them had essential hypertension, 49 had renovascular hypertension, 27 had Leriche's syndrome, 38 had aortal aneurysms and 7 had coarctation of aorta. MR-tomography (MRT) gave satisfactory enough findings in diagnosis of vascular pathology in patients with essential hypertension as well as in those with symptomatic hypertensions of vascular origin. Criteria for assessment of vessels state and symptoms of various types of vascular pathology, such as stenosis and aorta aneurysm, were developed. In essential hypertension MRT may show early aortic alterations and dependence of their degree on duration and severity of the disease. All patients underwent coronarography and catheterization of the coronary sinus. Blood samples were obtained from the left ventricle and coronary sinus in 3 points: at rest, peak pacing, 10 min after pacing. All patients with stable angina had positive atrial pacing test (pain and/or ECG deviations and lactate production), in control group similar changes were not observed. However, no statistically significant difference was found in platelet factor (IV) 4 levels between the groups and following pacing; the level of platelet factor IV was near 90 ng/ml. In 9 cases the dose was 50 mg. Maximal total reduction of ventricular ectopic complexes (VEC) per hour was 67 +/- 28% (mean value); in 5 cases (55%) it exceeded 90% of basal level. At peak of action, PQ interval increased by 15%, QRS--by 14%, QT was not significantly changed. In 14 cases the dose was 75 micrograms. Maximal VEC number reduction per hour was on the average 89 +/- 18% and in 12 cases (88%) exceeded 90%. PQ increased by 22%, QRS--by 23% and QT by 9% though JT was not changed (average peak values). It is thus concluded that single oral dose of allapinin has pronounced antiarrhythmic effect in patients with VEC and influences ECG intervals in the way characteristic for class IC antiarrhythmic drugs. For this purpose of 185 questions selected by expert method on the data of 236 cases of MI using the program of stepped discriminant analysis, 37 most informative questions were chosen. These questions allowed determination of the general response to MI (adequate or neurotic) as well as of the degree of overestimation or negation of the disease and the degree of anxious-depressive syndrome. On the basis of the obtained data a method of psychologic express diagnosis (PED) for assessment of the named variables was developed. The efficacy of the method could be highly increased (from 37% to 81%) if the investigation was performed in patients with murmurs above the heart area, with cardiovascular diseases in anamnesis and with marked ECG alterations. Dynamic exercise increased heart rate more and blood pressure less than isometric exercise; plasma norepinephrine (NE) level rose by 109%. Isometric exercise increased NE level only by 18%, but maximal changes (46%) occurred during recovery, probably reflecting a washout of NE from active muscles after the end of isometric contractions. The SHR have been shown to response to the intravenous sodium chloride load with exaggerated natriuresis as compared with the WKY group. The increase in natriuresis in the SHR did not depend on diuresis changes but was due to the increased urinary sodium concentration. Marked heart failure in patients with acute myocardial infarction compared to patients without heart failure results in reduced elimination rate, decreased clearance, significantly increased plasma allapinin concentration which should be taken in account when choosing the regime of drug infusion.
