It is found that all types of collagen and fibronectin participate in formation of diffuse cardiosclerosis while small cardiosclerosis is formed by interstitial types of collagen and to a lower extent of fibronectin. {"title":"[Quetelet's index in the male population aged 20 to 59 (epidemiological autopsy research in Riga)].","authors":"V A Volkov, A M Vikhert","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The increased body mass (Quetelet index greater than or equal to 30.0) in 1447 autopsies of Riga's male population aged 20-59 years was revealed in 8.6% of all cases. A significant rise of mean values of the Quetelet index was found in the presence of arterial hypertension, hypercholesterolemia, increased thickness of subcutaneous fat and diabetes mellitus. {"title":"[Effect of hemodynamic changes on cardiac rhythm disorders: experience with long-term intravenous administration of nitroglycerin in patients with chronic heart failure].","authors":"D V Kazeĭ, V Iu Mareev, N M Mukharliamov","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In 15 patients with chronic congestive heart failure (CHF), heart rhythm was analysed in normal haemodynamics achieved by 24-hour nitroglycerin (NG) infusion. The NG dose was titrated until the pulmonary wedge pressure (PWP) was reduced up to less than 15 mm Hg; during further 24 hours the infusion rate was unchanged (mean 144 +/- 16 mgk/min). The control group included 12 patients with CHF to whom no NG infusions were performed. There was no significant difference between the groups in mean daily number of ventricular extrasystoles (VE), coupled VE, runs of ventricular tachycardia and maximal gradation of ventricular rhythm disturbances (VRD). VRD in the first 12 hours, i. e. during greatest haemodynamics alterations, and in the next 12 hours of the infusion were compared separately as well as VRD in every 3 hours of the infusion. The groups did not differ significantly in number and degree of VRD. The validity of data obtained with US B-scanning and Doppler technique was estimated by comparison with direct artery diameter and US blood flow measurements made intraoperatively during reconstructive renal artery surgery in 25 patients. It was shown that US Doppler spectral characteristics obtained noninvasively before surgery and intraoperatively were identical. Posterior-lateral approach is optimal for Doppler examination. Differences between Doppler spectrum in the presence and absence of renal artery stenosis (RAS) were revealed. {"title":"[Disorders of cardiac rhythm in patients with ischemic heart disease and spontaneous stenocardia].","authors":"A B Guliev, A A Liakishev","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In 22 patients with spontaneous angina pectoris, diagnostic value of 24-hour ECG monitoring for cardiac rhythm disturbances registration was compared to that of cold test (CT) and hyperventilation test (HVT). Ventricular extrasystoles or paroxysmal tachycardia during anginal attack (either spontaneous or induced) was registered in 40.9% of patients. 24-hour ECG monitoring proved to be the most valuable method for determination of the anginal attack character and connected with it's rhythm disturbances. Combination with CT and HVT increases occurrence of angiospastic angina pectoris and cardiac rhythm disturbances determination during myocardial ischemia. HS included in the therapy complex was the only way to correct secondary hyperaldosteronism, to reduce antidiuretic hormone blood level, to increase diuresis and natriuresis and to reduce kaliuresis as well as to normalize blood electrolyte level. {"title":"[The advantageous effects of a prolonged intravenous load with a nitroglycerin solution on the lesser circulation in patients with chronic heart failure].","authors":"D V Kazeĭ, V Iu Mareev","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>To evaluate the sequence of hemodynamic changes and the potential loss of efficacy of i.v. 24-hour nitroglycerin (NTG) infusion at constant rate we studied 15 NYHA III Class patients. NTG infusion, titrated to reduce pulmonary artery wedge pressure (PWP) less than 15 mm Hg, was continued for 24 hours at mean rate of 144 +/- 16 (50-250) micrograms/min. Hemodynamic data were monitored every 3 hours. Acute changes were pronounced and significant in mean pulmonary artery pressure (mPAP), peak V-wave of indirect left atrial pressure, right atrial pressure (RAP), mean blood pressure (mBP), and heart rate (HR). Significant attenuation of these effects was registered by the 9th hour. RAP rose already by the 6th hour. From the 9th to the 24th hour PWP, mPAP, peak V-wave remained significantly decreased, but not mBP and RAP. HR rose during hours 9-12 in comparison to hours 0-3 and to the 21st hour.
