Different implants were used in order to observe the load distribution over teeth, implants, and adjacent bone using three-dimensional finite element analysis. A simulation of tooth loss of the first and second right molars was created with an implant placed in the second right molar and a prepared tooth with simulated periodontal ligament (PDL) in the second right premolar. Configurations of two types of implants and their respective abutments, i.e., external hexagon (EX) and Morse taper (MT), were transformed into a 3D format. Metal-ceramic fixed partial dentures were constructed with rigid and non-rigid connections. Mesh generation and data processing were performed on the 3D FEA results. Static loading of 50 N (premolar) and 100 N (implant) were applied. When an EX implant was used, with a rigid or non-rigid connection, there was intrusion of the tooth in the distal direction with flexion of the periodontal ligament. Tooth intrusion did not occur when the MT implant was used independent of a rigid or non-rigid connection. The rigid or non-rigid connection resulted in a higher incidence of compressive forces at the cortical bone and stress in the abutment/pontic area, regardless of whether EX or MT implants were used. MT implants have a superior biomechanical performance in tooth-implant supported fixed partial dentures. This prevents the intrusion of the tooth independent of the connection. Both types of implants that were studied caused a greater tendency of compressive forces at the crestal area.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"171 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"85010985","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This complication is due to several mechanical factors, including: type of connection, abutment-screw geometry, settling effects, and cyclical load. The purpose of the present in-vitro study was to compare and associate different times of re-tightening with reductions in preload losses. We evaluated 40 internal hexagon dental implants and 40 external hexagon dental implants, with their related abutment screws. The implants were embedded in acrylic resin in cylindrical polyvinyl chloride tubes (diameter, 26 mm; height, 20 mm). The abutments were fixed to the implants with screws to an initial torque of 35 Ncm using a digital torque meter with decimal precision. The two different types of connection were randomly divided in four subgroups of 10 samples each. One subgroup was used as control. The test groups underwent re-tightening to the same initial torque at increasing times from initial torque application for tightening of the abutment screws, to their re-tightening at 2 min, 5 min, and 10 min. The re-tightening time of 2 min shows significantly reduced preload loss. Randomized clinical trials are strongly required to provide clinicians with a beneficial standardized protocol of re-tightening that can be applied in routine clinical practice.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80327705","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A diagnostic cast demonstrated that the interocclusal distance was insufficient. A five-unit screw-retained implant-supported fixed partial denture (FPD) was used to restore missing maxillary teeth. The restoration of multiple missing teeth using an implant-supported FPD is challenging when the interocclusal distance is limited due to lack of retention and inadequate esthetics. In this case, a hexagonal, screw-retained, and sublingually located titanium-based zirconia implant-supported FPD with a conical abutment base was used for restoration to overcome the limited interocclusal distance. This implant-supported FPD, consisting of CAD/CAM-designed monolithic zirconia cemented to a titanium bonding base in the laboratory, is expected to facilitate predictable retention and adequate esthetics as well as provide ease of retrieval.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"37 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82852765","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Ten implants of each of the 3i (double hexagon + flat to flat; group 1), IDI (Internal hexagon + Morse taper; group 2), Swiss Plus (external bevel + internal octagon; group 3) and Tapered Screw-Vent (internal bevel + internal hexagon; group 4) systems were used in this in vitro, experimental study. Healing abutments were screwed to the implants with 10N/cm torque. Implants were immersed in Escherichia coli suspension for 24 hours. Samples were taken of the internal surface of implants and cultured. Number of grown colonies was counted after 24 hours of culture and after seven and 14 days of immersion in microbial suspension. The same was repeated with healing abutments torqued to 10 and 20 Ncm. With 10 Ncm torque, all specimens in all groups showed microleakage at one day with the highest microleakage in one sample in group 3. At seven days, the highest microleakage was noted in one specimen in group 2. With 20 Ncm torque, group 3 showed significantly higher microleakage than other groups at one and seven days (P<0.05). Increasing the torque decreased microleakage in all groups except for group 3. Microbial leakage occurred in almost all implant systems in our study. In one-stage implant placement, healing abutments should be preferably torqued to 20N/cm to minimize microleakage. Optimal tourque for healing abutment insertion should be analyzed individualy for each system.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"20 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"90911495","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The preparation of the socket shield, however, is technique-sensitive due to limited access to the extraction sockets and variations in root anatomies. The dynamic navigation system allows clinicians to visualize the movement of the cutting instrument and its position relative to anatomical structures in real-time. This greatly enhances clinicians' abilities to perform surgeries accurately. This report describes a novel method of using such system to prepare the socket shield in a precise and efficient manner.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"46 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87797831","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Twenty patients (mean age =59.32±4.1 years) with moderately controlled diabetes mellitus (HbA1c 8.1-10.0 %) were rehabilitated by maxillary and mandibular conventional dentures. Two small- diameter implants (3×12mm) were inserted in the canine areas of the mandible and immediately loaded by mandibular dentures. In a split mouth design, LLLI was applied to one of the two implants in a random order (study group, SG) and the other implant was left as a control (control group, CG). For each patient, Gallium aluminum-arsenide (GaAlAs) diode low-level laser (940 nm wavelength, 0.50±2 mW output power, .004 cm2 spot size, Epic Tm, Biolase®, Inc, San Clemente, California, USA) was applied around each implant with total delivered energy of 90 J (equally divided by 6 irradiation points) in 3 sessions. The application was made immediately after implant insertion, 3 days and one week after the surgery. The implant stability (PTVs, measured by Periotest®) and marginal bone loss (MBL, measured by cone beam CT) were evaluated at implant loading (T1), 6 months (T6) and 12 months (T12) thereafter. One implant failed in CG and no failures occurred in SG resulting in 95% and 100% survival rates, respectively. SG recorded higher PTVs at all observation times than CG. However, the difference was significant (p=.039) at T6 only. SG recorded reduced MBL values than CG. No difference in MBL was detected between groups or peri-implant sites (mesial, distal, buccal, and lingual) at T6 and T12. Within the limits of this study, LLLI has no effect on marginal bone around immediately loaded small diameter implants retaining overdentures in moderately controlled diabetic patients. However, it was beneficial in improving implant stability 6 months after overdenture insertion.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"112 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-08-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80660601","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Eighty implant sites were prepared in custom-made solid rigid polyurethane blocks with two different low cancellous bone densities (5 or 15 pounds per cubic foot [PCF]), equally distributed between piezoelectric (Surgysonic Moto, Esacrom, Italy) and conventional drilling techniques. Two short implant systems (Prama and Syra, Sweden & Martina) were tested by inserting 40 fixtures of each system (both 6.0 mm length and 5.0 mm diameter), divided in the four subgroups (drills/5 PCF density; drills/15 PCF density; piezo/5 PCF density; piezo/15 PCF density). Insertion torque (Ncm), implant stability quotient values, removal torque (Ncm), and surgical time were recorded. Data were analyzed by 3-way ANOVA and Scheffé's test (α = 0.05). With slight variations among the considered dependent variables, overall high primary implant stability was observed across all subgroups. Piezoelectric instrumentation allowed for comparable or slightly superior primary stability in comparison with the drilling procedures in both implant systems. The Prama implants group showed the highest mean reverse torque and Syra implants the highest implant stability quotient values. The major drawback on implant-based rehabilitation was bone resorption. Several studies had suggested that osseointegration might be achieved through immediate implant placement, but this method could cause multiple biological responses in the extraction socket. The guided bone regeneration (GBR) was another widely used technique to preserve the alveolar ridge. However, the bone graft placed between the implants and labial bony plate may not compensate the buccal bundle bone loss. The question remains on how to prevent the bone loss during tooth implantation. Therefore, it is eager to demand desirable solution. After tooth extraction, the remarkable alterations in the buccal bone plate were caused by periodontal ligament loss and the consecutive trauma. Intriguingly, it seemed bone resorption could be prevented by root retention. Hürzeler et al evaluated effects of retaining the buccal portion of the root in conjunction with immediate implant placement. Their results demonstrated that this technique could restrain the buccal bone resorption to a large degree. It also achieved significantly higher implant success rates compared with longitudinal data on immediate implant placement after complete root extraction. In previous reports, only 1 root fragment exhibited signs of resorption, at the same time, the implant remained functional. Notably, when implants were placed lingual to the tooth fragment, connective tissues could be observed in the gaps, which would affect implants and the coronal root fragments. On the other hand, placing implants in direct contact with tooth fragments could increase risk of displacing buccal root fragments or missing the buccal lamellar bone. Even with those concerns, it was evident that the socket-shield technique could maintain the healthy periodontium, gingival tissues, and original crestal bone height and contour. To further improve this method and reduce adverse effects, we attempted to keep the gaps and fill them with bone graft materials to minimize root resorption. This case report was targeted to evaluate the feasibility of this modified approach in a clinical practice setting.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"45 1","pages":"139-143"},"PeriodicalIF":0.0,"publicationDate":"2017-04-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"91344645","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
