{"title":"Diagnosis and treatment of idiopathic necrosis of the femoral head.","authors":"Y Z Li, T Yue","doi":"","DOIUrl":"","url":null,"abstract":"

Fifteen cases, 27 hips, of idiopathic necrosis of the femoral head (INFH) were included in our series. The roles of radiography, isotopic bone scanning, intraosseous pressure measurement and intraosseous venography in the diagnosis of INFH were evaluated. Radiological examination could not reveal early cases, while isotopic bone scanning was more sensitive. There was an increase of intraosseous pressure in most of the diseased hips, and intraosseous venography showed abnormalities in all examined hips. The latter two tests could therefore detect early INFH. Core decompression was performed in 23 hips, 21 of which were followed for 7 to 27 months. Functional evaluation was good or excellent in 18 hips. Radiological re-examination remained unchanged in 19 hips. Postoperative intraosseous pressure measurement and intraosseous venography revealed a decrease of intraosseous pressure and improvement of venous drainage. Core decompression is therefore an ideal method of treatment for early INFH.
","PeriodicalId":77596,"journal":{"name":"Proceedings of the Chinese Academy of Medical Sciences and the Peking Union Medical College = Chung-kuo i hsueh k'o hsueh yuan, Chung-kuo hsieh ho i k'o ta hsueh hsueh pao","volume":"5 2","pages":"88-92"},"PeriodicalIF":0.0,"publicationDate":"1990-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13395807","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The results were as follows: 19 out of the 31 cases belonged to the IgG class, including the following subclasses--IgG1-lambda, 7; IgG1-k, 5; IgG3-lambda, 1; IgG3-k, 2; double M myeloma, 2 (IgG3-lambda + IgG2-k, IgG1-lambda + IgG4-lambda); IgG-lambda, 1; and IgG1-k 1. Seven cases were light chain diseases, of which 4 belonged to lambda type, 2 were k, and one was a double light chain disease. Five cases belonged to the IgA class (IgA-lambda, 3; IgA-k, 2). We found no correlation between electrophoretic mobility and the antigenicity of M protein even within subclasses, and the highly concentrated BJP in urine had a tendency to form light chain polymers, possibly along different polymerization pathways.
","PeriodicalId":77596,"journal":{"name":"Proceedings of the Chinese Academy of Medical Sciences and the Peking Union Medical College = Chung-kuo i hsueh k'o hsueh yuan, Chung-kuo hsieh ho i k'o ta hsueh hsueh pao","volume":"5 2","pages":"79-83"},"PeriodicalIF":0.0,"publicationDate":"1990-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13277298","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Debulking surgery is of great value in the treatment of primary ovarian carcinoma, but for recurrent carcinomas success depends upon the pathological type. The 5-year survival rate was 75% for patients with immature teratomas, but only 6.1% for those with epithelial ovarian carcinomas and 3.6% with endodermal sinus tumors. Two cases of recurrent dysgerminoma were both alive and well more than 10 years after treatment. So repeated debulking is worthwhile for recurrent immature teratoma and dysgerminoma, while in cases of epithelial type or endodermal sinus tumors successful debulking surgery should be followed by some new kinds of adjuvant therapy, otherwise only failure will result.
","PeriodicalId":77596,"journal":{"name":"Proceedings of the Chinese Academy of Medical Sciences and the Peking Union Medical College = Chung-kuo i hsueh k'o hsueh yuan, Chung-kuo hsieh ho i k'o ta hsueh hsueh pao","volume":"5 4","pages":"219-22"},"PeriodicalIF":0.0,"publicationDate":"1990-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13443649","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The results indicate that serum cholesterol and the levels of cholesterol and cholesteryl ester in aortic tissue were significantly increased in cholesterol-fed rabbits. The percentage of alpha-lipoprotein was significantly decreased and the aortic atherosclerotic plaque area was significantly increased. The data also demonstrate that the level of plasma TXB2 was markedly increased, while that of 6-keto-PGF1 alpha was significantly decreased. The TXB2/6-keto-PGF1 alpha ratio (T/6) was significantly increased. The decrease of 6-keto-PGF1 alpha occurred prior to the increase of TXB2. Compound 8501 not only lowered serum total cholesterol and aortic total cholesterol and cholesteryl ester but also antagonized the decrease of alpha-lipoprotein and atherosclerotic plaque formation. In addition, 8501 prevented the decrease of plasma 6-keto-PGF1 alpha and the increase of TXB2, and so the T/6 ratio was significantly decreased.
","PeriodicalId":77596,"journal":{"name":"Proceedings of the Chinese Academy of Medical Sciences and the Peking Union Medical College = Chung-kuo i hsueh k'o hsueh yuan, Chung-kuo hsieh ho i k'o ta hsueh hsueh pao","volume":"5 4","pages":"228-32"},"PeriodicalIF":0.0,"publicationDate":"1990-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13443652","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

EIAV RT activity was found to be dependent on the bivalent cations Mg++ and Mn++. The optimal pH for enzyme reaction was pH 8.2. EIAV RT preferred a 70 mmol/L concentration of monovalent salts. Phosphonoformic acid (PFA) was an active inhibitor of EIAV RT, but phosphonoacetic acid (PAA) and N-ethylmaleimide (NEM) were not. The inhibition of EIAV RT activity by 5'-triphosphates of nucleoside derivatives was in the following decreasing order: FLTTP greater than AZTTP greater than nPrearaUTP greater than nPredUTP = CEdUTP greater than EtdUTP greater than nPrdUTP greater than HMdUTP. nPrearaUTP was a linear competitive and PFA a linear noncompetitive inhibitor of EIAV RT with respect to dTTP. The results showed an HPV-16 DNA positive rate in our series of about 74.4% (29/39) as determined by paraffin section in situ hybridization and about 71.8% (28/29) by dot blot hybridization. There was no significant difference between the two methods. {"title":"A proposed clinical classification of pelvic endometriosis.","authors":"A Sun, M Han","doi":"","DOIUrl":"","url":null,"abstract":"

Two hundred and forty-four cases of pelvic endometriosis were diagnosed by laparoscopy between Jan. 1979 and Dec. 1986. Consulting the principles designated by the American Fertility Society (AFS) and Revised American Fertility Society (RAFS) classification, diseases were staged as mild, moderate and severe. The results of clinical classification were compared with those of laparoscopic classification. The conformance rate between clinical and RAFS classification was 74.2%. The pregnancy rates after operation were 69.2%, 42.9% and 38.5% among 94 patients clinically classified as mild, moderate and severe, respectively, as compared with 65.2%, 41.4% and 35.7% in terms of RAFS classification. The authors point out that laparoscopy is an excellent procedure for diagnosing and staging endometriosis. In hospitals where laparoscopic equipment is not available, clinical classification can be used as a substitute for staging those highly suspected of having endometriosis.
","PeriodicalId":77596,"journal":{"name":"Proceedings of the Chinese Academy of Medical Sciences and the Peking Union Medical College = Chung-kuo i hsueh k'o hsueh yuan, Chung-kuo hsieh ho i k'o ta hsueh hsueh pao","volume":"5 3","pages":"163-7"},"PeriodicalIF":0.0,"publicationDate":"1990-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13306138","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
