{"title":"Sorafenib's inhibition of prostate cancer growth in transgenic adenocarcinoma mouse prostate mice and its differential effects on endothelial and pericyte growth during tumor angiogenesis.","authors":"Aldo V Bono,&nbsp;Tania Pannellini,&nbsp;Marcella Liberatore,&nbsp;Rodolfo Montironi,&nbsp;Sergio Cosciani Cunico,&nbsp;Liang Cheng,&nbsp;Francesco Sasso,&nbsp;Piero Musiani,&nbsp;Manuela Iezzi","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To evaluate Sorafenib's efficacy (60 mg/kg/d per os) in preventing the transformation of high grade prostate intraepithelial neoplasia (HGPIN) into adenocarcinoma (ADC) and in inhibiting the onset and progression of poorly differentiated carcinoma (PDC) in transgenic adenocarcinoma mouse prostate (TRAMP) mice.</p><p><strong>Study design: </strong>Forty-four TRAMP mice were randomly divided into 2 groups of 22 and assigned to daily treatment by gavage with vehicle only or Sorafenib from the 10th to the 26th week of age. At 26 weeks of age the mice were killed, and their genitourinary apparatus was removed and examined by histology, immunohistochemistry and confocal microscopy.</p><p><strong>Results: </strong>Sorafenib reduced HGPIN growth and progression to ADC and was probably also effective in PDC inhibition. The major effect of Sorafenib was on tumor angiogenesis. Interestingly a dissociation between endothelial cells and pericytes was noted in treated PDC since inhibition of pericyte recruitment was less complete than that of endothelial cells.</p><p><strong>Conclusion: </strong>Sorafenib's potent antiangiogenic action may be supposed to be exerted primarily by inhibiting endothelial proliferation and sprouting, whereas its inhibition of pericyte recruitment and maturation is less complete. Up to now, only 21 urachal urothelial carcinomas (UCas) have been reported. Here, we describe a case of high grade UCa arising from the urachal remnants.</p><p><strong>Case: </strong>A 66-year-old man presented with voiding difficulties. Prostate specific antigen (PSA) was 5.46 ng/mL. Prostatic adenocarcinoma (PCa) (Gleason score 6) was diagnosed by needle biopsies. After the diagnosis of high grade, muscle invasive UCa with intact mucosa on frozen examination of the dome of bladder wall during the radical prostatectomy operation (RPO), partial cystectomy was performed. Microscopically, among the tumoral islands, cystic structures lined by cells with a benign appearance, which are considered urachal remnants, were noted. Cytokeratin 7 and high-molecular-weight cytokeratin were strongly positive, PSA and carcinoembryonic antigen were negative. Radiotherapy was given for both UCa and PCa. Six months after the diagnosis, an undifferentiated tumor was detected in a bladder transurethral resection specimen; thus, chemotherapy was given. After 1 course of chemotherapy, the patient was doing well.</p><p><strong>Conclusion: </strong>The origin of urachal carcinomas is usually obscured as it is a highly invasive carcinoma. The patient presented here was diagnosed incidentally during RPO. Although the stage was advanced, the tumor was detected before urachal remnants were destroyed. The treatment choice for urachal carcinomas is cystectomy. Routine hematoxylin-eosin sections were prepared from paraffin-embedded tumor materials. A monolayer cell suspension was prepared from tumor cells that were enzymatically and mechanically isolated from biopsy material. DNA content of the nuclei was assessed on Feulgen-stained monolayer cell preparations with automated image cytometry. Ploidy assessments were done according to DNA indices.</p><p><strong>Results: </strong>Sixty cases of OT were evaluated. All cases had epithelial components. There were 30 pure ameloblastomas and 30 other OTs with epithelial components. All cases were diploid in DNA assessment.</p><p><strong>Conclusion: </strong>The current study showed that ameloblastomas and other OTs with ameloblastic epithelial components are diploid. {"title":"Fecal cytology in conjunction with immunofecal occult blood test for colorectal cancer screening.","authors":"Jin-qiu Sheng,&nbsp;Shi-rong Li,&nbsp;Hui Su,&nbsp;Jian-sheng Li,&nbsp;Zi-qin Sun,&nbsp;Zi-tao Wu,&nbsp;Xia Wu,&nbsp;Chang-hong Xia,&nbsp;Jianyu Rao","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To develop a simple method to extract and analyze the cytomorphology of epithelial cells from fecal samples and to compare the efficacy of fecal cytology with the immunofecal occult blood test (IFOBT) in colorectal cancer screening.</p><p><strong>Study design: </strong>Fecal cytology and IFOBT were performed on fecal samples obtained from 41 patients with colorectal cancer; 34 patients with a small, single adenoma (<0.5 cm); and 20 without abnormality. The samples were obtained prior to colonoscopic examination. For fecal cytology, epithelial cells were exacted through filtration, centrifugation and cytocentrifugation and stained with hematoxylin-eosin prior to morphologic analysis.</p><p><strong>Results: </strong>Fecal cytology and IFOBT test had similar levels of sensitivity for detecting colorectal cancer (75.6% vs. 68.3%, respectively), but fecal cytology had higher specificity than IFOBT (100% as compared to 85.2%, respectively, p<0.05 by chi2 test). Seven of 41 colorectal cancer patients (17.1%) with negative IFOBT were positive by fecal cytology analysis. There was a higher proportion of younger patients with a larger tumor and patients with lymph node involvement in the Chinese group as compared with Australian women. The cytopathologic diagnosis was compared with the final diagnosis as modified by subsequent FCM.</p><p><strong>Results: </strong>The cytopathologic diagnosis was benign in 61 cases (69%), atypical in 20 cases (22%) and malignant in 8 cases (9%). In patients without any prior clinical history, FCM study was positive in 2 cases and negative in 49 cases. In these patients, the working cytopathologic diagnosis was modified from benign/atypical to malignant in 2 (11%) cases and atypical to benign in 11 (33%) cases. In patients with a prior clinical history, FCM was positive in 23 cases and negative in 15 cases.</p><p><strong>Conclusion: </strong>FCM studies were helpful in the cytopathologic diagnosis in 35% of body fluid specimens, permitting appropriate cancer staging and management. A cervical biopsy diagnosis of CIN 2+ was considered as evidence of high grade disease.</p><p><strong>Results: </strong>The positive and negative predictive values of HC2 and FISH for detecting patients with CIN 2+ were 32% vs. 37% and 100% vs. 93%, respectively. The sensitivities of HC2 and FISH for CIN 2+ were not significantly different (100% vs. 90%, p = 0.25), while the specificity of HC2 was significantly lower than that of FISH (28% vs. 48%, p=0.003). A day after the administration of TNBS, L-NAME, MT1 and MT2 were given intraperitoneally for 3 days to the experimental groups. After the administration of TNBS, dissections of the rats in the L-NAME, MT1 and MT2 groups were performed at 3-day periods under ether anesthesia, and whole blood, bone marrow and colon were obtained.</p><p><strong>Results: </strong>On the third day, red and white blood cell values were increased, while platelet and bone marrow granule cells decreased in the L-NAME- and TNBS-induced group. On the third day, all the blood values increased in MT1 (1 and 2 mg/kg) and MT2 (1 and 2 mg/kg) in the TNBS-administered groups.
