Better resolution by increased pitch?].","authors":"G Rockstroh","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The longitudinal resolution of spiral CT has been investigated in dependence on table increment and beam collimation by use of a phantom. The results show clearly that the pitch (ratio of table increment per tube rotation to beam collimation) alone does not allow one to draw any conclusions about the resolution. Thus, a large pitch can yield a better resolution than the pitch of 1. The individual parameters table increment and beam collimation are more relevant with regard to the resolution than the rate of pitch. An alteration of the pitch can be achieved in two different ways. An increase of the pitch by a decreased beam collimation improves the resolution. In contrast, an increase of the pitch by an increased table increment decreases the resolution. A good spatial resolution of objects with high contrast (bone, lung, enhanced vessels) needs a narrow beam collimation, even if a pitch as high as 2 is necessary for a given scanning coverage. A large pitch does not cause gaps in the data set. In general, the methodological limitations concern only the less important detail findings such as representation of the neck of the diverticulum or the distinction between septated and multiple diverticula. In addition, possible complications and the pelvic anatomy are shown in one examination. {"title":"[CT-guided punctures: fine-needle biopsy vs. automated co-axial cutting biopsy].","authors":"H J Skamel,&nbsp;A Hanusch,&nbsp;K Mathias","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Purpose: </strong>Comparison between fine needle aspiration (FNAB) and automated coaxial core biopsy (ACCB) of CT-guided thoracal and abdominal biopsies.</p><p><strong>Method: </strong>98 patients had CT-guided biopsies of the liver (n = 26), lung (n = 36), pancreas (n = 8), presacral space (n = 16), retroperitoneum (n = 8), groin (n = 1) and mediastinum (n = 3). In 50 patients fine needle aspiration, in 48 patients ACCB was performed. Nine physicians, two of them with advanced experience, performed the procedure.</p><p><strong>Results: </strong>Histologically satisfying material was achieved in 76% of the biopsies with FNAB, in 100% with ACCB. The sensitivity within the FNAB group was 59.6%, within ACCB group 97.5%. Sensitivity for the whole group was 76.7%. In pulmonary lesions the sensitivity was increased to 100% by using the ACCB method in comparison to FNAB (58%), in liver lesions from 75% (FNAB) to 92% (ACCB). 19 of 20 false negative specimens were obtained with FNAB. The less experienced physicians had a false negative rate of 43% with FNA biopsies, but only 2.2% with ACCB.</p><p><strong>Conclusion: </strong>In pulmonary and abdominal lesions ACCB is a procedure with rare complications and low invasivity, which is not dependent on the performing person. {"title":"[Sensitivity differences of different emulsion numbers for different UV/blue-sensitive films].","authors":"C Blendl,&nbsp;K Goergens","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The German standard series DIN 6868 does not include a separate requirement for deviations in the sensitivity of different emulsion numbers or, respectively, the relative contribution of the film to sensitivity S of a screen film system (SFS) whose sensitivity is determined according to DIN 6867-1 [1]. The inclusion and interpretation of these contributions in the test body exposure according to DIN 6868-3 [2] has not proved to be of value since an unambiguous error recognition is only possible with considerable experimental experience (additional exposure of a sensitometer strip and interpretation of the resulting consequences). The investigations reported here demonstrate that the correlation of sensitometric parameters obtained from sensitometer strips generated with sensiometer exposure and X-ray exposure is so large that differences in sensitivity of different emulsion numbers of a screenfilm larger than delta log H > or = 0.015 or delta H > or = 4% can be surely detected on exact observance of the experimental limiting conditions. Thus, the statements by the manufacturers in \"ZVEI information Nr. 7 of the X-ray Regulation\" [3] with regard to the claimed scattering width of screenfilms can be tested and evaluated at least tendencially when the systemic fluctuations and measurement uncertainties, see in particular DIN 6868-55 Appendix E of Sept. 1996 (4), are correctly taken into consideration. Therefore they can easily be detected either by enteroscopy or ileocolonoscopy. Mostly the patients are asymptomatic so that it is a surprising finding without therapeutic consequences. But if a patient has an unclear enemea or malabsorption problems for which no explanation has been found in upper or lower gastrointestinal endoscopy, it is suggested to do an enteroscopy. We present the case of a 62-year-old woman who presented to our clinic with a first episode of melena. Although she needed blood transfusions no source of bleeding was found in the common diagnostic examinations such as upper and lower endoscopy and X-ray of the intestine. However, in the X-ray and the enteroscopy jejunal diverticula were seen on which angiodysplasias, detected in mesenteric angiography, projected. Because of the impossibility of stopping the bleeding by endoscopic interventions, the patient was referred to surgery and made an uneventful recovery. To a much lesser extent arterial aneurysms can also occur at any other vascular site due to a large number of possible reasons. In this article we conducted a Medline search of the world literature for reports on these rare vascular disorders and summarize the most important clinical aspects on this subject. The patient was a 31-year-old woman with a benign solid and papillary epithelial pancreatic tumor. Differentiation from a malignant tumor and from a hormonal active tumor was possible by the smooth boundaries and absence of contrast medium enhancement on CT. {"title":"[Value of MRI, CT and findings in staging of gynecologic malignancies].","authors":"M Lorenzen,&nbsp;J Braun,&nbsp;A Gehrckens,&nbsp;V Nicolas","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Purpose: </strong>The present study was designed to compare the value of MRI, CT and clinical examination in local tumor staging of gynaecologic malignancies.</p><p><strong>Patients and methods: </strong>99 patients with clinically suspected carcinoma of the cervix uteri, ovarian carcinoma or tumor recurrence after gynaecologic cancer underwent all three staging modalities. Furthermore CT and MRI were compared in detecting lymph node metastasis and peritoneal implants.</p><p><strong>Results: </strong>MRI was superior to CT and clinical staging in local tumor staging with an accuracy of 77% for cervical carcinoma and of 88% for recurrent tumors, whereas CT achieved an accuracy of 65% and 55% and clinical staging 60% and 63% accuracy for carcinoma of the cervix and recurrent cancer. Especially for local staging of these two tumor entities MRI is very useful. MRI and CT reached comparable results in the detection of ovarian tumors with an accuracy of 73% for MRI and 69% for CI. Evaluation of radiologic methods in the diagnosis of leiomyoma of the small intestine].","authors":"M Keberle,&nbsp;G Wittenberg,&nbsp;M Jenett,&nbsp;J G Müller,&nbsp;D Hahn","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Purpose: </strong>Various radiological methods to diagnose small bowel neoplasms and problems of differential diagnosis specially considering leiomyomas are discussed.</p><p><strong>Patients and methods: </strong>Two patients with leiomyoma of the ileum underwent enteroclysis, computed tomography of the abdomen, intra-arterial DSA and colour Doppler sonography. Another patient with leiomyosarcoma just underwent CT of the abdomen with CT-guided biopsy.</p><p><strong>Results: </strong>Due to the homogenous density and the smooth surface of the tumors in computed tomography and respectively enteroclysis and the presentation of the tumor vascularisation in the angiography and Colour Doppler sonography in both patients a leiomyoma of the small bowel was diagnosed. Postoperatively this diagnosis was histologically confirmed. The CT-findings of the patient with leiomyosarcoma were not suspicious of a malignant tumor.</p><p><strong>Conclusion: </strong>Radiologically it is not possible to determine the dignity of smooth muscle cell tumors safely. That is the reason why the diagnosis has to be achieved operatively. But the histopathological diagnosis based on the mitotic rate may be difficult. Chest CT scans and respective clinical data were compared.</p><p><strong>Results: </strong>16% of all examinations were indicated to resolve questions arising from the chest radiogram, 10% to confirm or exclude pulmonary embolisms and 10% to confirm or exclude aortic dissection. In 10% a focus of infection was sought. 57% of all CT examinations had an impact on therapy, in 7% further diagnostic tests were prompted. Among a total of 588 clinical decisions based upon chest CT, the most frequent therapeutic conclusions consisted in: minimally invasive CT guided interventions in 17%. A new drug was administered in 13%, surgical intervention was performed in 13%, bed-side interventions such as insertion of a drainage tube in 13%, and a given pharmacological therapy was continued in 11%.</p><p><strong>Conclusion: </strong>Chest CT has a strong impact on patient management in emergency and critical-care medicine. CT guided interventions are frequently used in critically-ill patients.
