[Interventional methods in breast diagnosis. Histological vs. cytological evaluation of core cut biopsies of the breast]. 乳腺诊断中的介入方法。乳腺核心切片的组织学与细胞学评价[j]。

It can be used prior to operation for the confirmation of a breast cancer and it can avoid unnecessary breast surgery in benign tumors. Unfortunately, only some centers are equipped with a pathology unit that allows an immediate histological examination. But often these institutes are provided with an experienced cystologist. Thus, the question arises if a cytology taken from the core cut biopsy is as reliable as an histological examination. In a prospective study we performed unroll-cytologies in core cut biopsies of 173 breast tumors in 169 patients consisting of 122 malignant and 51 benign tumors. Histology of core cut biopsies was proven by operational histology in all malignant and in 5 benign tumors. Histology of core cut biopsies could not be judged in 2 cases (lymphoma, gallert carcinoma). Cytological slide preparations were technically inadequate in 4 cases. The false negative rate of histology was 1/120 and 27/120 in cytology. Histology compared with cytology showed the following results: sensitivity 99.2% versus 77.5%, specificity 100% versus 95.9%, positive predictive value: 100% vs. 97.8%, negative predictive value: 98.1% vs. 63.5%, and accuracy: 99.4% vs. 82.8%. The sensitivity of cytology was much worse than that of histology of core cut biopsies. Thus, in our opinion cytology can provide a quick diagnostic orientation, but it cannot replace the more reliable histological examination. Parallel measurements were possible in 15 patients.</p><p><strong>Results: </strong>The phantom measurements proved a high stability of both methods. Regarding the absolute BMD the new method resulted in higher BMD values. This result was confirmed by parallel measurements in 15 patients. The new method demonstrated increased suitability due to less time consuming preparations. Dose reduced scan parameters could not be achieved in all patients, so that the time saving effect could not be considered. In these cases manual settings were required.</p><p><strong>Conclusion: </strong>A substitution of the established method for BMD measurement by means of SEQCT with the new method seems to be possible. {"title":"[Iodine delivery rate in catheter angiography under pressure conditions in manual injection].","authors":"H P Busch,&nbsp;K P Stocker","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Purpose: </strong>The aim of this study was to record the flow-rate and to calculate the \"iodine delivery rate\" (IDR) of contrast media of various viscosities when the contrast media are injected by hand.</p><p><strong>Methods: </strong>Five different catheters for coronary angiography were tested with the injection system Medrad Mark V Plus. Injections were performed with pressures of 100, 200 and 400 PSI. The contrast media examined were Imeron 350, Imeron 400, Omnipaque 350 and Ultravist 370. The IDR was calculated on the basis of the measured flow rate and the Iodine content of the contrast medium.</p><p><strong>Results: </strong>As was expected, the IDR was higher as the pressure increased. In addition to the Iodine content the viscosity of the contrast medium is a very important factor for the IDR. At both 100 PSI and 200 PSI the increase of the IDR was higher with Imeron 350 than with Imeron 400. The comparison of contrast media with identical Iodine contents but differing viscosities (Imeron 350, Omnipaque 350) clearly showed that the IDR depended on viscosity.</p><p><strong>Conclusion: </strong>The \"iodine delivery rate\" is a decisive factor in the opacification of arterial vessels. Both Iodine content and viscosity of a contrast medium are important for the IDR. Because of the low pressure at manual injection, contrast media with low viscosities should be used. {"title":"[Radiologic changes following cement-free implantation of hip prostheses. 5-year follow-up and clinical experience with the cement-free hip prosthesis Autophor-900-S].","authors":"K Wilhelm,&nbsp;R Conrad,&nbsp;H Reich,&nbsp;H Zieger,&nbsp;H Schild","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Purpose: </strong>To evaluate early reliable radiographic signs of loosening in cementless total hip arthroplasty compared to clinical follow-up.</p><p><strong>Methods: </strong>Within a time interval of 5 years 89 patients with 101 hips underwent total hip arthroplasty using the Autophor 900 S-prosthesis. Clinical and radiographic follow-up were first performed retrospectively followed by a prospective investigation.</p><p><strong>Results: </strong>Radiographic evaluation showed prosthesis fixation in 93%. Loosening of the prosthesis was identified in 4 cases of the acetabular component and in 4 cases of the femoral stem. Pain was associated with radiographically detectable loosening of the cementless arthroplastic components in only two cases.</p><p><strong>Conclusion: </strong>Within a follow-up period of up to five years, loosening of cementless total hip arthroplasty occurs in about 7%. The most reliable radiographic signs of loosening are complete radiolucent zones wider than 2 mm at the bone-component interface, progressive subsidence, migration, or tilt of the component. High contrast resolution in the xy-plane and in the z-direction were investigated with special phantoms. Noise levels were measured using a water phantom, the radiation exposure of lung parenchyma was investigated using an Alderson-Rande-Phantom. An anthropoid chest phantom was scanned to compare the image quality of the four protocols.</p><p><strong>Results: </strong>Protocol A showed the best resolution in the z-direction; the resolution in the xy-plane was equal for all protocols. Image noise in the center of the water phantom was 58 HU for protocol A and 40 HU for the other three. The following equivalent organ dose were measured for lung parenchyma: A 14 mSv, B 29 mSv, C 20 mSv, D 14 mSv.</p><p><strong>Conclusion: </strong>Scanning protocols with small collimation and a pitch of 2 are characterised by low radiation exposure and excellent resolution in the z-direction. This report describes the case of a patient with severe manifestation of PAN who developed a subcapsular hematoma from a spontaneously ruptured aneurysm in the spleen and a retroperitoneal hematoma from a renal cortex aneurysm. For the first time angiographic controls have been documented over years in this disease. The acceptance test should be performed after installation of the image documentation system by the manufacturer/specialists while constancy controls should be carried out weekly by the user. This often leads to its late diagnosis. The syndrome is characterised by portal hypertension and splanchnic congestion due to obstruction of hepatic venous outflow. This paper describes the treatment of three patients with Budd-Chiari syndrome by interventional therapeutic techniques and discusses alternative treatment modalities.</p><p><strong>Patients and methods: </strong>The first patient presented with veno-occlusive disease and was treated by the placement of a transjugular intrahepatic portosystemic stent-shunt. The second patient showed an occlusion of the major hepatic veins. After percutaneous recanalisation, a stent was placed in the right hepatic vein which remained patient. The third patient had a membranous obstruction of the right hepatic vein which was treated by percutaneous balloon dilatation.</p><p><strong>Results: </strong>In all patients the clinical symptoms resolved completely after treatment and no complications were encountered.</p><p><strong>Conclusions: </strong>The authors conclude that interventional therapeutic techniques offer a wide variety of possibilities for the treatment of patients with Budd-Chiari syndrome and are safe, effective and relatively inexpensive. The patient, 74 years old, was hit by a bullet in localization of the left masseteric region during world war II. The bullet entered the left Vena jugularis and was embolised into the middle lobe pulmonary artery (shock-position).
