{"title":"Women's evaluation of their childbirth performance.","authors":"M C Mackey","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To describe women's evaluation of their own performance during labor and delivery.</p><p><strong>Methods: </strong>Participants (N = 60) were Lamaze-prepared, married multigravidae, aged 21-37 years who responded to open-ended questions in tape-recorded interviews during their postpartum hospital stay. Qualitative data analysis was used to identify themes.</p><p><strong>Findings: </strong>Women evaluated their childbirth performance as managing well (45%), having difficulty (35%), and managing poorly (20%). The following helped them to manage well: short, fast labors; minimal pain; Lamaze; husband's encouragement; being informed; feeling strong; medication; and nurses' and physicians' encouragement, help with Lamaze, and information. Women who had managed well before and who were confident about managing well again tended to evaluate their childbirth performance positively. Males (n = 24) and females (n = 15); age range 3-15 days.</p><p><strong>Method: </strong>Retrospective, descriptive design. The author used a 10-item interview schedule and a 9-item clinical checklist, including observation of infants' umbilical cords and physical state.</p><p><strong>Findings: </strong>Of the 39 infants, 27 contracted neonatal tetanus; 11 died. All infants with tetanus were delivered at home by traditional, nonprofessional attendants. Factors contributing to high tetanus incidence included: lack of sepsis control, cord care, mothers' lack of immunization, delivery in settings.</p><p><strong>Conclusions & implications for nursing: </strong>Traditional nonprofessional attendants need training by professional nurses and midwives, and integration into the national health services. {"title":"The relationship between parental ratings of child behaviors, interaction, and the home environment.","authors":"D Koniak-Griffin,&nbsp;I Verzemnieks","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Problem: </strong>What is the relationship between child behaviors as perceived by the mother and father, mother-child interaction, and the home environment?</p><p><strong>Subjects: </strong>A subsample of 28 families of healthy, full-term newborns recruited from a larger longitudinal study.</p><p><strong>Methods: </strong>Mothers and fathers completed the Eyberg Child Behavior Inventory when their child was 24 months old. NCATS and HOME Inventory were administered. The Neonatal Perception Inventory was previously completed by the mother within 72 hours of birth and at 1 month postpartum.</p><p><strong>Findings: </strong>Significant interparent correlations for intensity of child behaviors; however, the association for behaviors considered to be problematic was not significant. Fathers tended to report fewer behavior problems and less frequent occurrences. Several significant relationships were found between child behavior ratings, interaction, and the home environment. Gender of child and maternal employment status did not significantly influence parental perceptions of child behavior. CONCLUSIONS AND IMPLICATIONS FOR NURSING: While parents may share perceptions of child behaviors, they may differ in interpretation of whether those behaviors are problematic. Administering behavior inventories to both parents may help them identify and respect each other's differing opinions. An in-depth analysis of the patient's significant memories six-and-a-half years after the incident is also presented.</p><p><strong>Sources: </strong>Two hour semi-structured, information gathering interviews with the adolescent when she was 16 years old.</p><p><strong>Results: </strong>The outcome of this case study analysis served as a partial debriefing of the experience for the adolescent girl.</p><p><strong>Implications for nursing practice: </strong>This case study provides a useful resource to practitioners and patients who may experience similar situations. The authors used an interview guide with 4 subsections to elicit self-reports of knowledge, perceptions, emotions, and evaluations.</p><p><strong>Findings: </strong>Major themes were anticipatory anxiety, pain, invasion of privacy, and being comforted. Average education: 13 years.</p><p><strong>Method: </strong>A short-term longitudinal correlational study to determine second-time mothers' moods relative to dimensions of support, stress, and timing of support. Instruments used: Profile of Mood States, Norbeck Social Support Questionnaire, Situation-Specific Support Scale, and the Crockenberg Stress Checklist.</p><p><strong>Findings: </strong>Depressed, angry, and/or tired mothers reported inappropriate amounts of support and dissatisfaction with support received.</p><p><strong>Conclusions & implications for nursing: </strong>Different types of support serve different functions before and after birth. Support must \"fit\" mothers' needs and is most effective if given when needed because of the limited \"carry over\" effect. The study also examined the sources of this knowledge. The convenience sample consisted of 33 healthy, married, middle-class women. For caretaking knowledge, on postpartum days 1-6, mothers learned most about feeding, and on days 7-13 about administering daily care. For personal knowledge, on day 1 mothers learned most about infant physical characteristics and on days 2-13 about infant activity. Mothers acquired significantly more personal knowledge about their infant than caretaking knowledge, both daily and across the 2 weeks. Knowledge acquisition was highest during hospitalization. A developmental model of stress and coping among children provided the theoretical framework for the study. A sample of 24 children, age 2.9 to 6.8 years old, participated. Mothers completed the Dimensions of Temperament Scale-Revised and a demographic information form. In the PICU, children's coping behaviors during chest tube removal were observed and recorded on the Children's Coping Strategies Checklist-Intrusive Procedures. Analysis used descriptive statistics, correlations, chi-squares, t-tests, and repeated-measures analysis of variance. Findings indicate that the predominant coping behaviors represented a self-protective approach to the procedure, followed by reaching out and controlling behaviors, and information-seeking behaviors. Specifically, the study sought to determine whether women who had PIH differed from women with normal pregnancies based on their descriptions and evaluations of weight, appetite, activity, and mood experiences for four sequential time intervals of pregnancy. Using a convenience sampling strategy and semistructured interview technique, data were collected from 38 women with PIH and 23 women with normal pregnancies. The body change data were analyzed using log-linear procedures with all statistical tests conducted at the p < .05 level. Study findings indicated the pregnancies of women with PIH differed from normal pregnancy. The children's behaviors were recorded on the Children's Coping Strategies Checklist-Intrusive Procedures; the helpful behaviors of health professionals were recorded on the Nursing Actions Checklist. Information about specific influencing variables was gathered through questionnaires and parent interviews. The children's report of procedural pain was correlated with the number of coping behaviors used, with the children's attempts to protect themselves, and with helpful nursing interventions. Socioeconomic status, nursing interventions, and preparation for the procedure had low to moderate correlations with number and type of the children's coping behaviors.
