A theoretical analysis of the solute transfer in a hemodialyzer is presented. Experiments were carried out with a continuous flow flat-plate dialyzer. Permeability of cuprophane membranes were calculated both for pulseless and pulsatile flows. It was found that the membrane permeability increases with flow rate but the asymptotic value of pulsatile flow is remarkably higher than that of the pulseless flow. However, the measurements of these variables before and after the dialysis procedure did not show any significant changes. In each case, significant graft recirculation due to stenosis of the venous limb caused the insidious onset of uremic symptoms in patients apparently receiving adequate hemodialysis. The uremic syndromes disappeared after correction of the faulty vascular access. Recirculation may be a \"silent\" cause of prosthetic graft malfunction which is promptly diagnosed by a simple mathematical formula. Routine estimates by a simple mathematical formula. Routine estimates of recirculation should be performed in any chronic hemodialysis patient with relapse of uremic symptoms or suspicious serum chemistries despite regular, uncomplicated hemodialysis. Relations were developed to calculate permeance based on a minimum number of experimental measurements. Permeance is the product of the effective peritoneal area times its mass transfer coefficient. The relations were used on clinical data obtained with and without nitroprusside, a direct vasodilator, added to the dialysate. Nitroprusside increased permeance 34% for BUN and about 100% for creatinine and inulin. The equivalent increase in clearance in a 50 minute exchange protocol would be 24, 60, and 83%. Normalized consecutive concentration data obtained every ten minutes in several long exchanges agree well with calculated values supporting the adequacy of the approach. An expression is also given to calculate the residual dialysate volume left from the previous dialysate exchange. Similar correlation was shown also in a group of moderately azotemic patients who had not yet entered the chronic hemodialysis program. Use of whole saliva in these tests instead of blood samples may reduce the iatrogenic component in anemia, the frequency of venipunctures and of blood samplings. So far, over 350 treatments in 8 patients have been performed. After 2 hours of treatment the removal of urea, creatinine, phosphate and uric acid, is similar to that obtained by 4-6 hours of haemofiltration. The alkalinazation of the patient through direct venous infusion of bicarbonate, makes predialysis acid-base significantly better than in standard haemodialysis and haemofiltration. Asymptomatic correction of severe fluid overload is easily obtained like in isolated ultrafiltration. The role of osmolality and vasopressors are discussed. A dry weight below the value obtained by previous dialysis treatment is achieved, and volume dependent hypertensions as in haemofiltration are corrected after 2-8 weeks. Predictability of and capacity for ultrafiltration were excellent and equally good for both products. Leak rates in Travenol coils were 9.3% as compared to 4.3% for Extracorporeal coils. Dialysances of BUN, creatinine, and phosphate were significantly higher with Travenol coils than with Extracorporeal coils, but this increase was not as great as might be expected from the greater surface area of the Travenol coil. Despite the greater functional efficiency of the Travenol coils we could detect no difference in the degree of biochemical control of the patients as judged by standard pre and post dialysis blood chemistries. Three patients are described; one with primary amyloidosis, a second with insulin dependent diabetes mellitus, and a third with heroin nephropathy who developed S.B.E. Early introduction of this modality is considered vital and factors of age, education, socio-economic level, sensory and environmental conditions are considered in the evaluation process. The social work relationship with patients is described within the context of family and community supports. Experience has demonstrated that patients with pre-existing adaptation problems may require more concentrated social work counseling. The goals of independence and rehabilitation are shown to be effected by patient motivation and economic restrictions. The social worker participating in the team approach is described as contributing to the patients opportunity for self-determination through patient advocacy, supportive services and social action.
