This incidence of 38% was significantly higher than the zero incidence of four prior years when no aluminum was added to city water (p less than 0.05) and also when compared to the zero incidence in the 2.5 years subsequent to deionization of dialysis water which lowered its aluminum content to less than 1 microgram/liter (p less than 0.0002). Experimental results show that with this technique: 1) urea clearance can be increased some 200% over control values, 2) the convective transport of urea is unimportant and the increased urea clearance is due primarily to increased peritoneal permeability, 3) net ultrafiltration and electrolyte balance can be easily controlled by variation of total electrolyte and glucose about an appropriate mean. The results strongly supported the hypothesis. Evidence incriminating aluminum as a neurotoxin among the dialysis population is strong. The source of this aluminum has not been clearly defined, although both gastrointestinal and parenteral routes may be involved. Fluid control was facilitated in oedematous patients but the number of hypotensive episodes during the combined procedure requiring intravenous 5% saline did not significantly decrease. No significant improvement in hypertension was noted. Ultrafiltration (U.F.) alone for acutely water overloaded, azotaemic patients proved very useful. Two to five liters of oedema fluid could be removed asymptomatically in one to three hours using transmembrane pressures of 250 to 500 mmHg and U.F. rates of 10 to 42 ml/min. Two patients became acutely and symptomatically hypotensive. It was administered intravenously in 1.5mEq/min for 5 min followed by 3.8mEq/min for another 5 min. As control, 1.71mEq/ml of sodium chloride was infused in a similar manner. Following the infusion of sodium acetate, heart rate increased slightly but significantly (p less than 0.01). Limb blood flow measured in an upper limb by the venous occlusion method, showed an increase in all of them and it was significant( p less than 0.001). The peripheral vascular resistance (=mean arterial blood pressure/limb blood flow) showed a significant fall (p less than 0.001). The analysis of systolic time interval showed a change suggestive of depression of heart function at the end of sodium acetate infusion (p less than 0.05). Such a vasodilating effect or a change in heart function was not observed following sodium chloride infusion. We report two chronic hemodialysis patients with unstable, disabling angina in association with severe coronary artery disease involving all three major vessels. Both successfully underwent coronary artery bypass surgery and subsequently experienced dramatic clinical improvement. The acquisition of immunity was assessed by hemagglutinin-inhibition titers of antibody to A/NJ/76. Seroconversion was seen in 67% of dialysis patients and 50% of transplant patients. Within the dialysis patient groups (29 patients) there was no correlation with age or length of time on dialysis to the immunization response. The only variables important in seroconversion in the transplant group (22 patients) were age and creatinine clearance. Transplant patients 18-25 years old had a much lower response (12.5%) than did the under 18 (75%) and over 25-year-old patients (90%). All patients with a creatinine clearance greater than 65fl'J/min. responded, whereas only 44% seroconverted with a creatinine clearance less than 65 ml/min.
