In the physiology, the reflection coefficient of urea is usually regarded to be negligibly small though it varies from one organ to another. The value obtained in the present paper is large and urea was supposed osmotically to be active affecting the fluid movement across the capillary wall. Lipid abnormalities were found in 72% of the transplant patients and 52% of dialysis patients. The mean serum triglyceride levels were significantly higher in the transplant and the dialysis group than age and sex matched controls, but were not significantly elevated in the steroid-treated patients with normal renal function. The mean cholesterol levels were normal for the transplant and steroid-treated groups, but significantly reduced in the dialysis group. Alkalinization converts ammonium into free ammonia which can be removed as a gas. Our approach is to use a capillary hemodialysis as a gas absorption unit with liquid moving through the blood compartment and air through the dialysate compartment to extract ammonia gas. This paper examines the effects of air flow rate, fluid flow rate, and concentrations of ammonia (and urea) in the fluid. It was observed that increasing air flow rate or dialysate flow rate increases the mass transfer rate for ammonia especially at low ammonia concentration levels. Ammonia removal rate increases with air flow rate linearly. Normal or only slightly abnormal hearing sensitivity was found in 9 of our 23 patients while the remaining 14 patients had marked hearing abnormalities at the onset of dialysis treatment. Neither group showed any significant change in their audiographic findings during the study period. Rapid changes in hemodynamic and biochemical parameters are known to occur during dialysis and may alter muscle performance. Ten patients maintained on chronic hemodialysis had measurements of quadriceps muscle and handgrip strength performed before and after dialysis. In seven of these patients maximum static inspiratory and expiratory pressures (PI max and PE max respectively) were similarly determined. Quadriceps muscle strength improved in 6 patients, decreased in 3 and remained unchanged in 1. Handgrip strength increased in 5 patients, decreased in 3, and remained unchanged in 1. PI max decreased in 6 patients and increased in 1, and PE max decreased in 4 patients and increased in 3. No significant correlation was found between the changes in strength of the tested muscle groups and the serum concentrations of sodium, potassium, calcium, phosphorus, urea nitrogen, and creatinine, changes in blood pressure of fluid balance. The dialysis-induced changes in biochemical parameters may exert opposing effects on neuromuscular performance. The determinations were carried out with undiluted plasma to which bilirubin, about 10 mg/100 ml, had been added. The free fraction of bilirubin in plasma (ratio of free to total concentration was (19.8 +/- 8.1) X 10(-5) in the surgically anephric, (10.4 +/- 4.0) X 10(-5) in the physiologically anephric, and (1.81 +/- 0.34) X 10(-5) in the normal subjects (mean +/- standard deviation). Based on this difference in plasma protein binding, patients in renal failure may be expected to have lower steady state plasma concentrations of total (free plus bound) bilirubin than do patients with normal renal function if the formation rate and intrinsic metabolic clearance of the pigment are the same as in normal subjects. We evaluated four currently available disposable adult dialyzers in vitro and in vivo during treatment of adult patients. All four dialyzers, Gambro GLP 1.36m2, 17 mu, Cordis Dow 3500, Gambro GF 120M, and Travenol HD 1000 were well suited to the dialysis of adult patients. Total plasma cholesterol and triglyceride concentrations and the content of these lipids in the VLDL, LDL, and HDL lipoprotein fractions were not significantly different when these dialysis periods were compared. When patients were subgrouped according to whether they were normo or hypertriglyceridemic or whether they had an adequate or deficient lipolytic response to heparin infusion, there were also no differences apparent between the acetate vs. bicarbonate dialysis periods.
