This paper delineates minimum ways for mandated social service to be delivered and made accountable. It also explores the role of the social worker as a member of the Network Coordinating Council and Medical Review Board. {"title":"Evaluation of three dry-sterilized hollow fiber artifical kidneys.","authors":"A Streifel, J Ebben, C Sahr, R Meyer, J Shideman, R Lynch, C Kjellstrand","doi":"10.3109/08860227809079321","DOIUrl":"https://doi.org/10.3109/08860227809079321","url":null,"abstract":"<p><p>We investigated three new dry sterilized hollow fiber artificial kidneys (HFAK) (Cordis Dow CDAK 1.3, Travenol CF 1200, Extracorporeal Tri-Ex 1). Dry sterilization makes these dialyzers more economical by shortening set-up time. Dry sterilization also eliminates iatrogenic administration of residual sterilant. Water of imbibition can significantly increase the blood compartment volume of the dialyzer during dialysis. Consequently, a corrected blood volume for each dialyzer was established; these corrected volumes varied from 13--36% greater than the volume determined before dialysate flow. With low dose heparinization of these dialyzers there was between an 18 and 45% decrease in the post dialysis volume, presumably due to fiber clotting during dialysis. This volume added to the residual blood loss measured by a colorimetric technique accounted for a possible blood loss ranging between 26.9 and 53.9 ml per dialysis for these three dialyzers. Our results suggest that a relationship between dialyzer clotting and decreased dialyzer efficiency may exist. These three capillary flow dialyzers had a much lower platelet drop (0--9% pre to post) when compared to 30--40% drop of flat plate dialyzers. The duration of bleeding was reduced by 50% compared to the control period in patients with various internal arteriovenous communications undergoing maintenance hemodialysis treatment. The patients were lymphopenic, but had normal T, B and null cell percentages, regardless of tehir primary renal disease. The background and provisions of the Rostenkowski Bill, H.R. 8423, are summarized. Primary aims of the bill are to remove disincentives for use of self-dialysis, remove certain disincentives to transplatation, and to provide for peer review and goal setting for network use of various treatment modalities. Twenty-seven dialysis patients had a two year cumulative survival of 74% compared to 54% for 22 transplantation patients. The cumulative survival of live-related donor recipients (77%) was similar to that of the dialysis group and significantly better than that of cadaveric allograft recipients (36%). While the incidences of cardiomegaly and of motor neuropathy were high among live-related donor recipients, dialysis patients more often demonstrated peripheral vascular disease. Causes of death in hemodialysis patients included cardipulmonary arrest and patient decision to discontinue therapy; in the transplantation group included cardiopulmonary arrest, sepsis, and stroke. Living-related transplantation remains the preferred mode of therapy because of the potential for rehabilitation. The psychological issues of dependence-independence, activity-passivity, and control and mastery are frequently referred to in the literature as being important in understanding the chronic stress of illness and treatment. The measure of successful coping should be determined by adaptive or maladaptive value of behavior rather than traditional concepts of normality or abnormality. Treatment planning and patient selection should be tailored to the psychological needs of the patient, the family and the staff. Several clinical vignettes are presented to illustrate that initial impressions of patient suitability for self-care and home dialysis may be deceiving. Plasma triglyceride concentrations remained constant during a 4-hour period of hemodialysis of uremic and non-uremic dogs against an acetate concentration of 39.5 mM (delivering 3 mEq/Kg/hr) plus infusion of 12.5 muCi/Kg/hr of acetate-1-14C, but rose progressively following dialysis. Radioactivities in plasma phospholipids, triglycerides and cholesteryl esters increased during dialysis and continued to rise in the post dialysis period, whereas 14C activity in free fatty acids and free cholesterol increased during dialysis and decreased post-dialysis. Acetate 14-C incorporation into plasma triglycerides was similar in the uremic and non-uremic groups, but incorporation into plasma cholesterol was higher in the uremics. Liver triglyceride concentrations and radioactivities at 12 hours were higher in the uremic dogs. At this time, adipose tissue 14C incorporation was also higher in uremic dogs but no differences were observed in aorta, heart or sciatic nerve incorporation. Corrected to a mean intraperitoneal dwell time of 36 minutes, mean clearances of creatinine and urea were 0.74 and 0.90 ml/kg/min in six rabbits. With intraperitoneal nitroprusside, 1.13 mg/kg clearances increased to 1.13 and 1.30 ml/kg/min (p less than 0.01) respectively. The 53% increment in creatinine clearance maintained the ratio clearance larger/smaller solute suggesting increased peritoneal permeability and/or area. Lower nitroprusside doses were less effective and not significantly above control. Nitroprusside also increased clearances during hypertonic peritoneal dialysis, but had no effect on osmotically induced water flux.
