Epidemiology in a health services administration curriculum: what topics are important to practicing healthcare administrators? 卫生服务管理课程中的流行病学:哪些主题对执业卫生保健管理人员很重要?

The goal of the research described here was to identify epidemiologic topics seen as important to healthcare executives. A survey was conducted of a convenience sample of alumni the Graduate program in Health Services Administration of Xavier University in Cincinnati Ohio. Alumni were asked to rank 68 epidemiologic topics on a likert-type scale: 1 = Not important; 2 = Slightly Important; 3 = Important; 4 = Very Important; 5 = Not applicable. Demographic characteristics were collected including: age of respondents, gender, years since graduation, and current position. All responses of important (3) and very important (4) were grouped together. Topics were then ranked based on the percent of respondents who answered the question and ranked the topic as a 3 or 4. A total of 858 surveys were mailed to alumni, and 177 completed surveys were returned for a 21% response rate. Respondents were 64% male, and there were similar number of graduates from all age groups and from year of graduation. The largest groups of respondents were CEOs and Managers. Clinical guidelines, Quality and Satisfaction, and Benchmarking ranked highest for number or respondents ranking the topic as important or very important. The majority of graduates indicated that almost all epidemiologic topics are important or very important. We report on our experience in managing 8 open online courses at http://nhs.georgetown.edu/open. Open courses have several advantages over password protected courses: (1) they are available through search engines and thus reduce the program's marketing cost, (2) continuous feedback from the web enables rapid improvements to the course, (3) customer relationship tools, tied to open courses, radically reduce faculty time spent on one-on-one emails while increasing student/faculty interaction. We provide details of one course. In 15 weeks, 803 emails were received by and 1181 sent by the faculty (all within 6% of a working week and 82% savings of faculty time). We show how open courses can be accessed through search engines, how students questions are answered on the web and how student projects, in popular sites such as You Tube and Face Book, improve course marketing. The many challenges in healthcare today create a special need for great leadership. This paper reviews the leadership competencies needed by physician-leaders and current experience with developing physician-leaders in healthcare institution-sponsored programs. On the basis of this review, six key leadership competency domains are proposed: 1. technical skills and knowledge (regarding operational, financial, and information systems, human resources, and strategic planning), 2. industry knowledge (e.g., regarding clinical processes, regulation, and healthcare trends), 3. problem-solving skills, 4. emotional intelligence, 5. communication, and 6. a commitment to lifelong learning. Review of current experience indicates that, in addition to leadership training through degree and certificate-granting programs (e.g., by universities and/or official medical societies), healthcare institutions themselves are developing intramural programs to cultivate physician-leaders. The Global Healthcare Management Faculty Forum surveyed both domestic and foreign program directors to get a sense of current attitudes toward globalization and resultant healthcare management educational issues. We also surveyed program directors in other countries. Program directors clearly understand the importance and impact of globalization on domestic healthcare services. However, they are just beginning to think about the implications of such developments on program mission and objectives, and curriculum development. There is a need for development of curricular resources and systems to assist programs in refining missions, curriculum change and course development. These factors are rapidly giving rise to new ecological paradigms of environmental stewardship and in healthcare environments that express sustainable theories and practices. This has given rise to a systematic system for promoting and assessing the energy performance and efficiency of healthcare facilities known as Leadership in Energy Efficient Environmental Design (LEED), and a parallel certification program, the Green Guide for Heath Care. These developments are examined in direct relation to the functions of managerial ethics. A series of ten sustainability-based ethical dilemmas are presented. This article describes cutting edge research used to create the theoretical foundation for a competency-based approach to diversity management education in the health services administration curriculum. Detailed implementation steps of the course pilot are provided, including pedagogical methods and outcome evaluations, which are largely absent from the diversity management education literature.
