Ce lien conceptuel relève pourtant d’une compréhension réductrice de la santé. Une herméneutique amplificatrice et qualitative de la prédiction, fondée sur une approche conceptuelle, narrative et identitaire de la santé, permet un soin mieux prédictif, c’est-à-dire, plus proactif, provocateur et plus centré sur l’identité narrative du malade.</div></div><div><div>In contemporary medical practice, the notion of prognosis is mainly linked to the question of probability and explanation, in relation to the cause/effect couple, and within the framework of evidence-based medicine (EBM). However, this conceptual link stems from a reductive understanding of health. Cette prise en charge souffre de contradictions internes. D’une part, elle postule encore trop souvent que les sans-abris sont dépourvus d’autonomie. D’autre part, elle consiste en une injonction paradoxale, puisqu’elle incite des individus à l’autonomie, tout en les privant des moyens de la mettre en œuvre. Elle crée donc les conditions de son propre échec.</p></div><div><p>This article questions the support of homeless people based on the concept of autonomy. This support suffers from internal contradictions. On the one hand, it still too often assumes that the homeless are deprived of autonomy. On the other hand, it consists of a paradoxical injunction, since it encourages individuals to be autonomous, while depriving them of the means to implement it. Castelli , R. de Crevoisier","doi":"10.1016/j.etiqe.2024.03.002","DOIUrl":"10.1016/j.etiqe.2024.03.002","url":null,"abstract":"<div><p>À partir d’une réflexion sur les enjeux éthiques d’une hospitalité incarnée dans le champ du soin, s’appuyant sur une méthode philosophique à l’écoute des préoccupations des professionnels de terrain, l’article interroge notre rapport sensible aux espaces, au carrefour de l’éthique et de l’esthétique, dans le contexte, peu étudié, de la radiothérapie et de ses spécificités. L’hospitalité suppose de la non-identité, de la non-appartenance, de la différence. Le risque est alors qu’une alliance thérapeutique de qualité ne puisse être nouée avec le patient auquel pourtant s’adressent les soins. Comment faire pour que le patient puisse se sentir <em>comme</em> chez lui tout au long des traitements ? En quoi le fait d’investir les espaces hyper-techniques d’une attention à autrui et d’une intention éthique peut-il contribuer à l’humanisation des parcours de soin et soutenir l’autonomie des patients ? Soignants et soignés sont enveloppés dans des espaces et des environnements très particuliers et vivent des expériences qui mettent à l’épreuve leur rapport au monde. Prendre au sérieux la corporéité, dans sa dimension à la fois active et passive, apparaît comme un préalable au développement de liens d’interdépendances féconds et au besoin de sens exprimé par les patients comme par les soignants. Si des écueils peuvent être identifiés, l’attention aux atmosphères, aux ambiances et au design peut cependant d’une part favoriser le bien-être des patients, leur (re-)donner du pouvoir pour agir, et d’autre part mobiliser l’inventivité dans les pratiques de soin, selon une exigence de justice et d’équité.</p></div><div><p>Based on a reflection on the ethical issues of genuine hospitality in the field of healthcare, and drawing on a philosophical method centered on the concerns of healthcare professionals, the article examines our sensitive relationship with spaces, at the crossroads of ethics and aesthetics, in the little-studied context of radiotherapy, which has its own specific features. Hospitality implies non-identity, non-membership and difference. The risk, then, is that a high-quality therapeutic alliance cannot be established with the patient, even though he or she is the beneficiary of the treatment. How can we ensure that patients feel “at home” throughout their treatment? How can investing hyper technical spaces with attention to others and an ethical intention help to humanise care pathways and support patients’ autonomy? Caregivers and patients are enveloped in very special spaces and environments in which they have experiences that put their relationship with the world to the test. Taking human corporeality seriously, in both its active and passive dimensions, appears to be a prerequisite for the development of fruitful interdependencies, in line with the need for meaning expressed by patients and carers alike.
