S’il n’est pas question de remettre en question l’apport d’une telle médecine en termes de recherche et de solutions thérapeutiques innovantes, il importe néanmoins de développer une analyse critique de ses présupposés. Une telle analyse conduit à souligner le rôle qu’on entend lui faire jouer au sein des pratiques médicales disponibles, et même, plus largement, au rôle que l’on accorde aujourd’hui à la médecine au sein des pratiques sociales et dans un marché de la santé qui tend de plus en plus à devenir un marché du risque et de la responsabilité individuelle.</p></div><div><p>In this article, we aim to explore the ethical, political, and social issues that arise from “personalized medicine”, particularly in its current manifestation as “precision medicine”. While there is no question of questioning the contribution of such medicine in terms of research and innovative therapeutic solutions, it is nevertheless important to develop a critical analysis of its presuppositions. Such an analysis emphasizes the role one intends it to play within available medical practices, and even, more broadly, the role medicine assumes today within social practices. Technological developments over the past few decades have given the impression of omnipotence to the point where humans feel either demiurge or overwhelmed. In this text, we will explore some of the current technological issues with their risks and their positive points. La notion de risque a permis de mesurer la fréquence et les conséquences d’un dommage dans l’objectif de les éviter ou d’en limiter les impacts. Mais cette vision probabiliste se heurte à bien des niveaux à l’incertitude et l’imprécision, notamment dans la relation de soin. La confiance, qui s’affranchit de l’incertitude et table sur la bienveillance d’autrui, dans un mouvement de réduction de la complexité du monde, s’invite en médiation du risque. La décision partagée est notamment un moment clef où incertitude, risques et confiance cohabitent. L’ambivalence de la notion de risque, à la fois danger et chance, fait opportunément écho à l’ambivalence du patient entre craintes et espoir, car si le pire peut arriver, le meilleur peut également advenir. Sinon, personne jamais ne ferait confiance.</p></div><div><p>Human has always moved forward in the world with the aim of mastering dangers, through knowledge, anticipation and forecasting. The notion of risk has made it possible to measure the frequency and consequences of damage, with the aim of avoiding them or limiting their impact. But this probabilistic vision comes up against uncertainty and imprecision at many levels, particularly in the healthcare relationship. Trust, which frees us from uncertainty and relies on the benevolence of others, in a move to reduce the complexity of the world, is invited to mediate risk. Shared decision-making is a key moment when uncertainty, risk and trust coexist. The ambivalence of the notion of risk, both danger and chance, is aptly echoed in the patient's ambivalence between fear and hope, for if the worst can happen, the best can also happen.
