{"title":"A Multicenter Study of Pulmonary Critical Care Trainees' Perception of Airway Management Training during Fellowship.","authors":"Christopher Ghiathi, Anthony R Lanfranco, Janae K Heath","doi":"10.34197/ats-scholar.2024-0033OC","DOIUrl":"10.34197/ats-scholar.2024-0033OC","url":null,"abstract":"<p><strong>Background: </strong>There is significant variability in airway management training among pulmonary and critical care medicine (PCCM) fellows.</p><p><strong>Objective: </strong>To assess the airway management training of PCCM fellows, specifically evaluating the role of the institutional approach to intubations (anesthesia-predominant primary operators vs. PCCM-predominant) to the overall fellows' educational experience.</p><p><strong>Methods: </strong>We performed a survey of PCCM fellows at 14 institutions to assess the perceptions of airway management training. Summary statistics were used to analyze the Likert scale responses. Based on our hypothesis of a relationship of a fellow's experience to the institutional approach to intubations, we stratified responses based on whether the primary operator for airway management for critically ill patients was generally anesthesiology or PCCM providers. Statistical significance was determined using either <i>t</i> testing (after converting Likert-style responses to continuous variables) or analysis of variance for categorical variables (<i>P</i> < 0.05).</p><p><strong>Results: </strong>Respondents included 132 fellows across 14 PCCM programs (47% response rate), spanning all fellowship years (29% in first year [<i>n</i> = 38], 31% in second year [<i>n</i> = 41], 26% in third/fourth year [<i>n</i> = 34]). Seventy percent (<i>n</i> = 80) of fellows were very or somewhat satisfied with their airway management training. In fellows training where anesthesia providers primarily perform airway management (29% [<i>n</i> = 34]), satisfaction with training was significantly lower, as was the self-assessment of one's airway management skills. The location of airway management experience in anesthesia-predominant programs was mainly the operating room (69% [<i>n</i> = 22]), versus mainly in the intensive care unit for PCCM-predominant programs (94% [<i>n</i> = 77]). The number of completed intubations was lower for fellows in anesthesia-predominant programs (<i>P</i> = 0.02).</p><p><strong>Conclusion: </strong>We found significant differences in fellows' experiences in airway management based on the institutional approach to intubations (anesthesia-predominant primary operators vs. PCCM-predominant). The use of artificial intelligence (AI) engines to generate content in academic spaces is controversial and has been meet with swift responses and guidance from academic journals and publishers regarding the appropriate use or disclosure of use of AI engines in professional writing. To date, there is no guidance to applicants of graduate medical education programs in using AI engines to generate application content-primarily personal statements and letters of recommendation. In this Perspective, we review perceptions of using AI to generate application content, considerations for the impact of AI in holistic application review, ethical challenges regarding plagiarism, and AI text classifiers. The Accreditation Council for Graduate Medical Education mandates PCCM fellows' competency in this procedure; however, the learning experiences vary across programs. After conducting a needs assessment, we developed a curriculum unique to our institution to supplement our fellows' existing EI experiences in the operating room and the intensive care unit.</p><p><strong>Objective: </strong>To assess the curriculum's short-term objectives: knowledge acquisition, maintenance, and practical skills 1 year after participation.</p><p><strong>Methods: </strong>We administered a survey to the graduating PCCM fellows for two consecutive years. We designed the comprehensive airway curriculum to include didactic lectures and simulation-based education. The knowledge acquisition and maintenance were measured by administering a 26-question knowledge survey before and after curriculum participation and after 1 year. The fellows also received a practical examination 1 year after participation. To compare knowledge survey scores, we used paired <i>t</i> tests and permutation tests.</p><p><strong>Results: </strong>In the needs assessment, 56% of graduating fellows believed they were proficient in performing EI, whereas 33% were undecided and 11% believed they were unprepared. Most believed they would need more than two courses after graduation to be confident in independently performing EIs. Most will only occasionally have backup for EI from anesthesiology or emergency medicine in their future jobs. One identified barrier to learning EI was the lack of a formal curriculum. In the knowledge assessment, nine first-year fellows participated in the curriculum. The cohort's mean presurvey score was 13.0 (standard deviation [SD], 4.5) versus 18.6 (SD, 3.6) mean postsurvey score. One year after participation, the mean survey score was 17 (SD, 1.2). The postsurvey and 1-year postparticipation survey scores were significantly higher than the presurvey scores (<i>P</i> < 0.05). One year after participation, the practical examination showed most fellows retained skills in EI using ramped position, video and direct laryngoscopy, bag-mask ventilation, and oropharyngeal airway placement.</p><p><strong>Conclusion: </strong>The airway curriculum enhances fellows' knowledge acquisition and maintenance 1 year after participation. This Perspective explores the importance of undergraduate critical care education, the current national and international landscape, innovative educational strategies and exemplar curricula, and recent advances in assessment that may better reflect learner-centered educational outcomes.
