This poses major barriers to conducting holistic application reviews. <b>Objective:</b> To create and implement a data-driven selection process that promotes holistic reviews within a tight timeline to select applicants for invitation to interview. <b>Methods:</b> In 2022, we conducted a survey of clinical faculty and fellows to ascertain the experiences, attributes, metrics, and characteristics deemed important for success in our training environment. We formed a selection committee and used the survey results to construct an automated screening tool and a faculty-completed application review form with resultant data summarized to aid in data-supported selection decisions. <b>Results:</b> Among the 60 survey respondents, 42 (71%) were faculty members and 17 (29%) were current fellows. The six most important items for trainee success fell under the domain of leadership attributes. Survey results were used to create a weighted screening score that was used for initial triaging of applications and a weighted faculty-reviewed application score standardized to each faculty reviewer and used to select applicants for interviews. {"title":"The Impact of Applicant Sex on Pulmonary and Critical Care Medicine Fellowship Letters of Recommendation: A Multisite Study.","authors":"Kaitland M Byrd, Michelle Spiegel, Edward F Kilb, Kelly Vranas, Liana Schweiger, Kathleen T Lee, Julia Schroeder, Alexandra Vita, Gareema Agarwal, Irada Choudhuri, Ishita Sunita Arora, Nandini Sarma, Snigdha Jain, Kathleen M Akgün, Janae K Heath, Elizabeth M Viglianti","doi":"10.34197/ats-scholar.2024-0074OC","DOIUrl":"10.34197/ats-scholar.2024-0074OC","url":null,"abstract":"<p><p><b>Background:</b> Letters of recommendation (LORs) are crucial for fellowship applications, but implicit biases can affect their quality. <b>Objective:</b> The aim of this study was to examine sex-based differences among multiinstitutional pulmonary and critical care medicine (PCCM) fellowship LORs. <b>Methods:</b> All 2021 PCCM fellowship applications submitted to five regional U.S. programs (South, Midwest, East, Northeast, and West) were deidentified, and applicant demographics and accomplishments were abstracted. The letter writer's gender was identified through an online search of professional websites. Linguistic Inquiry and Word Count was used to identify the frequency of adjective types (e.g., grindstone, communal). We measured the associations between applicant sex and the outcomes of total word count and degree of support using multivariable linear regression models adjusting for applicant demographics, accomplishments, and letter writer's gender for each program. <b>Results:</b> In total, 9,153 LORs were included in the analysis. The majority of applicants (64.1% [<i>n</i> = 1,703 of 2,658]) were male, and most letter writers (72.1% [<i>n</i> = 6,603 of 9,152]) identified as men. When adjusting for applicant demographics and accomplishments and letter writer's gender, female applicants' LORs had significantly more supportive words compared with those of male applicants in three of the five programs (Midwest, 2.4 more words [95% confidence interval (CI), 0.5-4.3; <i>P</i> = 0.01]; South, 1.9 more words [95% CI, 0.1-3.6; <i>P</i> = 0.03]; Northeast, 2.5 more words [95% CI, 0.2-4.9; <i>P</i> = 0.04]) and longer letters in two programs (South, 40.1 more words [95% CI, 12.3-68.0; <i>P</i> = 0.005]; Midwest, 29.8 more words [95% CI, 0.7-58.9; <i>P</i> = 0.045]). <b>Conclusion:</b> The length and supportiveness of PCCM fellowship applicants' LORs varied by applicant sex and region. However, pulmonary and critical care medicine (PCCM) fellowships across the country do not universally have dedicated lung transplantation curricula, and there is scarce literature in this area. We describe the generation of a content outline for curriculum development created through modified electronic Delphi methodology.</p><p><strong>Objective: </strong>To identify \"must know\" topics in lung transplantation for a graduating PCCM fellows.</p><p><strong>Methods: </strong>We developed expert consensus on \"must know\" lung transplant topics and subtopics (\"items\") for a graduating fellow through three survey rounds from July 2023 to October 2023. Thirty-three panelists were asked to rank author-provided items as \"must know,\" \"nice to know,\" or \"not necessary to know\" and to generate new items in round 1 to be submitted for ranking in subsequent rounds. \"Must know consensus\" was defined as ≥50% must know responses.</p><p><strong>Results: </strong>Survey response and completion rates across rounds were 58-70% and 96-100%, respectively. In round 1, panelists were provided eight topics and 30 subtopics for initial ranking. Panelists generated 82 new items in this round, of which 19 were included in round 2 for ranking after author review, together with one item from round 1 that required reranking. From round 2, only two items required reranking, and these items comprised round 3. Seven topics were identified as \"must know\": transplant immunology, evaluation of a lung transplant candidate, surgical and peri/postoperative management, rejection, immunosuppression, infections, and other complications, together with 36 subtopics.</p><p><strong>Conclusion: </strong>Self-identified lung transplantation experts obtained consensus on seven \"must know\" topics and 36 subtopics in lung transplantation for graduating PCCM fellows.
