The lesion was treated with a semi-compliant balloon and drug-eluted stent (DES). After the stent placement, a large arterial perforation with fistula to right ventricle was observed. There was no hemodynamic instability due to the fistula progression during the procedure and the perforation point was successfully closed with a stent-graft. It is more common in younger patients and women and it can cause acute coronary syndrome (ACS) in 0.1 to 4 percent of cases. We described three cases of different types of SCAD. In the first case, we described a male patient admitted due to chest pain after excessive physical activity. Electrocardiogram (ECG) showed inferior ST-elevation myocardial infarction (STEMI). SCAD of left anterior descending (LAD) was found by coronary angiography. Because of chest pain and signs of ischemia on ECHO, a stent was implanted. After procedure, the patient was pain-free and hemodynamically and rhythmically stable. In the second case, we presented a young multiparous woman with signs of acute coronary syndrome (ACS). A coronary angiogram showed a homogeneous stenosis of proximal LAD. Optimal medical therapy (OMT) was the initial therapy of choice. After a few days due to the recurrence of chest pain and an increase in troponin level, recoronarography was performed. It showed a dissection of the left main (LM), left circumflex artery (LCx) and LAD. It was treated with the two-stent technique procedure, after which, the patient was stable and pain free. In the third case, we showed a postmenopausal patient with subclinical hypothyroidism. Although considered the gold standard for diagnosis of myocarditis, endomyocardial biopsy in clinical pactice is often substituted with clinical, biochemical and imaging parameters. Echocardiography with speckle tracking strain analysis is a valuable tool for diagnosis of myocarditis, estimation of regional and global systolic function showing good correlation with cardiac magnetic resonance imaging (cMRI). We present the case of a 19-year-old male with chest pain, elevated troponin and ST segment elevation who underwent urgent coronary angiography. Normal angiogram steered diagnosis toward myopericarditis, considering information about recent enterocolitis and vaccination against SARS-CoV-2. Transthoracal echocardiography with speckle tracking analysis showed longitudinal strain reduction in several segments of the left ventricle. Small pericardial effusion alongside those segments was also found supporting our diagnosis. After 3 months of treatment with antiinflammatory drugs, ACE inhibitor and beta blocker, the patient recovered and a repeated echo showed complete recovery of systolic function without pericardial effusion. We conclude that echocardiography, including global and regional longitudinal strain analysis, is a valuable diagnostic tool in patients with myopericarditis, complementary with not widely available cMRI. Standard medication therapy was administered but the inferolateral ST elevation myocardial infarction (STEMI) occurred only a few hours after the admission. Coronary angiography was performed, and circumflex artery (ACx) occlusion verified. Initially, thromboaspiration was done, the lesion was predilated with a semi-compliant (SC) balloon and drug eluting stent (DES) was placed. The control angiogram showed a filling defect at the occlusion site, while the expanded stent was visualized in the left main (it did not adequately detach during inflation and was pulled retrograde to the guide catheter). We attempted to remove the stent by passing an SC balloon over the stent and it was pulled under fluoroscopic guidance to the brachial artery. An effort was then made to extract the stent using ˝twisting guide wire technique˝, but without success. Finally, the stent was grasped with an EN Snare® device and removed completely through the sheath. The procedure ended with the repositioning of the guide catheter and implantation of a new stent. It affects almost one third of patients over the period of 5 years, and more than 50% after 10 years following HTx and remains a common cause of late graft failure and mortality. Percutaneous coronary intervention (PCI) can be attempted for focal disease preferably with drug-eluting stents, but the only definite solution is re-transplantation reserved for selected patients with severe CAV. Some interventional tools can be helpful but can also result in new complications. This is a case report on an unsuccessfull attempt of resolving an acute complication after coronary artery bifurcation stenting with two-stent technique, despite all tricks we had in our pocket at that time. Blood flow through such arteries is disturbed and turbulent, which, with the activation of endothelium, leads to chronic thrombosis in the blood vessel wall. Percutaneous coronary interventions in ectatic / aneurysmal vessels carry a high risk of complications, primarily a \"no-reflow\" phenomenon. No-reflow is common in patients with acute coronary syndrome, especially ST elevation myocardial infarction (STEMI). In this article, we present the occurrence of the no-reflow phenomenon in a stable patient undergoing percutaneous intervention due to a significant stenosis of the aneurysmally altered right coronary artery. Despite the rapid placement and optimization of stents and the applied drug therapy combined with thromboaspiration periprocedurally, it was not possible to establish TIMI 3 flow. However, after the initial failure and development of periprocedural STEMI, the patient was successfully stabilized with unexpectedly good recovery of infarcted myocardial function.
