{"title":"[Clinical application of orbital fat autograft for tear trough deformity through tranconjunctival approach].","authors":"Zhibing Ma, Gang Chen, Jinlong Huang, Jinming Wang, Wenting Huang","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To discuss the orbital fat autograft for lower baggy eyelid with tear trough deformity and its therapeutic effect through tranconjunctival approach.</p><p><strong>Methods: </strong>The patients with lower eyelid bag and tear trough deformity were included into the study. The included 32 patients met the following criterias: ①less than 40 years old;②lower eyelid skin was not relaxed;③less than 1 s in Snap test; ④ Hirmand type Ⅰ and Ⅱ. They were treated with tranconjunctival lower blepharoplasty combined with the tears ligament relaxation. The orbital fat was repositioned on the depression site above orbital periosteum to correct tear trough deformity. The effect was evaluated through Tear Trough Rating Scale (TTRS) and satisfaction degree.</p><p><strong>Results: </strong>Three cases with Hirmand type Ⅱ didn't have good effect. Among them, the baggy eyelid was corrected in 2 cases, while the tear trough deformity need additional hyaluronic acid injection. One patient underwent secondary lower blepharoplasty through cutaneous incision. Good effect was achieved in the other 29 cases with no depression deformity and eyelid withdrawal. The TTRS scores 6 months after operation was significantly improved(P < 0.01).Satisfaction degree was 100% (14/14) and 83.3% (15/18) in type Ⅰ and Ⅱ patients.</p><p><strong>Conclusions: </strong>The technique can simultaneously improve lower eyelid bag and the tear trough deformity. {"title":"[Thinning of the free muscle flaps for the treatment of hand and foot defects].","authors":"Jianwu Chen, Baoqiang Song, Chen Chen, Dongliang Zhang, Na Wang, Xianjie Ma, Shuzhong Guo","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To investigate the feasibility of one-stage thinning of latissimus dorsi muscle, rectus abdominis muscle and gracilis flap in reconstruction of the hand and foot defects.</p><p><strong>Methods: </strong>From June 2009 to April 2015,24 free muscle flaps were thinned during transfer operation by removing their superficial muscle layers on the basis of their vessel pedicles running in the deep surface of flaps. The surface of the muscle flaps were covered by split-thickness skin grafts. There were 16 latissimus dorsi muscle flaps,4 gracilis flaps and 4 rectus abdominis muscle flaps. Flap size ranged from 6 cm × 4 cm to 20 cm × 12 cm. Thinned muscle flaps were used to resurface 7 hand defects and 17 foot defects.</p><p><strong>Results: </strong>All muscle flaps survived the thinning procedures without any circulation problems except for one case which suffered total flap necrosis due to venous thrombosis. Skins grafts on muscle flaps also took well. One patient experienced partial skin loss. Two patients underwent secondary debridement and thinning procedure for infection and bulkiness. During the 0.3-20 months follow-up, the contour of thinned muscle flaps matched well with the recipient areas. Reconstructed feet are able to wear regular shoes.</p><p><strong>Conclusions: </strong>Intraoperative immediate thinning of free muscle flaps can be safely accomplished during the primary reconstruction procedure. The mucosa tube was used to reconstruct distal urethra. Postoperative pressure dressing and earlier urination were recommended</p><p><strong>Results: </strong>From May 2007 to October 2015,16 cases with distal urethra defect or stenosis were treated with this method. The urethra defect was 2-4 cm in length. Urethral fistula happened in 3 patients. All flaps survived completely.Surgical incisions and wounds at donor sites and recipient sites healed primarily. All cases were followed up for 6-18 months (10 months on average) and cosmetic results were satisfactory without visible scar.</p><p><strong>Conclusions: </strong>The method of the superficial temporal artery fiontal branch flap combined with the retrograde retroauricular artery flap for the repair of preauricular a large skin defect is simple with less and inconspicious auxiliary incision. Lentiviral vector-mediated overexpression and knockdown of PRX-2 gene in epidermal stem cells were performed respectively,with empty vector-mediated epidermal stem cells as a control group. Overexpression blank control and know down group's PRX-2 expressions in gene and protein levels were detected using RT-PCR and Western blot technology. The tumor size ranged from 0.3 cm × 1.0 cm to 0.5 cm× 1.0 cm, and the defects size ranged from0.5 cm× 1.2 cmto0.8 cm×1.2 cm.The kite flaps were 0.6 cm × 1.2 cm to 1.0 cm × 1.5 cm.</p><p><strong>Results: </strong>All the 12 flaps survived completely with primary healing and no complication. The scar was inconspicious,which was hidden into brows. The brows showed good symmetric cosmetic effect.</p><p><strong>Conclusions: </strong>The kite flap has reliable blood supply and survival rate, which does not affect the brows symmetry. {"title":"[Thinning of the deep inferior epigastric artery perforator flap based on the vascular structure of the arterial perforator].","authors":"Bairong Fang, Yang Sun, Xiang Xiong, Jiyong He, Xiancheng WAng","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To explore flap thinning based on the study of the arterial structure and blood perfusion of the deep inferior epigastric artery perforator (DIEP) flap using computed tomography (CT) angiography.</p><p><strong>Methods: </strong>Clinical imaging study: Preoperative CT angiography was performed in 15 patients with DIEP flap reconstruction to investigate the vascular structure of arterial perforator. Cadaveric imaging study:10 abdominal specimens harvested from fresh cadavers were cannulated with trocar and injected with contrast medium in the deep inferior epigastric artery perforator. During the perfusion of the contrast medium in the flap,the flap was scanned by three-dimensional CT. The CT data was then sent to CT workstation and the images were processed and reformatted to study the vascular structure of arterial perforators and the blood perfusion.</p><p><strong>Results: </strong>75 artery perforators in clinical study and 40 artery perforators in cadaveric study were chosen and analyzed. The major deep inferior epigastric artery perforators run directly across the deep layer of adipose tissue without bifurcating beneath the Scarpa's fascia. Above the Scarpa's fascia, the artery perforators bifurcate and ultimately terminate in the subdermal vascular plexus. Blood perfusion mode: The subdermal vascular plexus served as the only pathway for blood perfusion between perforasomes. {"title":"[The orbicularis muscle partial resection combined with orbital fat flap or orbicularis muscle flap graft for treatment of crow's feet in women].","authors":"Zhaowei Chen, Tong Liu, Jingxiang Shao, Tong Zhu, Huasheng Liu, Huanli Wang","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To observe the effect of the outer orbicularis muscle partial resection combined with orbital fat flap or orbicularis muscle flap transfer to block orbicularis muscle for improvement of crow's feet.</p><p><strong>Methods: </strong>From October 2010 to May 2015,82 cases of females (aged from 27 to 54 years old) with crow's feet were treated with new method(n =41) and traditional method(control, n =41).The new method included partial resection and orbital fat flap or orbicularis muscle flap transfer to block orbicularis muscle. The traditional method was performed to lift the skin and orbicularis oculi muscle to improve the crow's feet. The operation time, the postoperative recovery time, eyes closing function and hematoma, nerve injury and other complications were recorded.1,3,6 and 12 months after operation, the effect was assessed by one cosmetic surgeon who is unkown the groups. Completely correction was assed as 5 points and no improvement or even worse as 0 point.12 months after operation, the patients satisfactory rate was recorded.</p><p><strong>Results: </strong>75 cases were followed up, including 39 in new method group and 36 in control group. The operative time and postoperative recovery time were (50 ± 5) min,(58 ± 4) min and (5 ±1) d,(6.0±1.5) d in the new method group and control group respectively (P<0.05).The two groups both had good eyes closing function, no eyes closing fatigue or difficult or other serious complications such as hematoma, nerve injury. Visual analog scale were 3.2 ± 0.6,3.5 ± 0.5,4.2 ± 0.8,4.3 ±0.8 and 3.1 ±0.6,3.4 ±0.9,3.0 ±0.6,2.8 ±0.5 in the new method group and control group at 1,3,6,12 months after surgery respectively. Obviously, the effect in new method group was much better than that in control group after 6 and 12 months with statistical significance (P < 0.05).12 months after surgery, the satisfaction surveys showed satisfactory in 33 cases; fairly satisfactory in 4 cases; dissatisfactory in 2 cases in new method group, and satisfactory in 10 cases; fairly satisfactory in 15 cases; dissatisfactory in 11 cases in the control group, showing significant difference between the 2 groups (P <0.05).</p><p><strong>Conclusions: </strong>The outer orbital orbicularis muscle partial resection combined with orbital fat flap or orbicularis muscle flap is reasonable and reliable to correct crow's feet with short recovery and less complication. Mimic operation was performed on another fresh specimens perfused with red latex.</p><p><strong>Results: </strong>There were three perforators originated from the deep palmar arch, which passed through the 2nd-4th dorsal interossei and then divided into an ascending branch and a descending branch at the dorsum of hand. Then the ascending branch anastomosed with the dorsal carpal artery, and the descending branch stretched to the 2nd-4th dorsal metacarpal arteries.
