[The application of iTRAQ quantitative proteomics in familial keloid]. iTRAQ定量蛋白质组学在家族性瘢痕疙瘩中的应用

{"title":"[The application of iTRAQ quantitative proteomics in familial keloid].","authors":"","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To identify the special biomarkers and the differentially expressed proteins in keloid tissue and to explore the pathogenesis characteristics of familial keloid by comparing the protein expression differences among familial keloid(FK),sporadic keloid (SK),hypertrophy scar (HS),normal scar (NS).</p><p><strong>Methods: </strong>The tissue specimens of FK,SK,HS and NS(6 specimens in each group),were digested, taged and analysed using quantitative proteomic isobaric tags for relative and absolute quantitation (iTRAQ) labeling technology. A difference greater than 1.2 folds and P < 0.05 were selection criteria for differencial expression proteins. Then bioinformatics analysis was applied. The expressions of 2 differential proteins were validated using Western Blot analysis.</p><p><strong>Results: </strong>A total of 2 450 differentially expressed proteins were identified. Compared with NS,250 up-regulated and 281 down-regulated proteins were identified in FK;281 up-regulated and 232 down-regulated proteins were identified in SK;199 up-regulated and 233 down-regulated proteins were identified in HS. Compared with SK,64 up-regulated and 164 down regulated proteins were identified in FK;79 up-regulated and 169 down-regulated proteins were identified in HS. Compared with HS,124 up-regulated and 115 down-regulated proteins were identified in FK. These different proteins were concentrated in several vital signal pathways such as the extracellular matrix protein, cell adhesion and biological metabolism pathways. The variation trend of 2 differentially expressed proteins (P3H1 and RCN-3) were validated by Western Blot.</p><p><strong>Conclusions: </strong>Proteomic analysis can identify the proteins with variance of keloid, hypertrophy scar and normal skin. {"title":"[Frontal and scalp expanded skin flap combined with laser hair removal for children congenital facial giant nevi].","authors":"Binbin Li,&nbsp;Changyong Chen,&nbsp;Wenzhi Li,&nbsp;Zhi Sun","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To investigate the feasibility and therapeutic effect of frontal and scalp expanded skin flap combined with laser hair removal for children congenital facial giant nevi.</p><p><strong>Methods: </strong>From January 2013 to December 2015,6 cases with congenital facial giant nevi were treated with expanded frontal and scalp skin flap combined with laser hair removal in the department of plastic surgery, An Zhen Hospital, Capital Medical University. The process of treatment could be divided into three stages. Stage 1:The expander capacity was determined according to the size of lesion. Expander was implanted under the galea aponeurotica in front and scalp, and expanded regularly postoperatively. Stage 2:The expander was removed and facial giant nevi was excised. The expanded skin flap was designed according to the shape and size of nevi. Stage 3:After the expanded skin flap healed completely, laser hair removal was performed for 4-6 times with 6 weeks of interval time.</p><p><strong>Results: </strong>300 ml to 350 ml expanders were selected with expanding frequency of 2 times a week. The expansion time lasted for 16 to 20 weeks(average,18.7 weeks).The complications such as leakage, angle and skin flap blood supply obstacle didnt occurr in the process of expansion. The size of frontal and scalp skin flap ranged from 12 cm × 11 cm to 20 cm × 17 cm. Venous drainage disorder happened in one flap and was treated by partial suture removal and acupuncturebleeding treatment. Skin flap necrosis didn't occurred. The defects at donor sites were sutured directly with inconspicious scar. The effect of semiconductor laser hair removal was good without recurrence during oneyear follow-up period. All expanded skin flaps had no obvious contraction with good match of color and texture.</p><p><strong>Conclusions: </strong>This method provides a new selection of donor site for larger facial defect with reliable effect. The facial giant nevi could be excised completely and repaired in one operation with short operation time. We harvest the three-dimensional reconstructive data of the contralateral sixth, seventh, eighth and ninth costal cartilage with VR technique.Three-dimensional solid models were 3D printed with nylon material according to the data exported in STL format.Preoperativesimulation was performed on the models, accordingly, we determined the strategies of costal cartilage harvest and framework fabrication, and operations were performed based on the pre-designed plan.</p><p><strong>Results: </strong>In all 6 patients, the actual costal cartilage harvest and framework fabrication process was consistent with the preoperative design and simulation results, and more scientific than before.The shapes of reconstructed ears were vivid and natural. No complications such as infection,absorption,distortion and chest deformity happened.</p><p><strong>Conclusions: </strong>Through costal cartilage VR and 3D printing technique, we could make more reasonable preoperative design and simulation. The split big toe nail was harvested according to the defects size without bone exposure at donor site.</p><p><strong>Results: </strong>Three grafted nail beds were necrotic completely and reconstructed with flaps. The 2 grafted nail bed was partial loss and healed after dressing. All the other grafted nail bed survived completely with primarily healing both in donor sites and recipient sites. The patients were followed up for 6-30 months(average,14 months).The therapeutic effect was graded as excellent in 8 cases, good in 2 cases and poor in 14 cases(good rate,76.9%).The nail matrix was excellent in 8 cases,good in 1 case and poor in 1 case(good rate,90%).The toe nails in donor sites grew well.</p><p><strong>Conclusions: </strong>The single fingernail defect with intact nail matrix can be reconstructed by split toe nail bed graft with good cosmetic and functional effect. There is no malfunction at donor site. All patients were diagnosed as LM after ultrasound, CT or MR scan. All patients were divided into Group 1 (427 patients, treated by endoscopic LM partial resection combined with cautery and postoperative intratumoral negative pressure and absolute ethyl alcohol),Group 2 (239 patients, treated by combined pinyangmycin and dexamethasone injection),Group 3 (330 patients, treated by surgical resection only).The clinical effects were observed in three groups, and therapeutic effect differences in gender, age, maximum diameter, location, range, histological typing, lymph property and treatments were analysed in cervicofacial LM.</p><p><strong>Results: </strong>Group 1:333 patients were cured (78.0%),Group 2:165 patients were cured (69.0%),Group 3:238 patients were cured (72.1%).The difference in cure rate between Group 1 and Group 2 or between Group 1 and Group 3 was significant(P <0.05).The number of patients with ≥ 2 treatments in Group 3 was significantly less than that in other two groups. The cure rate of LM in cervicofacial area was significantly lower than that in other parts of body (P < 0.05).In cervicofacial LM patients, the therapeutic effect differences in maximum diameter, range, histological typing, lymph property were statistically significant (P < 0.05).At the multivariable logistic regression analysis, LM range as well as histological typing were independent factors influencing the therapeutic effect (P < 0.05).</p><p><strong>Conclusions: </strong>Both treatment of endoscopic LM partial resection combined with cautery and postoperative intratumoral negative pressure and absolute ethyl alcohol, and surgical resection have good therapeutic effect on LM in infants. To further improve the anatomic knowledge of the premasseter space and to determine its application in rhytidectomy.</p><p><strong>Methods: </strong>The anatomy of the premasseter region was investigated in 12 fresh cadaver (24 hemi-faces) dissections;810 patients of SMAS rhytidectomy procedures were observed.</p><p><strong>Results: </strong>①There were several soft tissue spaces superficial to the masseter muscle, and the boundaries were strengthened by retaining ligaments. Adjacent spaces were separated by numerous weak fiber membranes connecting the SMAS and masseter fascia.② The space was further divided into two parts; one part was located under the platysma and another part was under the aponeurosis of SMAS. The former part was easier to identify and dissect. ③ The relaxation, expansion, and sliding of the space is an important reason in the face sags with aging, which is referred to as the layered ptosis theory.</p><p><strong>Conclusions: </strong>The soft tissue space superficial to the masseter muscle have two different characteristics. The different nature of the two parts of the space can easily mislead dissection to the wrong layer, which is the important anatomical factors for the risk of facial nerve damage in rhytidectomy. The layered ptosis theory seem to better explain the process of aging. Then repeated freeze-thaw,enzymatic digestion,organic solvent extraction and vacuum freeze-drying were performed.Adult adipose tissue extracellular tissue matrix was obtained. The traits of extracellular matrix scaffold were observed.HE staining, Masson staining and DAPI fluorescence staining were used to test the effectiveness of the decellularization.Immunohistochemistry was used to detect the reservations of extracellular matrix (Ⅳ collagen, laminin).Scanning electron microscopy was introduced to observe the ultrastructure of extracellular matrix scaffold, and universal mechanical testing machine was used to measure the mechanical properties of the scaffolds. Enzyme digestion method was used to extract human adipose-derived stem cells (hADSCs),and then the 3rd passage hADSCs were cocultured with extracellular matrix scaffold.CCK8 was introduced to assay cell proliferation activity,and scanning electron microscopy was used to observe cellular adhesion. The cells on the scaffold were induced to adipocytes and observed by freezing section and Oil Red O staining after 14 days.</p><p><strong>Results: </strong>The extracellular matrix scaffold of adipose tissue was porous sponges architecture. The cells in tissue were relatively removed. The collagen and laminin were preserved relatively, and the mechanical properties did not decline too much. {"title":"[Analysis of clinical diagnosis and treatment in patients with microtia in Klippel-Feil syndrome].","authors":"Jinxiu Yang,&nbsp;Leren He,&nbsp;Haiyue Jiang,&nbsp;Yongzhen Wang,&nbsp;Ye Zhang","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To analyze the clinical features and diagnostic and therapeutic procedure of microtia in Klippel-Feil syndrome(KFS),and to summarize the experiences on diagnosis and treatment of this kind of rare disease to avoid misdiagnosing. .</p><p><strong>Methods: </strong>Between May 2014 and July 2015,six patients with microtia were diagnosed with suspected cervical vertebral malformation through physical examination and X-ray. Then they underwent additional examinations to evaluate the degree of deformities and relative risks:pure tone test, chest CT,cervical spine CT,temporal CT, echocardiography and ultrasonic on kidney and ureters. Ear reconstruction was performed with soft tissue skin expander and autogenous rib cartilage framework.</p><p><strong>Results: </strong>The six patients were diagnosed as KFS with microtia, which had different degree of cervical fusion and thoracic vertebral fusion. Some of them had rib deformity, scoliosis, congenital renal malformation and so on. Of 6 patients, scar formation occurred in 1 case after ear reconstruction, whose new ears had good position and appearance at 1 month after stage Ⅱ. After operation,6 cases were followed up for 8-20 months (median,12 months),none of them had nerve injury. Five cases had completed the third stage. All of them were well-healed after one month. Three cases were followed up for 3-11 months, the reconstructed ears had a three-dimensional configuration, and the cranioauricular angle of the reconstructed ears were similar to the opposite ears</p><p><strong>Conclusions: </strong>The primary step of comprehensive therapy in microtia with KFS is to diagnose definitely. Enhancing perioperative management can reduce surgery-related risks. These 57 eases included tongue carcinoma at lingual margin (n =36),at ventral tongue (n =15) and at mouth floor (n=6). .</p><p><strong>Results: </strong>The size of AMT perforator flap ranged from 5.5 cm ×4.0 cm to 7.5 cm × 5.5 cm, the thickness of flap ranged from 1.0-1.7 cm. The length of pedicle from descending branch of lateral circumferential femoral artery is (8.2 ± 0.6) cm, found in 28 cases. The length of pedicle from profunda femoral artery is (8.5 ±0.4) cm, found in 17 cases. The length of pedicle from femoral artery is (8.1 ± 0.7) cm, found in 12 cases. All 57 perforator flaps survived uneventfully, the defects at donor site were closed directly in all cases. All patients were followed up for 12-24 months with satisfactory esthetic and functional results in reconstructed tongue. No local recurrence happened. The posterior craniotomy was performed from vertex, biparietally to a point above the occipital protuberance. Two distraction devices were fixed in the parasagittal,collinear position. After a latency of 3 days, the device was activated at 0.5 mm/day. After the distraction, the consolidation period was about 6 months.</p><p><strong>Results: </strong>The average distraction distance was 27.3 mm (range,25 to 30 mm).Cerebrospinal fluid leak happened in one patient. After taken the 3D CT scan, all of them were undertaken the second operation of removing the distraction devices.
